On June 19, 2021, Little Rock police attempted to stop a vehicle on Interstate 40 near the Morgan and Maumelle exit. The driver refused to stop, and police escalated a chase at high speed. As the vehicle approached Exit 142, the driver lost control and crashed off the roadway. The driver was killed in the crash. Brendan Charlet, 19, was reportedly acting erratically at the Ash Flat Livestock Auction. A Highland officer later attempted a traffic stop, but Charlet did not stop. Officers from multiple agencies escalated a pursuit into Hardy. At the intersection of Highways 62 and 63, Charlet crashed into a Hardy police vehicle, sending both into a ditch. As officers approached, they heard a gunshot and found Charlet with a self-inflicted gunshot wound. He was pronounced dead shortly after. Charlet reportedly had outstanding charges in Louisiana and Alabama.

On April 13, 2021, officers with the Tontitown Police Department attempted to stop a stolen Toyota 4Runner. The vehicle, driven by an unidentified individual, fled at high speed and failed to negotiate a turn on Bush Creek Road near US 412. The 4Runner crashed into a tree and burst into flames. Chassadee Costales, who was in the vehicle, died early that morning from her injuries. Officers noted one of their own was treated for minor injuries after entering the burning vehicle to assist survivors.

On 03/31/2021, a high-speed pursuit initiated by the North Little Rock Police Department ended in tragedy when officers requested assistance from the Arkansas State Police. The vehicle, traveling westbound on Interstate 40 with its headlights off, reportedly reached speeds exceeding 120 miles per hour before crashing. In March 2021, a Little Rock police officer chased a “stolen SUV” driven by a 12-year-old girl who had taken the vehicle without permission. During the pursuit, a supervisor ordered the officer to disengage, but the chase continued south along Chicot Road. The vehicle lost control, crashed, and overturned, killing 14-year-old passenger Zayne Ortiz and injuring two others.
The Little Rock Police Department did not disclose the fatal pursuit until more than a year later, prompting outrage from Ortiz’s family and city officials. Public records revealed that the pursuing officer violated pursuit policy by ignoring orders to end the chase. The officer resigned following an internal investigation, and another officer involved was suspended. The supervising officer or the pursuing officer’s names have not been publicly released.
After public pressure and local media coverage, the department revised its pursuit policy and adopted stricter public disclosure requirements. The case became a flashpoint for accountability in Little Rock’s police oversight and remains cited in discussions of transparency reforms.

On March 8, 2021, Dallas County deputies attempted a traffic stop that escalated into a pursuit involving 43-year-old Joel Weldon of Hot Spring County. Weldon reportedly stopped his truck and fired a weapon at deputies before fleeing again. When the pursuit ended along Highway 8 near Sparkman, Weldon exited the vehicle armed and refused to comply with commands. Deputies opened fire, killing him at the scene. The Arkansas State Police launched an investigation, and findings were referred to the Dallas County Prosecuting Attorney to determine whether the use of deadly force was lawful.

On February 20, 2021, officers from the Lonoke Police Department recognized a vehicle linked to a North Carolina child-abduction investigation parked at a McDonald’s in Lonoke, Arkansas. The driver, 38-year-old William Robert Ice of Pennsylvania, refused to stop, and police escalated a chase south on Arkansas Highway 31. The vehicle became disabled in a snowbank on Raprich Road. Ice exited with a firearm and opened fire on officers. Police returned fire. Ice was found inside the vehicle critically wounded from an apparent self-inflicted gunshot and later died at a Little Rock hospital. The Arkansas State Police is conducting the investigation. A 14-year-old girl in the vehicle was rescued unharmed.
