On 12/20/2021, Will County deputies searched for Alexander Wheeler after reports that he shot a family member and drove away. On 07/27/2021, Alexis C. Wilson was shot and killed by officers with the Dolton Police Department in Dolton, Illinois. On 07/14/2021, off-duty Chicago Police Department officer Michael Leverett was driving his pickup truck in the West Rogers Park neighborhood of Chicago when he failed to stop at a stop sign and struck 9-year-old Hershel Weinberger, who was riding his bicycle in a crosswalk. On 04/10/2021, Winnebago County sheriff’s deputies shot and killed 27 year old Faustin Guetigo outside and inside a residence on Bellwort Drive in Rockford. Deputies were following up on a domestic disturbance reported earlier in the day and located Guetigo outside the home. Deputies attempted to make contact. Guetigo went inside the residence and remained in the basement. Deputies entered the home and approached him. During the encounter, deputies opened fire and killed Guetigo. A deputy was injured during the incident and later released from the hospital. The Winnebago County State’s Attorney reviewed the case and ruled the shooting justified. No officers were charged.

On 04/09/2021, Stephenson County sheriff’s deputies shot and killed 52 year old David William Determan during a standoff at Citizens State Bank in Lena. Deputies responded around 9:14 a.m. after Determan entered the bank holding a firearm. After a deputy arrived on scene, Determan fired a round into the air outside the bank and remained in conversation with officers while holding the weapon. During the encounter, Determan lifted the firearm in the direction of deputies. Deputies opened fire and killed him at the scene. Authorities closed the downtown area during the response.

On 04/08/2021, 20 year old Zaydin J. Dudra was killed in a head-on crash on Illinois Route 48 near East 1550 North Road outside Taylorville. Dudra was a passenger in a vehicle traveling on the highway when an off-duty United States Capitol Police investigator crossed the center line and struck the vehicle head on. Dudra suffered fatal injuries and died as a result of the collision. The off-duty officer was arrested. Reporting states the off-duty Capitol Police investigator was under the influence of alcohol and later pleaded guilty to aggravated DUI. In August 2023, a Christian County judge sentenced Hodson to 18 years in the Illinois Department of Corrections, to be served at 85%, followed by 18 months of mandatory supervised release. He began serving that sentence immediately and is eligible for release around 2038.

During the encounter, police restrained Wallace, and he later died. Authorities did not publicly release details describing the specific actions that led to his death. On 04/01/2021, police responded to a welfare check at a residence on North Street in Brighton after James Andrew Iler made a call to authorities that prompted concern for his safety. Multiple deputies from the Macoupin County Sheriff’s Office and a Brighton police officer were present. Officers remained at the residence for an extended period speaking with Iler. During the encounter, police stated that Iler was holding a knife and moved toward a deputy. A deputy opened fire, and Iler was shot and killed at the scene. The incident was investigated by state authorities.

On 04/01/2021, Rock Island police identified DeShawn Latiwon Tatum in the area of 11th Street and began following him. Police said Tatum continued driving to a nearby gas station, where officers reported that he entered a vehicle occupied by a bystander. As officers attempted to remove him from the vehicle, police stated that the car began moving in reverse, resulting in officers being dragged. Police opened fire, and Tatum was shot and killed at the scene.

The Chicago Police Department attempted a traffic stop on Alvarez on Laramie Avenue at about 12:18 a.m.; when Alvarez refused to stop, officers escalated a foot pursuit. Alvarez fled through a parking lot and alley, holding a gun in his right hand and a cellphone in his left, then stumbled in the front yard of a residence near 5200 W. Eddy Street. Officer Solano confronted him, ordered him to drop the gun and fired five shots, shooting Alvarez in his back and thigh. Alvarez died at the hospital. Alvarez final words are "Why are you shooting me?" to which Solano replied "You had a gun." The Cook County State’s Attorney’s Office reviewed the case and found insufficient evidence to charge the officers.

On March 29, 2021, Chicago Police Officer Eric Stillman shot and killed 13-year-old Adam Toledo in an alley near the 2300 block of South Sawyer Avenue in the Little Village neighborhood. Officers had responded to reports of gunfire when Toledo fled on foot. Body camera footage later showed Toledo discarding a handgun behind a fence and turning toward the officer with his hands raised. Stillman fired a single shot that struck him in the chest. Adam was a seventh grader who loved video games and was close to his family. On 03/25/2021, a Calumet City police officer stopped Alvin Burrage for a traffic violation involving a stop sign and broken taillights. After Burrage exited the vehicle, police said he reached back inside the car and retrieved a handgun. According to the officer, Burrage began raising the gun in the officer’s direction. The officer fired, striking Burrage. Burrage was transported to a hospital, where he died more than a week later from his injuries. The Cook County State’s Attorney’s Office reviewed the killing and cleared the officer. No criminal charges were filed.

On 03/25/2021, a security guard at a Home Depot store in the Brighton Park neighborhood reported that 18 year old Travon Chadwell was shoplifting. During a physical struggle inside the store, Chadwell shot the security guard, critically injuring him. Chadwell ran from the store. Chicago police officers pursued him, and during the chase one officer was shot. Officers continued the pursuit into the neighborhood. During an exchange of gunfire with police, officers shot and killed Chadwell. The killing was reviewed by investigators, and no criminal charges were filed against the officers.

On 03/24/2021, Forest Park police responded to a call reporting that a man had entered a Jimmy John’s restaurant and waved a gun in the air. Officers located Tony Smith lying on a sidewalk nearby. As officers approached and made physical contact with Smith, police said he turned toward an officer and raised a gun toward the officer’s face. Officers fired their weapons, killing Smith at the scene. The Cook County State’s Attorney’s Office reviewed the killing and ruled the use of deadly force justified. On 02/02/2021, East Peoria police responded around 12:50 a.m. to a reported domestic violence incident elsewhere in the city in which a woman and a girl were injured. Joshua Crites left before officers arrived. Police located his vehicle a short time later in the CVS parking lot at 2540 E. Washington St. During the encounter, Crites stabbed Officer Jeffrey Bieber multiple times. Police opened fire and killed Crites at the scene. Illinois State Police took over the investigation, which was reported as ongoing at the time.
