The Chicago Police Department attempted a traffic stop on Alvarez on Laramie Avenue at about 12:18 a.m.; when Alvarez refused to stop, officers escalated a foot pursuit. Alvarez fled through a parking lot and alley, holding a gun in his right hand and a cellphone in his left, then stumbled in the front yard of a residence near 5200 W. Eddy Street. Officer Solano confronted him, ordered him to drop the gun and fired five shots, shooting Alvarez in his back and thigh. Alvarez died at the hospital. Alvarez final words are "Why are you shooting me?" to which Solano replied "You had a gun." The Cook County State’s Attorney’s Office reviewed the case and found insufficient evidence to charge the officers.

On March 29, 2021, Chicago Police Officer Eric Stillman shot and killed 13-year-old Adam Toledo in an alley near the 2300 block of South Sawyer Avenue in the Little Village neighborhood. Officers had responded to reports of gunfire when Toledo fled on foot. Body camera footage later showed Toledo discarding a handgun behind a fence and turning toward the officer with his hands raised. Stillman fired a single shot that struck him in the chest. Adam was a seventh grader who loved video games and was close to his family. His death sparked large protests throughout Chicago after the video’s release on April 15, 2021, and renewed debate over police use of force against youth.

In February 2021, 43-year-old Lakisel Thomas was struck and killed while crossing the street to pick up lunch for her son when a fleeing vehicle ran a red light and hit her. Chicago Police had attempted a traffic stop for an improperly displayed registration. The driver, Isaac Wade, 20, was chased by police at 80 mph and ran multiple lights before striking another vehicle and hitting Thomas in the 7400 block of South Racine Avenue. She died shortly after at St. Bernard Hospital. The pursuit violated department policy, and in January 2025, the city proposed a $4.5 million settlement to her estate.

On 01/22/2021, deputies from the Alexander County Sheriff's Office initiated a high-speed pursuit of a 2011 Ford Fusion for a traffic violation in East Cape, Illinois. The vehicle, occupied by four individuals from Missouri, fled and eventually crashed into a house at 328 Oak Street in Mounds, Illinois, killing the driver, Marquan Reed. The other passengers sustained varying injuries and were transported to local hospitals.
