Officer(s) : John Westerfield

Cause of Death : Vehicle

Event : 2 bystanders killed by police chase crash

OPEN RECORDS SUMMARY :

On August 30, 2024, at approximately 5:45 p.m., Officer Logan Westerfield of the Plainfield Police Department responded to a report of a person down at a Long John Silver's restaurant in the 2300 block of East Main Street. Upon arrival, Officer Westerfield encountered 38-year-old Bryan Goodmon, who was reportedly impaired and sitting in a vehicle. As the officer approached, Goodmon fled in a white Kia, nearly striking the officer.
.
Officer Westerfield initiated a pursuit with lights and sirens activated. The chase proceeded onto U.S. 40, where the vehicle crossed the intersection at Smith Road. As Officer Westerfield entered the intersection against a red light, his police cruiser collided with a gray Ford Fusion occupied by Bennie Joe Williams, 79, and his wife, Barbara Williams, 78, both of Clayton, Indiana. The couple had a green arrow and were turning onto U.S. 40 when the collision occurred.
.
Both Bennie and Barbara Williams sustained fatal injuries and were pronounced deceased at the scene. Officer Westerfield was transported to a local hospital with minor injuries and was later released.
.
The Avon Police Department conducted an investigation, revealing that Officer Westerfield was traveling at approximately 100 mph seconds before the crash and failed to exercise "due regard" when proceeding through the intersection. The report noted that emergency vehicles can outrun their sirens at speeds over 55 mph, likely preventing the Williamses from hearing the approaching cruiser.
.
Following the investigation, Officer Westerfield resigned from the Plainfield Police Department on October 18, 2024.
.
As of the latest available information, no criminal charges have been filed against Officer Westerfield in connection with the incident.

City : Fort Wayne

County : Allen

State : Indiana

Agency : Fort Wayne Police Department

Officer(s) : Officer Jason Fuhrman

Cause of Death : Gunshot

Event : Killed by police during apprehension for community corrections violation

OPEN RECORDS SUMMARY :

On July 30, 2024, Fort Wayne Police Department (FWPD) officers were requested by DeKalb County Community Corrections to locate 31-year-old Gawon Kaerey Benson after he failed to return to custody following a community release. Officers located Benson on the Purdue University Fort Wayne (PFW) campus. Body camera footage shows officers repeatedly instructing Benson to drop an object in his hand, later identified as a firearm. Benson reportedly manipulated the slide of the handgun. Officer Jason Fuhrman then fired his weapon, fatally wounding Benson.
.
While officers suggested the incident might constitute "suicide by cop," the basis for this claim stems from a note reportedly found in Benson’s pocket. Benson's family disputes this claim, arguing there is no conclusive evidence from the released body camera footage.
.
Benson’s family also asserts that he had no history of violence and that his initial incarceration was due solely to unpaid traffic violations. They have questioned the necessity of lethal force and criticized the limited and heavily edited body camera footage released by FWPD, stating that it fails to provide sufficient context.
.
Investigation Status:
As of December 6, 2024, the case remains under investigation by multiple agencies, including the Allen County Prosecutor's Office. Officer Jason Fuhrman has been placed on paid administrative leave, per department policy, pending the investigation’s outcome.

Ward v. Tinsley et al. (2011): Allegations of excessive force; evidence was excluded at trial.

Martin v. Wentz et al. (2013): Claims of excessive force during arrest; court allowed claims to proceed.

Mudd v. City of Fort Wayne et al. (2016): Alleged excessive force; summary judgment favored the defendants.
.
Current Status:

As of December 6, 2024, Officer Fuhrman remains on administrative leave and has not been cleared in the fatal shooting of Gawon Benson. The investigation is ongoing, with no determinations made regarding his actions.

Location : Tillotson and McGalliard

City : Muncie

County : Henry

State : Indiana

Agency : Henry County Sheriff’s Office

Officer(s) : Tanner Strelecky

Cause of Death : Vehicle

Event : Passenger killed by police chase crash

OPEN RECORDS SUMMARY :

On May 15, 2024, at approximately 6:00 a.m., Henry County Sheriff's Deputy Tanner Strelecky initiated a traffic stop on State Road 3 near County Road 650 N in Henry County, Indiana, alleging the driver of a silver 2013 Dodge Dart, later identified as 28-year-old Zacery Antrim of Van Buren, Indiana, was "suspected of impairment." Antrim did not stop, leading Deputy Strelecky to escalate the situation into a high-speed pursuit.
.
The chase extended approximately 20 miles into Delaware County, reaching speeds between 70 and 100 mph through Muncie's south side, downtown, and northwest side, before turning east on McGalliard Road. At the intersection of McGalliard Road and Tillotson Avenue, the pursuit continued through a red light, resulting in a significant collision with a white Jeep.
.
Nicholus (Nick) Wayne Woolums, a 25-year-old passenger in Antrim's vehicle, was pronounced dead at the crash site. Both Antrim and the Jeep's driver sustained injuries and were transported to a local hospital.
.
About Nicholus Wayne Woolums:
Born on December 3, 1998, in Muncie, Indiana, Nick was remembered as outgoing, kind, and caring. He was a devoted Indianapolis Colts fan and cherished time with his family, especially his father, whom he considered his best friend. Nick was known for his selflessness, always ensuring others were taken care of before himself.
.
Officer Background:
Rookie Deputy Tanner Strelecky was hired in July 2023 without prior law enforcement experience and had not graduated from the Indiana Law Enforcement Academy at the time of the incident. He had completed 588 hours of departmental and field training but initiated the pursuit without apparent supervisory oversight. Background of Deputy Tanner Strelecky.
.
Overview:
Deputy Tanner Strelecky was hired by the Henry County Sheriff’s Office in July 2023 without prior law enforcement experience. Indiana law requires new officers to complete a 40-hour pre-basic training course before being hired. After joining the force, Strelecky underwent 588 hours of departmental and field training, concluding in October 2023. At the time of the April 2024 incident, he had not yet attended the Indiana Law Enforcement Academy for comprehensive training.

In April 2024, Deputy Strelecky handled the pursuit leading to the death of 25-year-old Nicholus Wayne Woolums. The incident occurred at approximately 6:00 a.m., when Strelecky had just completed his shift but was still in uniform and driving his assigned vehicle. It is still unclear whether he was considered to be on duty at the time he attempted to make a stop based on a still unsubstantiated DUI allegation.
.
Lawsuit:
In September 2024, a civil rights lawsuit was filed against Deputy Strelecky, Henry County Sheriff John Sproles, the Henry County Sheriff’s Department, and Henry County. The case, titled Woolums v. Henry County Sheriff's Department et al, was filed in the U.S. District Court for the Southern District of Indiana. The plaintiff, James David Woolums, is seeking damages related to the fatal incident.
.
Ongoing Investigations:
The Indiana State Police are investigating the circumstances surrounding the pursuit and crash. The Delaware County Prosecutor's Office is reviewing the findings to determine whether charges will be filed against pursuit driver Antrim or pursuer Deputy Strelecky. Notably, no charges have been filed against Antrim to date, suggesting potential inability to proceed due to resulting scrutiny of the deputy's actions.
