On 07/27/2021, the Indianapolis Metropolitan Police Department initiated a pursuit after officers ran the plates of a vehicle at W. Troy Ave and Westbrook Ave and discovered the registered owner had active warrants for theft and controlled-substance possession. The driver refused to stop and led officers on a high-speed chase reaching up to approximately 100 mph before losing control and colliding with a tractor-trailer at the intersection of W. Raymond St. and S. Harding St. The collision resulted in the death of a female passenger, not involved in the chase, and left the driver critically injured.

On July 26, 2021, Indianapolis Police Department officers responded to a report of a stolen vehicle around 11 p.m. Upon arrival, a man fled in the vehicle, leading police on a pursuit reaching speeds of 60 mph. During the chase, the driver struck a median, lost control of the vehicle, and was ejected as the vehicle rolled down an embankment. The vehicle landed on the man, resulting in his death.

The vehicle reached speeds of approximately 60 mph during the chase. On July 7, 2021, at 8:57 a.m., an Indiana State Police trooper stopped a silver 2012 Ford Focus on I-94 eastbound at the 40-mile marker for several alleged traffic offenses, including following too closely and speeding in a construction zone with workers present. During the stop, the trooper discovered that the driver, Vernell F. Muhammad, had active warrants through La Porte County for aggravated assault. As the trooper attempted to place Muhammad into custody, he drove away. Police escalated a chase. Muhammad evaded stop sticks and continued driving until the 18.5-mile marker, where his vehicle rear-ended a semi-truck. Muhammad was ejected from the vehicle and killed.

Around noon on June 28, 2021, Avon Police responded to investigate a reported theft. Witnesses informed officers that the people had left and provided a detailed description of the vehicle and direction of travel. Officers located both parked in the driveway of a residence nearby. While officers were working to confirm their identities, Justin D. Murray drove the car through the front yard of the house and sped away. The vehicle traveled approximately 800 feet before crashing into a large tree. Murray was killed, and a female passenger sustained minor injuries.

On June 19, 2021, around 1:20 a.m., Gregory Hoskins disregarded a Hobart Police officer's attempt to pull over his gray SUV for running a stop sign. The officer activated emergency lights and sirens, but Hoskins sped westbound on 39th Avenue, ignoring several additional stop signs. The officer pursued Hoskins, who continued to disregard traffic signals, including a red light at 39th Avenue and Broadway in Gary, Indiana. As Hoskins entered the intersection, his SUV was struck on the driver's side by another vehicle traveling north on Broadway, resulting in his death.

On June 15, 2021, around 9:20 p.m., officers from the White County Sheriff's Office, Monon Police Department, and Wolcott Police Department attempted to serve an arrest warrant for Perry E. Boyd at a residence located at 402 W. Anderson Street in Wolcott, Indiana. Upon arrival, Boyd entered the home, and when police followed, an exchange of gunfire occurred between Boyd and the officers. Boyd was shot and killed during the confrontation, and a Wolcott town deputy marshal was wounded.

On June 8, 2021, around 3:30 a.m., a white Chevy Impala, driven by Jason A. Mitchell, crashed into a blue Nissan sedan on U.S. 41 near 101st Street in St. John, Indiana. Following the initial collision, the Chevy Impala struck a Lake County Sheriff's patrol car that was en route to assist Dyer police. The impact resulted in Mitchell's death.

On May 14, 2021, around 6:30 p.m., officers from the Indianapolis Metropolitan Police Department responded to an apartment complex in the 7800 block of Red Mill Drive following a report of an armed, suicidal person. Monolito Ford had allegedly fired at least one shot into the air before police arrived. Three officers encountered Ford, who was reportedly armed and walking toward them. After officers ordered him to stop, he allegedly pointed his firearm at the officers, prompting them to shoot and kill him.

On May 10, 2021, Deputy James T. Webster, 51, was off duty but driving his Jefferson County Sheriff's Department-issued 2016 Ford Explorer westbound on Clifty Drive in Madison, Indiana. During this time, he struck and killed Hunter Alan Robinson, 22, who was also traveling westbound on Clifty Drive in the roadway on a skateboard.

On May 1, 2021, around 6 p.m., the Porter County Sheriff's Office responded to a 911 call regarding a disturbance at a residence located at 716 Long Run Road in South Haven, Indiana. After several hours of attempting to communicate with a man inside the home without success, officers entered the residence. Inside a bedroom, they encountered Alexander T. Tuzinski, who was reportedly armed with a handgun. Officers shot and killed Tuzinski during the confrontation.

On April 24, 2021, officers with the Gary Police Department pursued a stolen U-Haul truck following a stolen vehicle alert in the area. The pursuit ended when the U-Haul collided with an SUV at 6th Avenue near Colfax Street, struck a utility pole, and rolled over. The male driver of the U-Haul was pronounced dead at the scene. One person in the SUV was transported to a hospital with non-life-threatening injuries. The crash caused power outages in the area as crews responded to repair downed utility lines. The Gary Police Department is investigating the incident.

On April 23, 2021, around 5:45 a.m., officers responded to a report of a crash with personal injury in the 1000 block of South Shadeland Avenue in Warren Park, Indiana. An off-duty Indianapolis Metropolitan Police Department (IMPD) officer was involved in the incident. The officer was traveling south on Shadeland Avenue when a pickup truck, heading north, reportedly crossed the centerline and collided with the police vehicle. The truck had two occupants; the driver was killed, and the passenger was transported to a hospital for treatment.

On April 17, 2021, Monroe County deputies escalated a chase and Malik Ali Malik was killed when the vehicle he was driving collided with a tree during the police pursuit. Deputies from the Monroe County Sheriff's Office chased Malik at speeds reaching up to 105 mph before the fatal crash occurred.

On April 14, 2021, a Henry County Sheriff’s Deputy attempted to stop a vehicle for a traffic violation and suspected impaired driving. The driver, identified as Zacery Kane Antrim, fled through Henry and Delaware Counties before crashing into another vehicle at McGalliard Road and Tillotson Avenue in Muncie. The passenger, 25-year-old Nicholas Wayne Wollum, was pronounced dead at the scene. Both Antrim and the other driver, 47-year-old Shanta Sharrel Schmiesing, were transported to a hospital in critical condition. The Indiana State Police are investigating the pursuit and crash.

On March 23, 2021, at approximately 5:51 p.m., multiple 911 callers reported that a man was firing a gun in the 100 block of West 29th Street in Anderson, Indiana. Witnesses stated that Irvin Silas Myers pointed a firearm at an officer, who ordered him to drop the weapon. An exchange of gunfire ensued, and Myers was shot and killed by the officer.

On March 10, 2021, Brown County police alerted other departments about a despondent person who was possibly armed around 11:40 a.m. A Columbus officer spotted the truck and attempted to initiate a traffic stop, but Martin Louis Douglas Jr. refused to stop and fled. Police from several departments pursued Douglas, who allegedly drove erratically and at high speeds. After striking a tire-deflating device, Douglas stopped in the roadway and fired approximately 15 rounds from an AR-15 before fleeing again. Following a second exchange of gunfire, police shot and killed Douglas.

On March 3, 2021, Jonathan Levi Allen, 28, who was wanted on a parole violation, was reportedly shooting at people and buildings from his SUV in Muncie, Indiana. After a police chase involving the Muncie Police Department, Allen sustained a gunshot wound and was transported to a hospital, where he later died. On February 26, 2021, around 1 a.m., the Liberty Police Department responded to a 911 call reporting a woman walking on US-27 South in Liberty, Indiana. Officer Lorenzo Shepler arrived and found Maggie Ann Dickerson, 29, standing in the middle of the road. When Officer Shepler approached Dickerson and attempted to move her, she allegedly resisted, leading to a struggle. During the altercation, Dickerson reportedly pulled out a knife and advanced toward the officer, who then shot and killed her.

On February 5, 2021, Muncie Police Department officers responded to a home in the 1600 block of North Riley Road in Muncie, Indiana, following reports of a domestic dispute. A woman who had left the house with a child and another family member informed officers that a man inside the home was loading a rifle and making threats to harm himself. It was also reported that a domestic battery had occurred. Officers contacted the man by phone but soon heard what they believed to be a gunshot from inside the residence. Over the next six hours, several more gunshots were heard. Eventually, police entered the home and discovered the man's body.

On January 31, 2021, Carmel Police Department officers responded to a disturbance at 441 W Main Street in Carmel, Indiana. A person, later identified as Keith Scales, 60, had reportedly threatened suicide and was firing shots from inside a residence shortly before noon. Upon arrival, officers observed gunfire coming from the home's windows and doors and began evacuating nearby residents. During the incident, Scales shot at several vehicles, including a police vehicle. When Scales allegedly raised a high-powered rifle toward the officers, they shot and killed him.

On January 28, 2021, around 5:45 p.m., officers from the Kosciusko County Sheriff's Office and Wabash County Sheriff's Office were serving a search warrant as part of a criminal investigation at 63 East Main Street in Wabash, Indiana. During the operation, police shot and killed Roger D. Hipskind, 37. Specific details regarding the circumstances leading to the shooting were withheld by police.

On January 24, 2021, at approximately 2:20 a.m., Alexandria Police Department Officer Zachary Taylor was on duty, driving a marked police vehicle southbound on State Road 9 in Alexandria, Indiana. During this time, Officer Taylor struck Dylan Kelly Stinson, 22, who was on a skateboard. Stinson sustained fatal injuries from the collision.

On January 17, 2021, Grant County Sheriff's Deputy Matthew Sneed responded to a domestic situation involving a stabbing at a residence in the 3200 block of South Central Avenue in Marion, Indiana. As Deputy Sneed approached the house, Daniel Young, 25, allegedly drove a Honda Civic toward the deputy. In response, Sneed jumped onto the hood of the moving vehicle and shot and killed Young through the windshield of the car.

On January 16, 2021, around 11 p.m., Michigan City police attempted a traffic stop on a vehicle that refused to stop. Police escalated a high-speed pursuit. Deputies from the LaPorte County Sheriff’s Office joined the chase. The vehicle crashed near U.S. 20 and Ohio Street. A male passenger was ejected and died at the scene. Officers detained the driver and another passenger. Investigators reported additional criminal charges against occupants of the fleeing vehicle. Public records available today do not identify the deceased passenger, and no officer names, charges, or discipline have been publicly released.

On January 10, 2021, around 1:40 a.m., Dyer Police Department officers attempted to stop a black Dodge Charger. The driver of the vehicle fled, leading officers on a pursuit. The driver eventually lost control of the vehicle, causing it to leave the roadway and roll over. Following the crash, the vehicle's two occupants fled on foot. Officers apprehended one individual, while the other, Mark Bivins, 28, continued to flee. During the foot chase, Bivins reportedly shot at officers, who then shot and killed him.

On January 1, 2021, a Marshall County deputy attempted to stop a pickup truck driven by 63-year-old Jeffrey L. Marvin near U.S. 30 and Pioneer Road in Plymouth. Police said Marvin refused to stop, and officers escalated a chase that continued through Plymouth into Starke County. During the pursuit, Marvin reportedly rammed three patrol vehicles before driving directly toward officers. Police opened fire, striking him. Marvin was transported to a nearby hospital, where he was pronounced dead.
The Indiana State Police led the investigation and confirmed that Marvin was unarmed at the time of the shooting. No officers were injured, and none were publicly identified. Officials did not announce any charges or discipline following the investigation.
Family members later questioned whether the shooting was necessary, noting that Marvin had a history of medical issues and confusion in the days before the incident. The case drew local attention as the first fatal police shooting of 2021 in northern Indiana.
