On July 6, 2021, at approximately 6:01 a.m., Danville Police Department officers responded to a domestic disturbance at the Super 8 Motel located at 3663 South Danville Bypass in Danville, Kentucky. Upon arrival, officers encountered 33-year-old Shelby Ray Hardin, who was holding his mother hostage at knifepoint. Officer Kyle Lyons opened fire, killing Hardin. The Kentucky State Police conducted the investigation.

Officers with the Glasgow Police Department responded to a domestic violence call. Upon the officers' arrival, Joshua Shane Hartigan fled on a motorcycle. Officers escalated a chase as Hartigan traveled toward Scottsville Road. Hartigan lost control and went off the roadway, crashing and killing himself. The Kentucky State Police conducted the investigation.

On June 30, 2021, U.S. Marshals attempted to serve multiple arrest warrants for 33-year-old Jeremy Andrew Nelson at a residence on Garden Drive in Florence, Kentucky. Upon contact, Nelson allegedly fired a weapon at the officers. A brief standoff ensued, during which Nelson reportedly displayed a firearm again. In response, officers discharged their weapons, fatally wounding Nelson. The incident is under investigation by the Kentucky State Police.

On June 17, 2021, Daniel Ray, a 35-year-old male, was shot and killed by officers during an arrest attempt by officers of the Covington Police Department in the 3000 block of Frazier in Covington, Kentucky, for reasons they have not publicly explained.

On June 16, 2021, Michael D. Mason, a 46-year-old white man, was chased by Lexington Police Department officers for undisclosed reasons. He crashed at the intersection of Eastland Parkway and Gayle Drive in Lexington, Kentucky, and died as a result.

On June 1, 2021, Jonathan Ivar Thompson, a 29-year-old male, was shot and killed by officers from the Independence Police Department and the Kenton County Sheriff's Office during an attempted arrest. The incident occurred in the 800 block of Ridgepoint Drive in Independence, Kentucky. Officers shot and killed him for reasons they have not publicly explained.

On May 30, 2021, Ryan Bernal, a 32-year-old white male, was shot and killed by officers from the Louisville Metro Police Department during an attempted arrest. The incident occurred in the 3600 block of Georgetown Place in Louisville, Kentucky. Officers were investigating a report of a stolen vehicle when Bernal fled on foot. During the pursuit, Bernal brandished a firearm, leading officers to discharge their weapons. Bernal was transported to University of Louisville Hospital, where he succumbed to his injuries on May 31, 2021.

On 05/27/2021, Hardin County sheriff’s deputies and Kentucky State Police responded to a call reporting that Alejandro Clarke Jr. was experiencing a psychotic episode and had assaulted his girlfriend near Nolin Road in Sonora. Officers attempted to take Clarke into custody. During the encounter, police reported physical resistance, followed by Clarke being taken to the ground. Multiple officers used batons, fists, boots, and flashlights, deployed Tasers multiple times, and released a police dog during the arrest. An ambulance was present, but Clarke was placed in the back of a police vehicle. He was later found unresponsive and died shortly afterward. A grand jury declined to bring criminal charges against officers. Clarke’s family filed a civil lawsuit, and reporting later confirmed a settlement was awarded to the family, with additional civil claims reported as pending at the time.

On April 12, 2021, at approximately 1:36 a.m., Angelina Sue Raad, a 38-year-old white female, was driving her vehicle the wrong way on the inner loop of New Circle Road near Harrodsburg Road in Lexington, Kentucky. She collided head-on with a police cruiser driven by Officer Dawn Miller, who was off duty and heading home after her shift. Both drivers were extricated from their vehicles and transported to a local hospital. Raad was pronounced deceased at around 4 a.m., while Officer Miller sustained serious injuries but was reported to be in stable condition.

On April 9, 2021, at approximately 6:57 a.m., Georgetown Police Department officers responded to reports of a man attempting to break into cars in a shopping center parking lot near Connector Road in Georgetown, Kentucky. Upon arrival, officers encountered 31-year-old Deshund Andrea Tanner, who fled on foot and attempted two carjackings during his escape. When officers confronted Tanner during the second carjacking attempt, he produced a firearm. Officers opened fire, killing him. The officers involved—Sgt. Joseph Payton, Officer Cole Centner, Officer Gregg Muravchick, and Officer Christian Squires—were later cleared of any wrongdoing and were honored for their actions during the incident.

On April 4, 2021, Michael Ray Wagner, a 47-year-old white male from Geneva, Indiana, was killed during a police chase in Shelbyville, Kentucky. After reports that Wagner had stopped his vehicle on the roadway and pointed a gun at another motorist, officers located him. He drove eastbound on Frankfort Road (US-60), and police escalated a high-speed chase. Approximately two to three miles into the pursuit, Wagner lost control of his 1999 Pontiac Firebird near the intersection of US-60 and Ski Area Road. He crashed, was ejected, and was pronounced deceased at the scene by the Shelby County Coroner.

On March 29, 2021, at approximately 4:30 p.m., Deputy Justin Conley of the Fleming County Sheriff's Office observed a vehicle on Kentucky Route 11 near Maysville, Kentucky, with a male driver and a female passenger who appeared to be involved in a physical altercation. Deputy Conley attempted to initiate a traffic stop, but the driver, 48-year-old Terry Darnell Jackson, exited the vehicle and fled on foot into a wooded area. During a physical struggle, Deputy Conley opened fire, killing Jackson. Jackson was airlifted to the University of Cincinnati Hospital, where he was pronounced deceased. Deputy Conley was treated for minor injuries at Meadowview Regional Medical Center and was subsequently placed on administrative leave, per department policy.

On February 27, 2021, at approximately 11:00 p.m., a Kentucky State Police trooper conducted a traffic stop in the community of Malone, Kentucky. During the stop, the trooper requested assistance from the Morgan County Sheriff's Department and the West Liberty Police Department. Jerry Lynn Roseberry, a 48-year-old man, exited the vehicle with a firearm and refused commands to drop the weapon. A West Liberty police officer opened fire, striking Roseberry. He was pronounced deceased at the scene by the Morgan County Coroner. The Kentucky State Police Critical Incident Response Team is investigating the shooting.

On February 20, 2021, at approximately 11:00 p.m., Villa Hills Police Department officers Sean Dooley and Jacob Bolton conducted a traffic stop on Interstate 75 near Erlanger, Kentucky, for a speeding violation. Randall Lee Lockaby, a 57-year-old man, exited his vehicle and produced a gun. Both officers opened fire, killing him. The Kentucky State Police investigated and justified the shooting. In February 2022, Lockaby's family filed a wrongful death lawsuit against the Villa Hills Police Department and the involved officers, alleging excessive force and constitutional violations.

On February 14, 2021, at approximately 3:30 a.m., off-duty Corbin Police Department Sgt. Stephen Meadors was contacted at his residence on Dogwood Cove in Corbin, Kentucky, by a neighbor reporting a prowler. Sgt. Meadors went outside and encountered 19-year-old Phillip N. Davenport. An altercation ensued, during which Sgt. Meadors opened fire, killing Davenport. Davenport was transported to Baptist Health Hospital in Corbin, where he was pronounced deceased by the Whitley County Coroner. The Kentucky State Police investigated the incident. In October 2021, a Whitley County grand jury returned a "no true bill," clearing Sgt. Meadors of any wrongdoing.

On February 11, 2021, Brian D. Ellis, a 44-year-old man who was on felony warrants, was located by police in the parking lot of Walmart at 120 Sam Walton Drive in Russellville, Kentucky. Ellis was driving a 2020 Kia, with police claiming that he drove it toward them. Officers opened fire, killing him. Ellis succumbed to his injuries on February 12, 2021, in the hospital.

On 01/30/2021, Bullitt County sheriff’s deputies were called to check on a vehicle parked in the middle of a roadway along Cedar Creek Road. Deputies attempted to get the driver, identified as Eric Kessler, to exit the vehicle. According to authorities, the vehicle then accelerated away. During the encounter, a deputy fired his weapon, striking Kessler. He was later pronounced dead. The shooting was placed under investigation, and no criminal charges against the deputy were announced at the time of reporting.

On January 21, 2021, at approximately 10:20 p.m., Grayson County Sheriff's Office deputies Andy Vincent and Jason Luedke responded to a residence at 504 North Main Street in Caneyville, Kentucky, following a tip regarding the whereabouts of 34-year-old Brian Richard Abbott, who was wanted for absconding post-incarceration supervision. Police entered the home and alleged they encountered him with knives. Deputy Vincent fired and struck Abbott three times. Abbott was pronounced dead in his home by the Grayson County Deputy Coroner. The Kentucky State Police investigated the shooting.

On January 17, 2021, at approximately 1:30 a.m., a Warsaw Police Department officer attempted to stop a vehicle on US-42 in Warsaw, Kentucky, for speeding. The driver, 31-year-old Josh Hodges of Florence, Kentucky, did not stop. Police escalated a chase during which he lost control of the vehicle while navigating a curve and collided with a concrete wall. A passenger, 54-year-old Shanda Marie Withrow of Hamilton, Ohio, was pronounced dead at the crash site. Hodges and another passenger sustained injuries and were sent to a hospital.
