On 06/11/2021, Maryland Transportation Authority Police responded to a call involving a man with a "Be On The Lookout" alert from Howard County Police, related to a domestic incident. The man was located in a vehicle in a parking garage on Fuller Rad in Baltimore. Officers established a perimeter around the vehicle shortly before 11 a.m. Negotiators were called to the scene in an attempt to resolve the situation peacefully. At approximately 6: 40 p.m., officers heard gunfire from inside the car. Upon approaching the vehicle, they found the man deceased, having apparently committed suicide under police duress.

On 05/17/2021, Charles County Sheriff's Office deputies responded after a family member reported that Denzell Nathan Clarke, who was wanted by police, was experiencing a mental health crisis. Upon arrival at the 6300 block of Josephine Road in Waldorf, two deputies were wounded while attempting to serve a warrant. Clarke allegedly shot at the deputies, who then exchanged gunfire. Clarke subsequently locked himself inside the home. When officers entered, they found Clarke dead from gunshot wounds.

On 05/16/2021, Baltimore Police Department officers responded to a call at 1800 block E. Lafayette Avenue in Baltimore at 10: 39 a.m. regarding a family disturbance. Upon arrival, they were informed by someone outside that a man, later identified as Timothy Fleming, was inside with a knifeand holding a woman at knifepoint. Officers entered the home and proceeded to the second floor, where they encountered Fleming, the woman, and another family member. Fleming allegedly grabbed the woman by her hair and made a motion as if to stab her. Two officers then fired their weapons, killing Fleming.

On 05/09/2021, officers from the Baltimore County Police Department and firefighters responded to 911 calls about a fire and an armed man at the 7500 block of Maury Road in Windsor Mill at around 6: 40 a.m. Upon arrival, they found Everton Garfield Brown outside the burning residences. Four police officers opened fire, resulting in Brown's death. Additionally, two other men and a woman were killed during the incident, while another man was treated for non-life-threatening injuries at a hospital.

On 05/02/2021, around 2 a.m., Berlynn Matthews stopped to assist Joshua Day, who had been struck by a car while crossing North Point BoulevaRd in Dundalk. While Matthews was helping Day, off-duty Officer William Collazo-Brown on a motorcycle struck both of them, leading to their deaths. Collazo-Brown was subsequently arrested and charged with DUI-related offenses.

On 04/13/2021, at around 1: 30 p.m., Maryland State Police received two 911 calls regarding a person possibly armed with a gun. The first call provided only a phone number, while the second gave a street address in the 23000 block of Hollywood Road in Leonardtown. Upon arrival, Trooper Joseph Azzari confronted Peyton Ham in the driveway. Azzari shot Ham twice, resulting in his death.

On 04/07/2021, at approximately 5 a.m., officers from the U.S. Pentagon Force Protection Agency responded to a report of shots fired in the parking lot of a condominium complex located at 7333 New Hampshire Avenue in Takoma Park. An off-duty officer, David Hall Dixon, approached them, stating he suspected a car was being burglarized. He engaged with Dominique Williams, 32, and James Lionel Johnson, 38, who did not comply with his commands. Both Williams and Johnson attempted to flee in a vehicle, at which point Dixon shot and killed them. Following the incident, Dixon was charged with second-degree murder.

On 03/21/2021, while officers of the Baltimore Police Department pursued a stolen vehicle, Darius Gore was struck by the fleeing vehicle and severely injured. He was placed into a coma and died less than a month later. Gore’s mother, Rowena Simmons, filed a federal lawsuit against the city and the officers, alleging negligence in the pursuit that led to the crash.

On 03/18/2021, around 11 a.m., two officers from the Anne Arundel County Police Department were dispatched to a residence in the 7800 block of Bodkin View Drive in Pasadena following a call from a mental health facility to check on a man reported to be suicidal. Upon arrival, the officers entered the home through an unlocked door and encountered Leonard Popa holding a handgun. One of the officers then shot and killed Popa.

On 02/25/2021, around 9: 22 p.m., officers from the Baltimore Police Department's Inner Harbor unit were flagged down in the area of Market and Pratt streets following a non-fatal shooting. One officer provided aid to the victim, while others pursued a man who fled into the Harbor Park Garage. On the ramp into the garage, the man, identified as Benjamin D. Tyson, allegedly turned around, produced a handgun, and attempted to fire it. Multiple officers then shot and killed Tyson.

On 02/06/2021, around 8 a.m., Montgomery County police received multiple 911 calls reporting a man driving erratically in a Volkswagen sedan on Olney Laytonsville Road and Fieldcrest Road in Gaithersburg. Before Deputy Frank Pruitt arrived, the driver had already struck two vehicles. Upon arrival, Pruitt observed Kevin Costlow approaching two individuals with a large stick. A confrontation ensued where Costlow struck Pruitt with the stick. Pruitt attempted to use his Taser on Costlow, but it was ineffective. Subsequently, Deputy Pruitt shot and killed Costlow.

On 02/04/2021, officers were searching for Dontae Green after he allegedly shot at officers at a grocery store in North Baltimore due to a dispute over his paycheck. Green was wanted on 19 charges, including attempted murder, armed robbery, and handgun violations. While police were clearing a residence associated with Green in the 1400 block of N. Mount Street, he allegedly fired at them from inside a closet. Officers returned fire, resulting in Green's death. During the incident, one deputy U.S. Marshal was seriously injured.
