On July 24, 2021, 22-year-old Daniel Milton was arrested at Staten Island University Hospital in Ocean Breeze after allegedly grabbing the buttocks of a 22-year-old female hospital employee. Police were called and took Milton into custody.

On 07/08/2021, New York City police officers on patrol in the Bushwick neighborhood of Brooklyn responded to the sound of gunfire outside a liquor store near Wilson Avenue. Officers located Steven Leconte and chased him on foot. According to reporting, officers stated that Leconte was holding a firearm and did not put it down during the encounter. Officers engaged Leconte and opened fire, killing him at the scene. Leconte was 53 years old. The New York Attorney General later reviewed the shooting and found it justified. No criminal charges were filed against the officers involved.

On July 8, 2021, at around 10:30 p.m., officers from the City of New York Police Department responded to reports of gunshots near Halsey Street and Wilson Avenue in the Bushwick area of Brooklyn. Upon arrival, the officers approached a group of people outside a liquor store and discovered three individuals with gunshot wounds.
The officers were informed that an armed man had fled on foot, heading westbound on Wilson Avenue. They pursued him and activated their body-worn cameras. The officers proceeded halfway down the block and located a man crouched on the sidewalk, holding a firearm in his left hand.

The officers ordered the man to drop the weapon. When he allegedly failed to comply and raised the firearm toward the officers, they opened fire, killing him.

On June 12, 2021, officers from the Town of Wallkill Police Department responded to a call from Christopher Van Kleeck’s mother who requested help for her son, who was exhibiting signs of a mental health crisis. The Orange County Crisis Call Center relayed that Van Kleeck had threatened to harm responding officers. When Officer Daniel Graham arrived at the home in Middletown around 3:35 p.m., he observed Van Kleeck chasing a family member on the lawn while holding two knives. Van Kleeck ran toward the patrol car with the knives raised. Officer Graham fired his service weapon through the patrol car windshield three times, striking Van Kleeck. Responding officers and emergency medical services then rendered aid and transported Van Kleeck to a hospital, where he was pronounced dead. The New York State Office of the Attorney General investigated and concluded that criminal charges were not warranted based on the evidence and applicable law.

On June 4, 2021, police officers attempted to arrest 29-year-old Timothy Flowers in connection with a triple shooting that occurred in Sumner Park in May 2021. During the initial shooting, Flowers allegedly fired 14 shots into a vehicle at close range, injuring three people, one of whom was left paralyzed.

On 05/24/2021, Marcelo Pelaez was crossing Woodhaven Boulevard at Rockaway Boulevard in a marked crosswalk in Queens when he was struck by a vehicle driven by an off-duty New York City police officer. On May 20, 2021, at around 6:00 a.m., Buffalo Police responded to a report of two people shot on Layer Avenue. The victims, 27-year-old Megan Wisniewski and 63-year-old Margie Ramos, were killed in the shooting. Later, at approximately 11:00 a.m., police located the shooter, 36-year-old Rafael Perez, and engaged in a standoff with him. Around noon, police reported that Perez was found dead from a self-inflicted gunshot wound, ending the standoff.

Police responded to a 911 call reporting a person lying in the back seat of a maroon sedan parked on Bauer Avenue and Frontier Trail. Suffolk County Police Officer James Skidmore arrived on the location and approached the vehicle. Upon engaging with Jesse Bonsignore, the officer claimed that Bonsignore exited the car holding a knife and made threats against him. Officer Skidmore physically grabbed Bonsignore, during which Bonsignore reportedly tried to take the officer’s firearm. Officer Skidmore then shot and killed Bonsignore.

Officers responded to a ShotSpotter activation on Lyell Avenue around 4:21 a.m. Surveillance cameras captured a vehicle leaving the location. The vehicle was later spotted on Glasser Street, where officers attempted a traffic stop. During the stop, officers claimed that Mark D. Gaskill was holding a handgun. Two Rochester police officers then shot and killed Gaskill.

On April 28, 2021, New York State Police identified Barry K. Stewart, 55, concerning a double homicide at BridgeView Real Estate Services in Watertown, New York. A State Police K9 unit spotted Stewart's vehicle, a gray 2020 Ford F-150 pickup truck, on Route 11B in Dickinson, Franklin County. When troopers attempted a traffic stop, Stewart's vehicle left the roadway. Troopers found Stewart inside the vehicle with a self-inflicted gunshot wound; he was pronounced dead at a nearby hospital.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. On 04/05/2021, New York City police shot and killed 55 year old Abdou Jallow during an evening police encounter in the Bronx. Police were conducting a traffic stop near East 166th Street and Findlay Avenue when officers reported hearing gunshots nearby. Police pursued two men they said were fleeing the area. During the pursuit, police opened fire. Jallow, who was not involved in the police encounter, was struck by police gunfire and fatally wounded. Witnesses and family members said Jallow was a neighborhood resident who had been returning home after praying at a local mosque when he was shot. Police stated the shooting occurred amid an exchange of gunfire, and the incident was placed under investigation.

Gabriel Casso, 21, was running toward New York City Police officers while being pursued by two unidentified men near East 166th Street and Findlay Avenue in the Bronx on April 3, 2021. Police opened fire during the encounter, resulting in Casso being fatally shot. On March 10, 2021, Rochester Police Department officers responded to a report of a man with a knife at the Open Door Mission on 210 W Main Street in Rochester, New York, around 3 a.m. Tyshon Jones, 29, had reportedly taken a knife from the Samaritan House and walked away. When officers confronted him, Jones was reportedly cutting himself with the knife and refused commands to drop it. Officers stated that Jones threatened to kill them and ignored attempts to subdue him with a Taser. As he advanced toward officers, one officer fired, fatally shooting him.

On February 11, 2021, Alexis Leigh Davis-Gooden, 35, was initially stopped by a Menands Police Department officer on Broadway in Menands, New York. After providing her information, the officer returned to his patrol car to run checks through state and federal systems. Before the officer could complete the stop, Davis-Gooden fled the scene in her vehicle, heading toward Watervliet. Shortly thereafter, she lost control of the vehicle and crashed into the front gate of the Watervliet Arsenal. Davis-Gooden was pronounced dead at the scene.

On January 28, 2021, at approximately 3:49 p.m., police received a report of an armed robbery in Hudson Falls, New York. Officers identified a vehicle matching the man's description and attempted a traffic stop. The driver, later identified as Sheldon R. Streicher, 38, drove away. Hudson Falls Police, Washington County sheriff's deputies, and New York State Police pursued Streicher through Hudson Falls, Kingsbury, and into Fort Ann. The vehicle came to a stop on Clements Road near NY-149. As officers approached the vehicle, Streicher reportedly shot and killed himself inside.

On 01/21/2021, New York City police officers were stopped at a traffic light in the Wakefield section of the Bronx when they heard a disturbance and calls for help from nearby. Officers exited their vehicle and observed a woman running along the street, followed by David McFarlane, who police said was holding a knife. Officers ordered him to drop the knife. According to police, McFarlane turned toward the officers while still holding the knife, and officers opened fire. He was pronounced dead at the scene. The shooting was placed under investigation. Reporting later indicated McFarlane had a history of mental health issues, according to family members.

On January 3, 2021, at approximately 12:30 p.m., Suffolk County Police officers were pursuing Malcolm Stewart, 29, who was allegedly driving a 2014 Mercedes-Benz in Medford, New York. Stewart ran a red light during the chase, causing his vehicle to collide with a 2021 Chevrolet driven by James Palumbo, 56, at the intersection of County Road 101 and Station Road. The crash resulted in Palumbo’s death.
