Officers responded to a ShotSpotter activation on Lyell Avenue around 4:21 a.m. Surveillance cameras captured a vehicle leaving the location. The vehicle was later spotted on Glasser Street, where officers attempted a traffic stop. During the stop, officers claimed that Mark D. Gaskill was holding a handgun. Two Rochester police officers then shot and killed Gaskill.

Troopers were dispatched to locate Philip Stanley Watros, who was reportedly armed and making homicidal and suicidal threats to an estranged girlfriend. Officers located him and his vehicle at Mexico Community Park. Due to the nature of the threats and concerns for public safety, officers established a perimeter around the area. After a 12-hour standoff, Watros reportedly shot and killed himself with a rifle.

On April 28, 2021, Robert A. Sanders, a 48-year-old man from Lakewood, New Jersey, was involved in a fatal incident following a police pursuit in Saratoga County, New York. The chase began when U.S. Park Service rangers approached Sanders' vehicle, which was reported as suspicious at the Saratoga National Historical Park. Sanders fled the scene, leading to a pursuit that involved multiple law enforcement agencies, including the Stillwater Police Department. The chase continued through several towns before Sanders' vehicle crashed into a guardrail near Route 9 and Goldfoot Road in the Town of Malta, where he was found deceased.

On April 28, 2021, New York State Police identified Barry K. Stewart, 55, concerning a double homicide at BridgeView Real Estate Services in Watertown, New York. A State Police K9 unit spotted Stewart's vehicle, a gray 2020 Ford F-150 pickup truck, on Route 11B in Dickinson, Franklin County. When troopers attempted a traffic stop, Stewart's vehicle left the roadway. Troopers found Stewart inside the vehicle with a self-inflicted gunshot wound; he was pronounced dead at a nearby hospital.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. Despite officers' commands to drop the weapon, Green reportedly cut his girlfriend's leg and swung the knife toward the child. In response, Sergeant Pajerski fired two shots, striking Green. Green was transported to Albany Medical Center, where he was pronounced dead.

Gabriel Casso, 21, was running toward New York City Police officers while being pursued by two unidentified men near East 166th Street and Findlay Avenue in the Bronx on April 3, 2021. Police opened fire during the encounter, resulting in Casso being fatally shot. It remains unclear whether the bullets that killed Casso or injured a nearby bystander were fired by police or the men they engaged. The bystander sustained injuries and was transported to a local hospital for treatment.

On March 14, 2021, Yonkers Police Department officers responded to a welfare check for Peter Kastsaridis, 50, at 11 Livingston Street in Yonkers, New York, around 10:45 a.m. Upon arrival, Kastsaridis fired at officers from inside the residence. Police returned fire, prompting Kastsaridis to barricade himself, resulting in an hours-long standoff. At approximately 6:40 p.m., officers entered the residence and discovered Kastsaridis deceased on the floor with a semiautomatic rifle nearby. The investigation concluded that he died from a self-inflicted gunshot wound.

On March 10, 2021, Rochester Police Department officers responded to a report of a man with a knife at the Open Door Mission on 210 W Main Street in Rochester, New York, around 3 a.m. Tyshon Jones, 29, had reportedly taken a knife from the Samaritan House and walked away. When officers confronted him, Jones was reportedly cutting himself with the knife and refused commands to drop it. Officers stated that Jones threatened to kill them and ignored attempts to subdue him with a Taser. As he advanced toward officers, one officer fired, fatally shooting him.

On February 11, 2021, Alexis Leigh Davis-Gooden, 35, was initially stopped by a Menands Police Department officer on Broadway in Menands, New York. After providing her information, the officer returned to his patrol car to run checks through state and federal systems. Before the officer could complete the stop, Davis-Gooden fled the scene in her vehicle, heading toward Watervliet. Shortly thereafter, she lost control of the vehicle and crashed into the front gate of the Watervliet Arsenal. Davis-Gooden was pronounced dead at the scene.

On March 1, 2025, 22-year-old Messiah Nantwi died at Mid-State Correctional Facility in Marcy, New York, following multiple assaults by correctional officers. An emergency response team, including Officers Jonah Levi and Caleb Blair, responded to an incident in Nantwi's cell. Despite Nantwi having his hands raised and being unarmed, officers began beating him with fists, boots, and batons. The beating continued as he was moved to the infirmary, including additional beatings in a stairwell and a holding cell. Nantwi died from massive head trauma and 79 other injuries. Ten officers have been indicted in April of 2025, with charges ranging from second-degree murder to conspiracy and evidence tampering. The incident occurred during a statewide correctional officers' strike, with National Guard troops deployed to maintain prison operations.

On January 28, 2021, at approximately 3:49 p.m., police received a report of an armed robbery in Hudson Falls, New York. Officers identified a vehicle matching the man's description and attempted a traffic stop. The driver, later identified as Sheldon R. Streicher, 38, drove away. Hudson Falls Police, Washington County sheriff's deputies, and New York State Police pursued Streicher through Hudson Falls, Kingsbury, and into Fort Ann. The vehicle came to a stop on Clements Road near NY-149. As officers approached the vehicle, Streicher reportedly shot and killed himself inside.

On December 22, 2020, Trooper Christopher Baldner stopped an SUV driven by Tristan Goods for speeding on the New York State Thruway in Ulster County. During the traffic stop, Baldner allegedly sprayed pepper spray into the vehicle, prompting Goods to drive away. Police escalated a chase on the Thruway at high speed. Baldner rammed the family’s SUV twice with his patrol vehicle, causing it to overturn several times. Eleven-year-old passenger Monica Goods was ejected from the vehicle and killed. Her parents and sister were injured. Baldner was later charged with second-degree murder, manslaughter, and reckless endangerment for his actions during the pursuit.

On January 3, 2021, at approximately 12:30 p.m., Suffolk County Police officers were pursuing Malcolm Stewart, 29, who was allegedly driving a 2014 Mercedes-Benz in Medford, New York. Stewart ran a red light during the chase, causing his vehicle to collide with a 2021 Chevrolet driven by James Palumbo, 56, at the intersection of County Road 101 and Station Road. The crash resulted in Palumbo’s death.
