On December 5, 2021, Lawton Police Department officers responded to a 911 call reporting that Quadry Sanders was inside a residence in violation of a protective order and allegedly waving a firearm. Upon arrival, officers established a perimeter and attempted to communicate with Sanders. Sanders exited the residence and was ordered to show his hands. He initially complied but then moved behind a refrigerator. Officer Robert Hinkle fired four shots, striking Sanders. After falling, Sanders sat up with his hands raised, at which point Hinkle fired seven additional shots, and Officer Nathan Ronan fired four times. Sanders was unarmed at the time of the shooting and died from his injuries.

On November 6, 2021, at approximately 2:40 a.m., Officer Caleb Sherman of the Broken Arrow Police Department attempted to stop a vehicle for a traffic violation near the 2900 block of E Kenosha Street. Upon stopping, two occupants fled on foot, while the remaining occupant, identified as 31-year-old Corey Gene Batt, opened fire, striking Officer Sherman in the leg. Officer Sherman returned fire as Batt fled into a nearby wooded area. Officer Sherman was transported to a local hospital, treated, and released. Approximately two hours later, officers located Batt deceased in the wooded area near the traffic stop. The investigation suggests that Batt died from a self-inflicted gunshot wound.

On November 4, 2021, Tulsa Police officers responded to a QuikTrip convenience store at 3050 S. Sheridan Road after receiving a call from the mother of Derek Collins, 36, reporting that he was suicidal and armed with a knife. Upon arrival, officers found Collins inside the store with self-inflicted cuts. Officers convinced him to exit the store into the parking lot, where he grabbed a passing cyclist and held a knife to the cyclist's throat. A Tulsa Police officer opened fire, fatally striking Collins.

On October 25, 2021, Oklahoma City Police attempted to stop a vehicle for a traffic violation near I-44 and SW 104th Street. The driver, identified as 16-year-old Milton Neal, fled. Police escalated a high-speed pursuit. Police allegedly called off the chase immediately before Neal ran a red light at the intersection of SW 104th Street and S Pennsylvania Avenue, colliding with bystander Sean Donahue's vehicle. Donahue, 35, was driving a company vehicle for Diagnostic Laboratory of Oklahoma at the time and was killed instantly. Neal and his two passengers sustained non-life-threatening injuries and were transported to a local hospital.

On October 23, 2021, Tulsa Police Department officers responded to a 911 call from Leon Perry, who was incoherent during the call and allowed a child to speak, indicating they needed help. Upon arrival at the Seminole Hills apartment complex, officers encountered a 7-year-old boy with bound wrists and neck, who informed them of an argument and assault on his mother. Officers then heard a gunshot from the apartment. As they entered, Perry pushed the lifeless, bound body of Tyanna Blades down the stairs. During the ensuing confrontation, officers shot and killed Perry and rescued a 4-year-old girl who was also bound.

On October 22, 2021, at approximately 3:39 a.m., Oklahoma City Police officers responded to a reported robbery involving two men fleeing in a red Saturn. An officer spotted the vehicle minutes later and initiated a traffic stop. The vehicle initially pulled over in a parking lot but then sped away eastbound on a westbound service road. The vehicle entered the westbound lanes of I-240, driving against traffic. The pursuing officer paralleled the vehicle by driving eastbound in the eastbound lanes to maintain visual contact. The man vehicle collided head-on with another vehicle, resulting in the death of the driver, identified as Melvin Eugene Simpson, 51. His passenger sustained injuries from the crash.

On October 13, 2021, a Geary Police officer saw a 46-year-old man holding a knife near the I-40 and US-281 interchange in Hinton, Oklahoma. An off-duty officer driving past the scene stopped to assist, and a third officer arrived soon after. The third officer attempted to use a taser. Police allege that the man lunged at the off-duty officer with the knife when he saw the Taser. One of the officers opened fire, striking the man and fatally wounding him.

On 10/08/2021, around midnight, Tulsa Police officers saw Tommy Carmichael, 46, driving a white truck in West Tulsa. Carmichael was a person of interest in the death of his mother, Carol Longacre, who had been found dead in her bathtub earlier that day. Officers followed Carmichael as he drove toward the home where Longacre’s body was discovered.
Carmichael arrived at the house near South Utica Avenue and East 61st Street and tried to enter the property. Before officers could apprehend him, Carmichael shot himself and was pronounced dead at the home.

On September 11, 2021, around 3:30 a.m., Lawton Police officers attempted to stop a black Chevrolet Camaro for alleged reckless driving. The driver, 21-year-old D'monte L. Harris, did not stop. Police escalated a high-speed chase that continued until Harris lost control of the vehicle at the intersection of SW Sheridan Road and SW Jefferson Avenue, resulting in a fatal crash.

On August 28, 2021, Fort Smith Police responded to reports of unauthorized individuals tampering with vehicles at a construction site on Old Greenwood Road. Upon arrival, officers discovered a damaged and abandoned Ford van and evidence of multiple construction vehicles being hot-wired. A water truck had been stolen and later found crashed through a fence at a nearby business, where additional vehicles had been tampered with. A box truck stolen from the business was tracked to an area near South E Street in Fort Smith. When officers attempted to engage the man, he drove away. Police escalated a chase into Arkoma, Oklahoma. In Arkoma, police attempted to stop the box truck using patrol vehicles. The driver rammed the police vehicles, nearly driving over a small police sedan. Officers then discharged their firearms, striking the man. He was pronounced dead at the scene. A female passenger in the truck was unharmed and taken into custody. Another woman, allegedly connected to the incident, was arrested after arriving at the scene at high speed. Due to the man being Native American, the FBI assumed jurisdiction over the investigation, in accordance with federal guidelines.

On July 27, 2021, Chickasha Police responded to a 911 call reporting a suicidal woman with a firearm at a residence in the 1100 block of West Tennessee Avenue. Upon arrival, officers encountered 49-year-old Lisa Hight outside the home with a gun. Despite repeated commands to drop the weapon, Hight retreated into the residence, firing multiple shots from inside. At one point, she allegedly pointed the gun at officers, prompting an officer to discharge their weapon. Subsequently, officers heard another gunshot from within the house. Upon entry, they found Hight deceased.

On 07/22/2021, around 4:05 p.m., the Duncan Police Department responded to a 911 call reporting a domestic disturbance at a residence in the 300 block of West Maple Avenue. Officers encountered 43-year-old Shawn Daniel Tillison, who emerged from the residence with a firearm pointed into the air. Officers fired less-beanbags in an attempt to subdue him; however, Tillison allegedly pointed the firearm at the officers. They shot and killed him.

On June 30, 2021, Emmitt Weaver died following a foot pursuit involving Tulsa Police Department officers. Officers were patrolling near 31st Street and Memorial Drive when they approached a group of men. Weaver fled briefly before stopping and showing signs of respiratory distress. He was briefly handcuffed but officers removed the cuffs so he could use his inhaler. Weaver requested medical help and collapsed. He was transported to a hospital where he was pronounced dead. The cause of death has not been publicly disclosed.

On June 7, 2021, around 11:45 a.m., a two-vehicle collision occurred near the intersection of East 220 Road and South 500 Road, northwest of Afton, Oklahoma. The crash involved a 2016 Ford Explorer driven by 48-year-old Douglas "Chad" Goodner, an off-duty reserve officer with the Collinsville Police Department, and a large farming vehicle. Goodner and his passenger, 59-year-old William McWhirt, were both pronounced dead at the scene due to massive injuries. Another passenger in the Explorer sustained serious injuries and was transported to a nearby hospital. The driver of the farming vehicle was not injured.

On May 30, 2021, around 2:45 a.m., Pushmataha County Sheriff's deputies conducted a traffic stop on 48-year-old Hank Miller near Coffee Creek Road in Antlers, Oklahoma, citing an obstructed license plate.

On 04/11/2021, Chouteau Police officers initiated a traffic stop on a truck allegedly involved in a larceny incident at approximately 4 a.m. The driver did not stop. Police escalated a multi-jurisdictional pursuit through Wagoner, Pryor, and Okay. During the chase, someone in the vehicle reportedly fired shots at police. Police engaged in a shootout. Police claim that they lost site of the vehicle during which time it crashed along one 38–1 64 N. 6 mile Road in Fort Gibson. The driver was ejected from the vehicle and pronounced dead at the scene.

Nathaly Medina, 19, was a passenger in a car that Tulsa police attempted to stop for reckless driving. The driver, Jose Lara-Garcia, did not stop, leading officers to initiate a pursuit. The chase continued until the vehicle crashed into several poles before entering the Arkansas River. Medina died in the crash. The incident is under investigation by the Tulsa Police Department.

On March 27, 2021, at the Oklahoma County Jail located at 201 North Shartel Avenue in Oklahoma City, inmate Curtis Montrell Williams, aged 34, took a detention officer hostage on the 10th floor. Williams held a makeshift weapon to the officer's throat, leading to a standoff. Oklahoma City Police Department officers, including Lt. Coy Gilbert and Officer Kevin Kuhlman, responded to the situation. Upon entering the pod, they observed Williams threatening the officer's life. In response, both officers discharged their firearms, fatally striking Williams. The detention officer was rescued and treated for injuries sustained during the incident.

On March 6, 2021, at approximately 2:30 a.m., Valley Brook Police attempted to stop a vehicle on Southeast 59th Street and Shields Boulevard. The driverdid not stop. Police escalated a chase that ended when he crossed shields Boulevard and collided with two other vehicles.

The crash resulted in the death of Derrick Dewayne Alexander, as well as injuries to others. A passenger from one of the other vehicles was transported to a local hospital in critical condition and later died.Lujan was remembered as a kindhearted individual who had taken custody of his 12-year-old brother three years prior.

On February 28, 2021, Joseph Dugan, a 17-year-old white male, and Farrah Rauch, also 17, both juniors at Blair High School in Nebraska, were involved in an attempted carjacking in Muskogee, Oklahoma. After failing to steal one vehicle, they successfully stole another. A Muskogee Police Department officer spotted the stolen vehicle and attempted a traffic stop, but the driver refused to pull over. Police escalated the situation into a pursuit, which ended in a crash.
Following the crash, Dugan and Rauch fled the scene on foot. During the foot chase, police engaged Rauch in a shootout . He was fatally shot by police. Dugan continued to flee into a wooded area, where he reportedly shot and killed himself as officers closed in. The Muskogee Police Department is investigating the incident.

US marshals and the Oklahoma Highway Patrol attempted to arrest Michael Richard Stambaugh, 39, who locked himself inside a residence on Houser Street. U.S. Marshal's Office and Oklahoma Highway Patrol, responded to the scene. Stambaugh exited the home holding a firearm. Officers opened fire, killing him.

Around 11 p.m. on February 4, 2021, Oklahoma City police responded to a 911 call reporting a domestic disturbance. Officers were informed that the individual involved, later identified as 30-year-old Tilford "TJ" Barton, was armed with a firearm and had discharged it earlier that night. Upon arrival, officers encountered Barton hiding behind a tree. He produced a pistol and fired at Sergeant Gerald Ritter III, who returned fire, striking Barton. Barton fell to the ground and died at the scene. Sergeant Ritter was not injured during the incident.

On January 31, 2021, at approximately 5:20 p.m., the McCurtain County Sheriff's Office responded to a domestic assault report involving Franklin Gray, a 39-year-old male. Gray was allegedly restraining his mother on a couch, causing her breathing difficulties. When deputies arrived at the residence in the 1700 block of Lavender Road in Broken Bow, Oklahoma, they encountered Gray outside the home and attempted to arrest him. During the altercation, Gray reportedly tried to take a deputy's firearm, resulting in the weapon discharging. Deputies deployed a taser in an effort to subdue Gray but were unsuccessful. The confrontation escalated, and at least one deputy opened fire, fatally shooting Gray.
The Oklahoma State Bureau of Investigation (OSBI) is investigating the incident. The findings will be reviewed by the District Attorney's Office to determine the appropriateness of the deputies' actions.

On January 17, 2021, at approximately 3:30 a.m., Lawton police officers conducted a compliance check at the Lavish Lounge, where 24-year-old Zonterius "Zon" Johnson was celebrating his birthday. During the check, shots were fired from outside the club, with at least one bullet striking the building. Officers observed Johnson exchanging gunfire with an unidentified individual. Johnson fled the scene on foot. Officer Nathan Ronan pursued Johnson into a dark alley. Johnson allegedly produced a firearm. Officer Ronan opened fire, striking Johnson. Johnson was later pronounced dead at the hospital.
The Oklahoma State Bureau of Investigation (OSBI) conducted an investigation into the incident. The Comanche County District Attorney's Office reviewed the findings and determined that Officer Ronan's actions were justified. Body camera footage from the incident was released to the public.

On January 13, 2021, at approximately 3:30 a.m., Oklahoma Highway Patrol (OHP) troopers and Oklahoma City Police Department officers escalated a chase of a stolen Lincoln Navigator up to 115 mph. The vehicle was driven by 17-year-old Vinnie Hamlet, the twin brother of Chris Hamlet, with Chris and a 17-year-old female passenger. Troopers used a Tactical Vehicle Intervention (TVI) maneuver, directly causing the SUV to flip multiple times and land in a field near Southwest 119th Street and South Meridian Avenue. Vinnie Hamlet was ejected from the vehicle and pronounced dead at the scene. Chris Hamlet and the female passenger sustained injuries but survived.
The Oklahoma Highway Patrol conducted an internal self-investigation. The findings were presented to the District Attorney's Office, which determined that the deadly pursuit was justified.
