On July 22, 2021, around 6:30 p.m., a large fight erupted in the 3000 block of North Water Street in Philadelphia's Kensington neighborhood. Carlos Rodriguez, 40, produced a handgun and allegedly began firing into the crowd, with two shots directed toward responding plainclothes officers. Officers Suarez and Lesesane discharged their weapons, firing at least six shots, fatally striking Rodriguez.

On July 20, 2021, around 5:00 a.m., Pennsylvania State Police responded to reports of gunfire in the 7000 block of Fishing Creek Valley Road in West Hanover Township, Harrisburg. Mitchell J. Shuller, 34, was reportedly firing a rifle at neighboring homes, with bullets striking at least two occupied residences. Upon arrival, troopers ordered Shuller to drop his weapon; however, he refused and fired in the direction of the officers. The troopers discharged their firearms, fatally wounding Shuller.

On June 23, 2021, around 8:00 p.m., Pennsylvania State Police attempted to stop a 2004 Nissan Titan driven by Daryl J. Gakin, 34, of Spearfish, South Dakota, near mile marker 178 on Interstate 80 in Lamar Township, Clinton County. Gakin continued to drive, and officers escalated a high-speed chase for approximately 12 miles that ended near mile marker 165.7 in Marion Township, Centre County. Gakin's vehicle exited the roadway, struck a guardrail, and rolled over multiple times during the chase. Gakin was ejected from the vehicle and killed. The reasons for escalating this fatal chase were not disclosed by police.

On June 17, 2021, Moon Township police responded to a domestic disturbance involving John Geisler, 37, and his mother. Geisler threatened to shoot his mother and any officers who arrived. After fleeing the scene, he was located in his vehiclewith a gun and rammed a police car before fleeing again. A SWAT team surrounded his residence, and after several hours during which flash bang grenades were fired inside, Geisler was found dead from an apparent self-inflicted gunshot wound.

On 06/15/2021, Pennsylvania State Police attempted to stop Jimmy Nguyen near Route 248 and Jandy Boulevard in Nazareth. Nguyen did not stop and continued driving. Pennsylvania State Police escalated a chase that lasted until he struck a vehicle exiting Wegmans Drive and continued at high speeds through several municipalities. Near the intersection of Route 248 and Jandy Boulevard, Nguyen lost control, traveled into the opposite lane, clipped a Toyota RAV4, collided with a Mercedes SUV, and then crashed into a concrete barrier. He died in the crash. A bystander driver in the Mercedes was moderately injured. The officers involved were cleared of any wrongdoing.

Police responded to a 911 hang-up call at a residence. William Michael Kradlak Jr. exited the home. Officers escalated a standoff. Kradlak fired a handgun toward officers. Police opened fire and killed him.

Thomas Razzi was involved in an armed police standoff that escalated into multiple house fires. Razzi was found dead in the rubble of his home. He remained inside his property during the chaotic incident, which destroyed two other townhomes and damaged three more. The confrontation began around noon after police responded to a 911 call reporting that a township code enforcement officer was being chased by a man with a gun. The officer had been at the property for a follow-up inspection due to known hoarding conditions.

Pennsylvania State Police were sent to do a welfare check on Jeffry E. Christopher around 10 p.m. He did not come out of his home, and they returned with a mental health warrant. Christopher reportedly fired two shots at police from around his home around 6 a.m. Around 9:15 a.m., police entered into Christopher's home and took him into custody after he resisted. The manner of death was withheld by police.

On May 23, 2021, Pennsylvania State Police responded to a 911 call reporting a disturbance at 940 Berger Road in Williams Township, Northampton County. Upon arrival around 2:34 p.m., troopers encountered 55-year-old Edward James Shadder outside the residence. A confrontation ensued, during which a trooper fired two rounds from their department-issued Sig Sauer P227 handgun, fatally striking Shadder. The specific details leading to the shooting were not disclosed. An autopsy was scheduled for May 25, 2021.

On April 13, 2021, Pennsylvania State Police responded to a domestic violence call in Saltlick Township, Fayette County. A woman had fled the residence and reported being assaulted and strangled by her husband, Keith Robert Tafoya. Upon arrival, troopers attempted to make contact with Tafoya, who fired a shotgun through a door, grazing one trooper. After several hours of standoff, Tafoya exited the home and raised a shotgun. Officers shot and killed him.

On April 7, 2021, around 6:45 p.m., Philadelphia Police officers conducted a traffic stop on a blue Kia Optima in the 1500 block of West Somerville Avenue after it failed to stop at a stop sign. The vehicle had four occupants. Upon running their information, officers found that two individuals, including 24-year-old James "Poppie" Alexander, had outstanding warrants. As officers attempted to remove Alexander from the vehicle, he allegedly drew a firearm and fired at them, striking Officer Matthew Ponente in the foot. Alexander then exited the vehicle and reportedly fired additional rounds at the officers. In response, Officers Christopher Burton, Charles McCairns, Michael Braun, David Tamamoto, and Matthew Ponente discharged their weapons, fatally wounding Alexander.

On March 15, 2021, the Berks County Department of Emergency Services received a 911 call from a woman reporting that Stephen James Hughes was threatening her with a knife. Muhlenberg Township police responded to the home on the 700 block of Wagon Wheel Lane. After Hughes ignored verbal commands to drop the weapon, an officer fired a Taser. A second officer then opened fire, striking Hughes once in the chest. He was transported to Reading Hospital, where he was pronounced dead. The Berks County District Attorney ruled the shooting justified.

On February 19, 2021, Catasauqua Police Department officers responded to a domestic disturbance involving Ryan Daniel Shirey and his ex-girlfriend at a residence on the 100 block of South 14th Street. Upon arrival, Shirey fled into the basement and retrieved a revolver. Officers followed him and ordered him to drop the weapon. When Shirey did not comply, Officer Joelle Mota discharged his firearm, fatally striking Shirey. He was pronounced dead at the scene. The Lehigh County District Attorney later ruled the shooting justified.

On February 11, 2021, Pennsylvania State Police responded to a domestic dispute at the Harvest Moon Trailer Park in Woodward Township, where 38-year-old Clifford E. Wilbur Jr. had allegedly assaulted his brother-in-law, Fred Peterson, with a handgun and made threats to kill family members. Wilbur fled the scene before troopers arrived and subsequently committed an armed robbery at a nearby Sheetz convenience store, stealing two 24-ounce cans of beer. Troopers located Wilbur's vehicle and initiated a pursuit toward Williamsport on Route 220. Despite the deployment of spike strips that flattened his tires, Wilbur continued to evade police, eventually crashing his car into a utility pole in the 3500 block of West 4th Street. After the crash, Wilbur brandished a handgun and ignored multiple commands to surrender. Officers attempted non-lethal measures, including firing pepper balls into the vehicle, but Wilbur continued to wave his firearm. In response to the imminent threat, troopers and officers discharged their weapons, fatally striking Wilbur. He was pronounced dead at the scene by the Lycoming County Coroner. The Lycoming County District Attorney later ruled the use of lethal force as justified.

On February 9, 2021, around 7 p.m., the Upper Perkiomen Police Department responded to a 911 call from a family member reporting that Trey Bartholomew was in a state of agitation and had left the home carrying a sword. An officer encountered Bartholomew at East Eighth Street and Long Alley. When Bartholomew allegedly threatened the officer with the sword and refused to comply with commands to drop it, the officer shot and killed him.
