Providence PD pulled Michael Pinto over for a traffic stop and claim that he “tried to run an officer over.” An unknown altercation took place and Pinto left with Providence PD chasing him. Two more officers joined the chase. Police had seen during a traffic stop that Pinto’s teenage daughter was in the car with him but they nevertheless hunted him until he stopped and then shot and killed him in front of his daughter. The daughter was also shot in the leg when police shot up the vehicle.

On November 22, 2021, Warwick Police Department officers responded to a welfare check at the Lockwood Condominiums, located at 3524 West Shore Road in Warwick, Rhode Island. The call was initiated by a family member concerned about Frank J. Nappa Jr., who was reportedly experiencing a mental health crisis and armed with a loaded firearm. Nappa exited the condominium multiple times, each time holding a rifle and pointing it at the officers before retreating inside. Despite repeated commands to disarm, Nappa emerged a third time, advancing toward the officers with the rifle aimed in their direction. Officer Justin Martin open fire, striking Nappa. Immediate medical assistance was provided, and Nappa was transported to Rhode Island Hospital, where he was pronounced dead. Subsequent investigations by the Rhode Island Attorney General's Office and the Rhode Island State Police concluded that the use of lethal force was legally justified.

On September 2, 2021, Michael Ouellette, 37, president of the Italian-American Club in Warren, Rhode Island, fatally shot Assistant Fire Chief Brian Remy, 66, and wounded Jason Furtado, 41, inside the club. After the shootings, Ouellette fled the scene on foot. Bristol Police Chief Kevin Lynch and Major Scott McNally, responding to the incident, encountered Ouellette on Water Street. Ouellette fired at the officers, who returned fire. Ouellette then retreated into an alley off Water Street, where he was later found deceased from a self-inflicted gunshot wound.

On May 7, 2021, at approximately 12:30 a.m., Providence Police officers responded to reports of a man screaming in the street near 345 Angell Street, possibly under the influence of narcotics. Upon arrival, they encountered 34-year-old Joseph L. Ventre Jr., who was rolling on the ground and groaning unintelligibly. Officers attempted to calm Ventre and requested assistance from the Providence Fire Department. Due to his erratic behavior and for the safety of all involved, officers decided to restrain Ventre. Body camera footage shows officers holding Ventredown on his stomach for approximately 90 seconds while handcuffing him. Shortly thereafter, Ventre became unresponsive. Emergency responders initiated CPR and transported him to Rhode Island Hospital, where he was pronounced dead at 1:41 a.m. The Rhode Island Medical Examiner later determined that Ventre's death was due to the combined toxic effects of methamphetamine and methadone during police restraint.
