On July 1, 2021, Spotsylvania deputies, Orange officers, and deputy U.S. Marshals went to a home to arrest a man wanted on probation violation charges from Dinwiddie and Colonial Heights, as well as pending gun- and theft-related charges in Orange County and Spotsylvania. Upon police arrival, the man allegedly fired multiple shots at officers. A deputy returned fire before the man barricaded himself inside the house. Police spent several hours attempting to negotiate with the man, but he reportedly started a fire inside the house, which resulted in his death from burns and smoke inhalation.

On June 28, 2021, a deputy observed a motorcycle traveling recklessly northbound on Eastern Bypass and attempted to stop the driver. The motorcyclist, Josemanuel "Chema" Antonio Delgado Alva, continued onto U.S. 29 at a high rate of speed. Additional deputies and Virginia State Police joined the pursuit. The motorcyclist attempted to turn into a crossover but lost control while traveling too fast. The motorcycle crossed the median into the southbound lanes of Route 29, where it struck two vehicles, resulting in the motorcyclist's death.

On June 22, 2021, De'Shon J. Hill allegedly took two hostages inside a convenience store in Luray, Virginia. After a standoff with police, Hill exited the store with a gun pointed at officers. Police responded by shooting and killing him.

On June 16, 2021, at approximately 5:39 p.m., Virginia State Police were asked to assist the Buckingham County Sheriff's Office in arresting a wanted felon. Troopers located a man matching the description of the man, along with a woman inside a Chevrolet Tahoe. When approached, the driver of the vehicle fled the scene, hitting one of the troopers' vehicles. A pursuit ensued, and the driver jumped from the moving vehicle, which then ran off the road and into a ditch. Troopers then confronted Kendall Allen Jamerson, who was armed with a firearm. Police engaged Jamerson, and he was shot and killed.

On June 13, 2021, at approximately 7:23 a.m., deputies responded to a 911 call reporting a domestic dispute. Despite Griffitt telling the dispatcher it was an accidental dial, a woman and her children could be heard screaming in the background. Griffitt's wife and children fled the home, and she stated in a follow-up 911 call that Griffitt had choked and assaulted her the previous night. She also informed authorities that Griffitt was suicidal and armed with a pistol. Police surrounded the home, but after several failed communication attempts, they found Griffitt on the second-floor landing of the house. He died from a gunshot wound, which was determined to be self-inflicted.

On May 30, 2021, a Dinwiddie County deputy attempted to stop a black Mercedes SUV for speeding on I-85. The deputy reportedly lost sight of the SUV on Route 703 and ended the pursuit. Moments later, the SUV driver lost control of the vehicle on wet pavement, veered off the road, and crashed into a tree, resulting in their death.

On May 20, 2021, Washington County Sheriff's deputies were dispatched to the Economy Inn on Lee Highway in response to a suspected violation of a protective order. Upon arriving at the motel room of Brandon Scott Odell, deputies obtained consent to search both the room and the vehicle. During the search, deputies discovered a gun in the vehicle and illegal narcotics in the room. As deputies attempted to take Odell into custody, he allegedly reached under a mattress and retrieved a handgun. One of the deputies shot and killed him during the confrontation.

On May 19, 2021, at approximately 11:53 p.m., a Virginia State Police trooper conducting stationary radar on eastbound Interstate 64 noticed a motorcycle and a Hyundai Genesis speeding at 102 mph in a 65 mph zone. The trooper attempted to stop the vehicles, but both drivers fled the scene. A second trooper joined the pursuit, with speeds exceeding 125 mph. The driver of the Genesis lost control of the vehicle as he approached the Bowers Hill Interchange, ran off the road, and crashed into a tree line. The motorcycle continued and was lost by the troopers. The driver of the Genesis, Javaud Aubrey Ahangari, was killed in the crash.

On May 16, 2021, at approximately 2:14 a.m., officers observed a vehicle driven by Marcus "Marc the Barber" Jeffries driving erratically. Suspecting he was under the influence of drugs or alcohol, officers attempted to stop the vehicle. Jeffries fled the scene, and a pursuit followed through surrounding neighborhoods. The chase ended when Jeffries crossed railroad tracks at a high rate of speed and crashed into a pole, killing him.

On May 14, 2021, deputies with the Augusta County Sheriff's Office responded to a 911 call reporting a break-in at a home on Blue Fish Lane. Upon arrival, deputies encountered a man who fled the scene and ran into the woods behind the house. The deputies followed the man, but as they reached the wood line, Jeffrey Bruce allegedly turned around, had a knife and charged toward a deputy. The deputy responded by shooting and killing Bruce.

On May 10, 2021, Shakeela Imonai Johnson was reportedly fleeing from a domestic incident in Henrico County. Police attempted to stop her vehicle while she was driving on I-95 near Woods Edge Road in Colonial Heights, Virginia. As officers approached, Johnson reportedly shot herself, resulting in her death.

On 05/08/2021, Deputy R.W. Marcelle of the Frederick County Sheriff’s Office attempted to stop a 2003 Cadillac traveling recklessly on Martinsburg Pike (U.S. 11) near Welltown Road. The driver, identified as Kondwani Rogers of Frederick County, continued driving at speeds exceeding 100 miles per hour. The deputy activated his emergency equipment, but Rogers continued south on North Frederick Pike (U.S. 522) toward Winchester. During the pursuit, the Cadillac crossed the center line, sideswiped a Honda Civic, then collided head-on with a Chevrolet Trailblazer. The Cadillac overturned and caught fire. Rogers was pronounced dead at the scene. One driver was hospitalized with serious but non-life-threatening injuries, and another declined treatment.

Central Intelligence Agency and Federal Bureau of Investigation officers responded to a report of a "suspicious vehicle" outside of CIA headquarters in McLean, Virginia, around 6 p.m. Roy Gordon Cole emerged from his car holding a gun and was shot and killed by law enforcement officers.

On April 4, 2021, Virginia State Police attempted to stop a motorcycle after it was seen speeding. The motorcyclist, Lorrence M. Thompson, fled the scene, and the chase was reportedly broken off by the police after two attempts to stop the motorcycle. Shortly after, Thompson crashed his motorcycle into the rear of a pickup truck, leading to his death.

On March 30, 2021, Helen Marie Quarles was driving home from her job at the state probation office. As she attempted to make a left-hand turn onto eastbound Route 3, her vehicle was struck by a Culpeper County Sheriff's Office-issued Chevy Tahoe SUV driven by Lt. Ashleigh Baughan. The officer, who was allegedly driving 75 mph in a 55 mph zone, was not responding to an emergency or pursuing another vehicle, and her emergency lights and sirens were not activated at the time of the crash.

On March 30, 2021, Officer Johnathan Brown of the Bristol Police Department responded to a call about gunshots at the Rodeway Inn. Upon arrival, Officer Brown and other officers encountered Jonathen Blake Kohler, who was sitting in his vehicle. After a brief interaction, Kohler drove toward the officers, and Officer Brown discharged his weapon, fatally wounding Kohler. The incident resulted in a lawsuit filed by Kohler's family, which was later dismissed in favor of Officer Brown, with the court ruling the officer's use of deadly force was justified.

On March 29, 2021, at approximately 9:11 a.m., officers responded to a domestic incident at a home involving Pierce L. Flinchum. When approached by police, Flinchum ran into the house and barricaded himself inside. Police obtained an arrest warrant for him on charges of assault and battery, and possession of a firearm as a felon. Throughout the day, police made multiple attempts to contact him by telephone. At approximately 10:30 p.m., officers entered the home and discovered Flinchum dead from a self-inflicted gunshot wound.

On March 28, 2021, deputies with the Greensville County Sheriff's Office responded to a disturbance call at the Quality Motel on U.S. Route 301. The caller complained of a white Ford Crown Victoria playing loud music in the parking lot. When a deputy arrived, the vehicle, driven by Daiquan Tarez Wright, was seen leaving the motel at a high rate of speed. The deputy attempted to stop the vehicle, but Wright fled down Liberty Road, reaching speeds of 60 mph. Wright's vehicle ran through standing water, lost control, struck a mailbox and fire hydrant, and then crashed into a tree, resulting in his death.

On 03/28/2021, Prince William County police initiated a pursuit of a vehicle that refused to stop for speeding. The chase continued north on Interstate 95 and into Fairfax County, where the fleeing driver, 24-year-old Curtis Armstead Jr., lost control of the car on Interstate 495 near the Route 123 interchange. The vehicle struck a barrier and burst into flames. Armstead died at the scene.

On March 13, 2021, around 11 a.m., officers with the Portsmouth Police Department attempted to stop a 2015 Jeep Wrangler whose driver was wanted on several violations. On February 25, 2021, at approximately 11 p.m., a deputy with the Augusta County Sheriff's Office was traveling westbound on VA-42 when he noticed a vehicle approaching quickly from behind. The vehicle tailgated the deputy, passed him on a double-yellow line, and accelerated to 85 mph. The deputy attempted to stop the vehicle but began a pursuit when the driver allegedly improperly passed another motorist and drove on the wrong side of the road, reaching speeds of 80-100 mph. The driver lost control of the vehicle, a 1993 Lincoln Town Car driven by Ronald Alvin Colvin, which ran off the road, struck a mailbox, spun across the road, hit a ditch, became airborne, and hit a utility pole. Colvin was killed in the crash.

On February 8, 2021, Jonathan O'quin entered Hillcom and fired multiple rounds, killing Kenny Hill in his apartment above the business. A deputy pursued O'quin for about three miles before O'quin turned onto Bartlick Road. O'quin stopped at Bartlick Bridge, got out of the vehicle, and fired multiple rounds at the deputy. The deputy returned fire but did not hit him. O'quin then fatally shot himself in the head with a high-powered rifle.

About 5 p.m., someone called 911 after noticing an unconscious man in the driver's seat of a vehicle stopped in the road. The driver, identified as Richard Fenton Thomas, woke up when deputies arrived. During the ensuing confrontation, deputies shot and killed him. Deputies were called around 10:15 a.m. to a shooting in the parking lot of the Tuckahoe Motorcycle Club. When deputies arrived, they found Ely Grimes, 47, of Fredericksburg, with a gunshot wound to the chest. George Batts was still in the parking lot with a handgun. He reportedly refused commands to drop the weapon, pointed the weapon toward deputies, and deputies shot and killed him.

Police arrived at a residence to serve warrants on Jeffrey D. Kite for violating a protective order. Kite, who lived at the residence, refused to come out of the home. Police subsequently obtained a search warrant to enter the home and additional arrest warrants for stalking and violation of a protective order. About 9 p.m., Kite stopped communicating with them. Around 3 a.m., officers entered the home and found him hiding in a storage area. Police allege that Kite produced a gun and fired at the officers, who shot and killed him.

About 4:35 a.m., a state trooper observed a car traveling 98 mph in a 65 mph zone. The trooper pursued the car, with the chase surpassing more than 120 mph. The driver attempted to make a U-turn, then slid down an embankment into the median. Two troopers approached the vehicle. The driver, identified as Xzavier Deyonte Hill, allegedly displayed a gun, and police shot and killed him.
