On November 6, 2021, at approximately 2:30 a.m., Henrico County Police Officer Timothy Grant Million III responded to a single-vehicle crash on Interstate 64 near the I-295 interchange. The vehicle, driven by 53-year-old Tony Elliott Singleton of Norfolk, Virginia, had overturned multiple times. Upon arrival, Officer Million encountered Singleton, who was carrying a bladed weapon in his waistband. During the encounter, Officer Million discharged his weapon, fatally wounding Singleton. The officer was later indicted on charges of voluntary manslaughter.

On November 6, 2021, an officer attempted to pull over Chelsae Clevenger-Kirk, who was riding her motorcycle without a helmet and with no tags. She refused to stop and continued to her home, where she went inside. The officer tried to make contact but was unsuccessful. As the officer walked back to his patrol car, he reportedly heard gunshots from the residence. A standoff ensued, and Clevenger-Kirk allegedly set fire to her home. After some time, she exited the residence, reportedly holding a gun. Police shot and killed her. Tear gas canisters were deployed into the home, which startled her family members.

Police attempted to stop a vehicle connected to an aggravated assault, suspected abduction, and suspected DUI. The driver did not pull over and eventually entered the interstate. Virginia State Police knocked the vehicle off the road, causing it to overturn, resulting in the death of 47-year-old Amity Jo Grey. Brian Price, 45, the driver, fled on foot. After a brief pursuit, troopers caught up to him, and a fight ensued. During the struggle, a trooper shot and killed Price.

At 3:45 a.m., a Virginia State Police trooper attempted to stop a Dodge Charger traveling 72 mph in a 55 mph zone. The driver, 24-year-old Curtis Armstead, fled and lost control of the vehicle on the Route 123 ramp. The car struck a Jersey wall and then hit the bottom of the overpass for the I-95 Express Lanes, causing it to catch fire. Both the driver and passenger, 23-year-old Miquel D. Jenkins, were pulled from the wreckage but were killed in the crash.

On August 23, 2021, Virginia State Police initiated a pursuit of a man driving a gray Jeep on Interstate 81 in Botetourt County. The driver, who was wanted in Pennsylvania, failed to pull over when attempted to be stopped by a trooper. The chase, reaching speeds over 100 mph, continued through Roanoke County, Salem, and into Montgomery County. During the pursuit, the man communicated with dispatch, stating he was armed. Spike strips were deployed, and the chase ended when police forced the vehicle off the road, causing it to crash. After the crash, the man reportedly shot and killed himself.

On 08/09/2021, the Clifton Forge Police Department attempted to stop a vehicle that fled from officers and entered a pursuit. The vehicle then collided with the moped driven by Randall L. Tinsley on Route 696 resulting in Tinsley’s death at the scene. The fleeing driver continued, crashed off the side of Route 696, and then fled on foot.

On August 3, 2021, around 10:40 a.m., Austin William Lanz exited a bus at the Pentagon Transit Center and immediately attacked Officer George Gonzalez with a knife. Officer Gonzalez was killed during the attack. Lanz then took the officer's gun and shot and killed himself.

On July 18, 2021, at 12:34 a.m., an officer patrolling near the I-64 and I-464 interchange noticed a 2016 Chrysler 200 speeding with its headlights off. The officer attempted to stop the vehicle, which stopped briefly before speeding away on I-64 East. At 12:47 a.m., traffic was slowed due to a prior crash at the 299-mile marker on eastbound I-64, and the Chrysler struck a 2005 Honda Accord from behind. The force of the crash caused the Honda to strike another vehicle and catch fire, killing the driver, Jahsani Jean-Baptiste, and injuring the driver and passenger of the vehicle it struck.

On July 17, 2021, deputies with the Wise County Sheriff's Office were searching for two missing juveniles. While searching the area, a deputy reportedly spotted a vehicle parked in bushes with two people inside. As the 16-year-old male exited the vehicle, an altercation ensued. The juvenile reportedly stabbed the deputy, and the deputy responded by shooting and killing him. The 13-year-old girl inside the vehicle was not injured.

Christopher Lawrence Seay initially stopped, but he allegedly provided false information to officers. When police instructed Seay to park and turn off the engine, he fled the scene. Officers pursued Seay until his vehicle collided with another at the intersection of Williamsburg Road and S Airport Drive. Seay's vehicle overturned several times, ejecting him from the vehicle. He was killed in the crash.

On July 1, 2021, Spotsylvania deputies, Orange officers, and deputy U.S. Marshals went to a home to arrest a man wanted on probation violation charges from Dinwiddie and Colonial Heights, as well as pending gun- and theft-related charges in Orange County and Spotsylvania. Upon police arrival, the man allegedly fired multiple shots at officers. A deputy returned fire before the man barricaded himself inside the house. Police spent several hours attempting to negotiate with the man, but he reportedly started a fire inside the house, which resulted in his death from burns and smoke inhalation.

On June 28, 2021, a deputy observed a motorcycle traveling recklessly northbound on Eastern Bypass and attempted to stop the driver. The motorcyclist, Josemanuel "Chema" Antonio Delgado Alva, continued onto U.S. 29 at a high rate of speed. Additional deputies and Virginia State Police joined the pursuit. The motorcyclist attempted to turn into a crossover but lost control while traveling too fast. The motorcycle crossed the median into the southbound lanes of Route 29, where it struck two vehicles, resulting in the motorcyclist's death.

On June 16, 2021, at approximately 5:39 p.m., Virginia State Police were asked to assist the Buckingham County Sheriff's Office in arresting a wanted felon. Troopers located a man matching the description of the man, along with a woman inside a Chevrolet Tahoe. When approached, the driver of the vehicle fled the scene, hitting one of the troopers' vehicles. A pursuit ensued, and the driver jumped from the moving vehicle, which then ran off the road and into a ditch. Troopers then confronted Kendall Allen Jamerson, who was armed with a firearm. Police engaged Jamerson, and he was shot and killed.

On 06/16/2021, Waynesboro Police responded to reports of a barricaded individual in the West Main Street area. Officers established a perimeter and attempted negotiations. During the standoff, police opened fire and the man was pronounced dead at the scene. Authorities did not publicly release the individual’s identity. On June 13, 2021, at approximately 7:23 a.m., deputies responded to a 911 call reporting a domestic dispute. Despite Griffitt telling the dispatcher it was an accidental dial, a woman and her children could be heard screaming in the background. Griffitt's wife and children fled the home, and she stated in a follow-up 911 call that Griffitt had choked and assaulted her the previous night. She also informed authorities that Griffitt was suicidal and armed with a pistol. Police surrounded the home, but after several failed communication attempts, they found Griffitt on the second-floor landing of the house. He died from a gunshot wound, which was determined to be self-inflicted.

On May 30, 2021, a Dinwiddie County deputy attempted to stop a black Mercedes SUV for speeding on I-85. The deputy reportedly lost sight of the SUV on Route 703 and ended the pursuit. Moments later, the SUV driver lost control of the vehicle on wet pavement, veered off the road, and crashed into a tree, resulting in their death.

On May 20, 2021, Washington County Sheriff's deputies were dispatched to the Economy Inn on Lee Highway in response to a suspected violation of a protective order. Upon arriving at the motel room of Brandon Scott Odell, deputies obtained consent to search both the room and the vehicle. During the search, deputies discovered a gun in the vehicle and illegal narcotics in the room. As deputies attempted to take Odell into custody, he allegedly reached under a mattress and retrieved a handgun. One of the deputies shot and killed him during the confrontation.

On May 16, 2021, at approximately 2:14 a.m., officers observed a vehicle driven by Marcus "Marc the Barber" Jeffries driving erratically. Suspecting he was under the influence of drugs or alcohol, officers attempted to stop the vehicle. Jeffries fled the scene, and a pursuit followed through surrounding neighborhoods. The chase ended when Jeffries crossed railroad tracks at a high rate of speed and crashed into a pole, killing him.

On 05/08/2021, Deputy R.W. Marcelle of the Frederick County Sheriff’s Office attempted to stop a 2003 Cadillac traveling recklessly on Martinsburg Pike (U.S. 11) near Welltown Road. The driver, identified as Kondwani Rogers of Frederick County, continued driving at speeds exceeding 100 miles per hour. The deputy activated his emergency equipment, but Rogers continued south on North Frederick Pike (U.S. 522) toward Winchester. During the pursuit, the Cadillac crossed the center line, sideswiped a Honda Civic, then collided head-on with a Chevrolet Trailblazer. The Cadillac overturned and caught fire. Rogers was pronounced dead at the scene. One driver was hospitalized with serious but non-life-threatening injuries, and another declined treatment.

On April 4, 2021, Virginia State Police attempted to stop a motorcycle after it was seen speeding. The motorcyclist, Lorrence M. Thompson, fled the scene, and the chase was reportedly broken off by the police after two attempts to stop the motorcycle. Upon arrival, Officer Brown and other officers encountered Jonathen Blake Kohler, who was sitting in his vehicle. After a brief interaction, Kohler drove toward the officers, and Officer Brown discharged his weapon, fatally wounding Kohler. The incident resulted in a lawsuit filed by Kohler's family, which was later dismissed in favor of Officer Brown, with the court ruling the officer's use of deadly force was justified.

On March 29, 2021, at approximately 9:11 a.m., officers responded to a domestic incident at a home involving Pierce L. Flinchum. When approached by police, Flinchum ran into the house and barricaded himself inside. Police obtained an arrest warrant for him on charges of assault and battery, and possession of a firearm as a felon. Throughout the day, police made multiple attempts to contact him by telephone. At approximately 10:30 p.m., officers entered the home and discovered Flinchum dead from a self-inflicted gunshot wound.

On March 28, 2021, deputies with the Greensville County Sheriff's Office responded to a disturbance call at the Quality Motel on U.S. Route 301. The caller complained of a white Ford Crown Victoria playing loud music in the parking lot. When a deputy arrived, the vehicle, driven by Daiquan Tarez Wright, was seen leaving the motel at a high rate of speed. The deputy attempted to stop the vehicle, but Wright fled down Liberty Road, reaching speeds of 60 mph. Wright's vehicle ran through standing water, lost control, struck a mailbox and fire hydrant, and then crashed into a tree, resulting in his death.
