On 08/28/2022, Tacoma Police Department officers Jose Figueras, Zachary Hobbs and Christopher Munn shot 40-year-old Peter Tyler Collins during an armed confrontation that developed after police responded to a reported domestic assault in South Tacoma. Collins was struck by police gunfire and also suffered what investigators described as an apparently self-inflicted gunshot wound. The Pierce County Medical Examiner determined that the fatal injury was a rifle wound to the chest and classified Collins's death as a homicide.
Police were dispatched at approximately 12:26 p.m. after Collins's nephew called 911 and reported that Collins had assaulted him. The nephew said Collins had not used a weapon during the alleged assault but warned dispatchers that Collins owned several firearms and might shoot at police. Officers later viewed home-surveillance footage of the alleged assault and said they developed probable cause to arrest Collins for fourth-degree domestic-violence assault.
Officers initially met Collins's nephew about half a block from the residence. While police were reviewing surveillance footage, Collins came outside. Figueras called to Collins and asked him to speak with police, but Collins returned to the house. Hobbs requested additional officers.
The encounter escalated shortly before 3 p.m. According to the Pierce County Force Investigation Team, Collins subsequently fired at police with rifles. Released body-camera and surveillance footage documented portions of the confrontation rather than leaving the sequence dependent solely on officer accounts. Investigators determined that Collins fired 15 rounds from two rifles during approximately four minutes of gunfire.
Figueras, Hobbs and Munn returned fire with department rifles, collectively firing nine rounds. Collins moved through the neighborhood during the exchange, and officers repeatedly broadcast his position and warned other officers that he had a long gun and was continuing to fire.
The final police shot was fired by Christopher Munn from a substantial distance. Munn positioned himself approximately 183 yards from Collins and fired a rifle round that struck him. Investigators subsequently determined that Collins had two gunshot wounds: one consistent with police rifle fire and another that appeared to have been self-inflicted.
Police approached Collins after the gunfire stopped and found him down with firearms nearby. Medical personnel were permitted to approach at approximately 3:08 p.m., and Collins was pronounced dead.
The Pierce County Force Investigation Team conducted the independent investigation. The investigation was ultimately completed, and the City of Tacoma's deadly-force case records identify all three involved officers as Figueras, Hobbs and Munn.
The later evidence significantly expanded the initial reporting. Early accounts did not disclose which officers fired, how many shots police fired or precisely how the gunfire unfolded. The subsequent video investigation established that all three identified officers fired their rifles, that police collectively fired nine rounds and that Collins fired 15 rounds.

OFFICER BACKGROUND:
Jose Figueras was 32 and had served approximately four years with the Tacoma Police Department when Collins was killed. Figueras was one of the three officers who fired during the confrontation.Zachary Hobbs was 27 and had served approximately three years with Tacoma police. Hobbs was also one of the three officers who fired at Collins. Hobbs later became a witness in the criminal trial of Pierce County Sheriff Ed Troyer because Hobbs had responded to Troyer's controversial January 2021 confrontation with Black newspaper carrier Sedrick Altheimer. Hobbs's body-camera footage became evidence in that case; Hobbs testified that Troyer did not tell him at the scene that Altheimer had threatened him or that Troyer was afraid, although Hobbs acknowledged that he had not specifically asked.
Christopher Munn was 38 and had served approximately six years with Tacoma police. Munn fired the long-distance rifle shot from approximately 183 yards that struck Collins. No reliably documented prior fatal police shooting involving Munn was located.

On 08/21/2022, Spokane County Sheriff’s Office Sergeant Darin Staley shot 36-year-old Terry Allen Starkweather Jr. during an attempted arrest at Lyons Self Storage, 1707 E. Lyons Avenue in Spokane. Starkweather remained hospitalized for three days and died from his wounds on 08/24/2022.

The encounter began after Staley received information at approximately 6:25 a.m. that Starkweather was at the storage facility. Starkweather was wanted on outstanding felony warrants for unlawful possession of a firearm, second-degree assault and possession of a stolen motor vehicle. Staley located Starkweather at a storage unit and called Deputy Roman Maier to assist with the arrest.

Staley and Maier developed a plan to approach Starkweather from opposite directions in the narrow lane between two rows of storage units. Maier drove a marked sheriff’s patrol vehicle into the lane from the south and positioned it in front of Starkweather’s pickup truck, while Staley approached on foot from the north.

According to the police investigation, Maier told Starkweather that he was under arrest and ordered him to put his hands in the air. Police reported that Starkweather instead moved from the bed of his pickup into the driver’s seat.

Police alleged that Starkweather rapidly reversed the pickup toward Staley, who was standing in the confined space between the storage buildings. Staley fired his service weapon as the truck moved backward. The pickup then struck one of the storage buildings.

Police alleged that Starkweather subsequently shifted the pickup into drive and moved forward toward Staley, who remained confined by the parallel storage buildings. Staley fired again. The pickup then stopped. Staley was the only officer identified as firing during the encounter.

Deputies provided emergency medical treatment until medical personnel arrived. Starkweather was transported to a hospital in critical condition. He died from the gunshot wounds on 08/24/2022.

The Spokane Independent Investigative Response Team investigated the shooting, with the Spokane Police Department serving as the managing agency because Spokane County Sheriff’s Office personnel were involved. Investigators obtained body-worn-camera footage, surveillance-camera recordings, forensic evidence and statements from Staley and Maier. The completed investigation was forwarded to the Spokane County Prosecutor’s Office in January 2023.

On 03/31/2023, the Spokane County Prosecutor’s Office announced that Staley would not face criminal charges. Prosecutors accepted Staley’s account that he believed the pickup was about to strike him and concluded that his use of deadly force constituted lawful self-defense.

On 08/22/2022, Olympia Police Department Officer Jordan Anderson shot and killed 37-year-old Timothy Green after officers responded to reports that Green was behaving erratically at businesses near Martin Way E and Sleater Kinney Road. Green was experiencing a behavioral health crisis, and his mother later said he had been diagnosed with bipolar disorder as a teenager and had a long history of mental health crises.

Police were initially called about Green's behavior at an AM/PM convenience store and then a nearby Starbucks. Officers were not responding to a reported violent felony or attempting to arrest Green on an outstanding warrant. After officers contacted him, Green left the business area and walked into traffic. Officers then decided to take him into custody.

The encounter was especially controversial because Olympia officers had interacted with Green only two days earlier, on 08/20/2022. Later records cited by Green's family showed that Jordan Anderson and Sergeant Joseph Bellamy were among the officers who encountered Green during that earlier incident and learned about his psychiatric disability. Green's family argued that this prior knowledge should have affected how police handled his behavior two days later.

On 08/22, officers followed Green as he moved through the area. Police alleged that Green possessed a knife. When officers attempted to take him into custody, two officers deployed Tasers, but the electrical weapons did not incapacitate him.

Police alleged that Green continued holding the knife and approached Jordan Anderson. Video of the encounter shows Anderson moving into Green's path and then walking backward with his handgun pointed at Green. Anderson fired three rounds into Green's torso. Green fell into bushes after being shot.

The circumstances immediately preceding the gunfire became a major point of dispute. Police maintained that Green approached Anderson while armed with the knife. Green's family and community advocates questioned whether Green actually presented an imminent threat and argued that officers escalated an encounter involving a man they knew was experiencing a behavioral health crisis. Video of the shooting was subsequently released publicly.

Officers provided medical aid after the shooting. Green was transported to a hospital but died from his wounds. He was the father of two children. His mother, Millie Green, said she had long feared that her son's mental illness could ultimately result in police killing him during a crisis. She had previously gathered signatures in support of Washington Initiative 940, which required additional police de-escalation training.

The Capital Metro Independent Investigations Team investigated the killing. Olympia Police Department detectives did not participate in the investigation because Olympia officers were involved. The investigation was completed in January 2023 and forwarded for independent prosecutorial review.

The Clark County Prosecuting Attorney's Office subsequently concluded that the officers' actions were lawful. In December 2023, the Thurston County Prosecuting Attorney's Office announced that no criminal charges would be filed against Anderson or the other Olympia officers involved.

Green's family filed a $21 million tort claim against the City of Olympia. In August 2024, Olympia agreed to pay $600,000 to settle the claim. The settlement also required significant police training changes, including 40 hours of crisis-intervention training for patrol officers and an additional eight-hour course addressing force options when dealing with people experiencing behavioral health crises.

The settlement additionally required Police Chief Rich Allen, deputy chiefs and the officers involved in Green's death to undergo state training concerning the historical intersection of race and policing. Green's family also sought further review of Anderson's actions and filed a complaint with the Washington State Criminal Justice Training Commission alleging failures to de-escalate and intervene and raising concerns about officer conduct.

Olympia's internal Use of Force Review Board subsequently concluded that the officers acted within department policy and training. The city's police auditor reviewed the case and found the internal review thorough and consistent with department standards.

On 08/06/2022, Sunnyside police responded to a report that a person had been shot multiple times at the Fraternal Order of Eagles, 100 East South Hill Road, in Sunnyside. Officers identified 26-year-old Ezequiel Ayala as the person they were attempting to locate in connection with the shooting and relayed his possible location to the Yakima County Sheriff's Office.
Yakima County deputies located Ayala near his mother's residence and attempted to stop his vehicle. Ayala continued driving away and deputies pursued him into the Lower Yakima Valley. The pursuit ended in a vineyard in the 3900 block of Gilbert Road near Zillah after Ayala left the vehicle and ran into the vineyard.
According to the Yakima Valley Special Investigations Unit, Ayala fired multiple rounds at deputies while running through the vineyard. Deputies Tyler McCallum and Jonathan O'Dell returned fire, striking Ayala. He died at the scene. Investigators recovered a 9 mm handgun with a partially expended magazine near Ayala.
The Yakima Valley Special Investigations Unit conducted the independent investigation. On August 9, 2022, the deputies who fired their weapons were identified as Deputy Tyler McCallum, who had served with the Yakima County Sheriff's Office since 2019, and Deputy Jonathan O'Dell, who had served since 2020.

On 08/03/2022, Dominic Shears and two other men committed an armed robbery at the Oxford Suites in Spokane Valley. After dropping off the other two men in downtown Spokane, Shears crashed the getaway van into a business at the intersection of East 3rd Avenue and South Sheridan Street. Responding Spokane Police officers encountered Shears, who fired multiple rounds at them from inside the van. Officers returned fire, and Shears remained barricaded inside the vehicle. One officer sustained minor injuries from shrapnel.
SWAT personnel from multiple agencies responded and negotiated with Shears for nearly five hours. During the standoff, officers observed him still armed and refusing repeated commands to surrender. Shortly before 6:00 a.m., Shears exited the van while still armed. Detective Derek McNall of the Spokane County Sheriff's Office and Spokane Police Officers Jacob Siegel, Robert Riggles, Brandon Lynch, and Scott Lesser fired their service weapons, striking Shears. He was pronounced dead at the scene.
The Washington State Patrol managed the investigation through the Spokane Independent Investigative Response (SIIR) Team. In June 2024, the Spokane County Prosecutor concluded that the officers' use of deadly force was legally justified and declined to file criminal charges.
