On 12/08/2021, members of the U.S. Marshals Service's Southern Ohio Fugitive Apprehension Strike Team (SOFAST) served an arrest warrant for a person wanted for aggravated robbery at a residence in the 2700 block of Fourseasons Drive in Columbus, Ohio.

While the individual named in the warrant was being taken into custody, 18-year-old Donta R. Stewart, who was not involved in the warrant, allegedly began firing at officers from a nearby closet, striking a deputy U.S. Marshal in the upper torso. Officers opened fire, critically injuring Stewart. He was transported to Mount Carmel Hospital, where he was pronounced dead. The injured deputy was taken to Grant Medical Center in stable condition and was expected to survive.

On November 23, 2021, at approximately 9:00 a.m., a Vermilion Parish Sheriff's Deputy conducted a traffic stop on a motorcycle near the intersection of LA-700 and Shelly Road in Kaplan, Louisiana, due to the rider not wearing a helmet. The motorcyclist, identified as 43-year-old Kelly Vaughn (also known as Kelly Griffin), did not provide identification and reportedly gave false information regarding his identity. Police alleged that he retrieved a handgun from his saddlebag; however, they admitted that he did not threaten them. Officers drew their firearms, and Vaughn fled on foot into a dense grassy field. Law enforcement established a perimeter, and the Vermilion Parish Sheriff's Office Special Response Team, along with Louisiana State Police, deployed aerial drones to locate him. Negotiators demanded his surrender, but Vaughn responded by firing shots at the drones and stated he would end his life rather than be taken to jail. At approximately 4:24 p.m., many hours after the standoff began, Vaughn died from a self-inflicted gunshot wound during the SWAT standoff. It is unknown whether he had committed any crimes, as police did not comment further.

On 11/22/2021, U.S. Marshals were attempting to serve an arrest warrant in the 700 block of N Sunset Avenue in West Covina, California. According to law enforcement, the individual ran from officers and allegedly produced a firearm. A West Covina police officer assigned to the U.S. Marshals Pacific Southwest Regional Fugitive Task Force opened fire, killing the individual.

On September 15, 2021, members of a U.S. Marshals' task force attempted to serve an arrest warrant at 6 Vinewood Place in Rochester, New York, for 24-year-old Dedrick Jamon James. As officers attempted to apprehend him, James allegedly returned back into the residence and struggled with officers.

On July 9, 2021, members of the Great Lakes Regional Task Force, including Chicago police officers, U.S. marshals, and Cook County deputies, attempted to apprehend 34-year-old Klevontaye White, who was wanted on multiple warrants, including charges of aggravated sexual assault with a firearm and escape from electronic monitoring. The task force located White in his vehicle on South Kilpatrick Avenue in Chicago. When officers approached, White allegedly pointed a firearm at them. On July 1, 2021, Spotsylvania deputies, Orange officers, and deputy U.S. Marshals went to a home to arrest a man wanted on probation violation charges from Dinwiddie and Colonial Heights, as well as pending gun- and theft-related charges in Orange County and Spotsylvania. Upon police arrival, the man allegedly fired multiple shots at officers. A deputy returned fire before the man barricaded himself inside the house. On June 24, 2021, at approximately 7:25 p.m., Pitt County Sheriff's Office deputies responded to a domestic disturbance at 5505 River Creek Drive in Washington, North Carolina. Deputies encountered 48-year-old Chad Necessary, who was reportedly armed with multiple firearms. During the encounter, Necessary allegedly pointed a loaded gun at one deputy and then at his wife. Believing that Necessary was about to shoot his wife, Deputy Doug Dolenti discharged his firearm, striking Necessary three times: once in the hip, once in the thigh, and once in the torso.

During the altercation, Necessary turned his weapon on himself and fired a single gunshot to his head. He was pronounced dead at the scene. No deputies were injured. The North Carolina State Bureau of Investigation reviewed the incident, including body-worn camera footage and witness statements. An autopsy confirmed four gunshot wounds, including the self-inflicted shot. The Pitt County District Attorney's Office concluded the use of deadly force was justified under North Carolina law, and no charges were filed against the deputies.

On 05/26/2021, an hours-long standoff occurred at 1454 Union Street in Manchester, New Hampshire. On April 9, 2021, U.S. Deputy Marshals responded to a residence on the 300 block of North Hampton Road in Springfield, Ohio, at sunrise to arrest 50-year-old Douglas C. Barton. Barton was wanted for allegedly threatening a judge.

On March 24, 2021, U.S. Marshals and other law enforcement officers arrived at a residence in the 1700 block of Regent Avenue NE in Canton, Ohio, to apprehend 33-year-old Aaron Pierre Thomas, who was wanted for armed robbery and aggravated assault in Danville, Virginia.

Thomas allegedly attempted to escape through a window before exiting the front door and opening fire on the officers. During the exchange, a U.S. Marshal was injured. Officers opened fire, striking Thomas. Both the injured officer and Thomas were transported to a nearby hospital. The officer was treated and released, while Thomas remained hospitalized in critical condition.

The incident was investigated by the Stark County Prosecutor's Office, which presented the case to a grand jury. The grand jury declined to indict the officers involved. Approximately six months later, Thomas died from his injuries sustained during the shooting.

On March 24, 2021, U.S. Marshals and Phoenix Police officers responded to a call regarding a man who had fled a Home Depot after allegedly stealing tools. A witness followed the individual, and a collision occurred as the vehicle continued to flee. U.S. Marshals and local officers attempted to stop the vehicle, but the man drove erratically and headed westbound on U.S. 60, entering Phoenix. After a brief pursuit, the vehicle stopped near 24th Street and Jefferson Street. When officers attempted to approach, the individual allegedly moved toward them, and officers shot and killed him.

After exiting his vehicle, Cook reportedly walked away, turned towards law enforcement, and brandished a gun. Despite commands to surrender, he was shot and killed by two deputies.

On March 23, 2021, U.S. Marshals attempted to arrest 32-year-old Frankie Jennings at a gas station in Charlotte, North Carolina. Jennings was wanted on multiple warrants, including assault with a deadly weapon against law enforcement and possession of a firearm by a convicted felon. During the attempted arrest, Jennings reportedly reached for a firearm inside his vehicle. Senior Inspector Eric Tillman fired three shots, fatally wounding Jennings. The Mecklenburg County District Attorney's Office later determined that the use of deadly force was justified.
