On November 28, 2021, at approximately 2:04 p.m., the BP Express gas station in Culver, Indiana, reported an intoxicated individual attempting to purchase alcohol. The individual, later identified as 33-year-old Dylan Wade Bush, departed the scene in a white Chevrolet Cavalier before officers arrived.

A Culver Police Department officer located Bush's vehicle near State Road 17 and Washington Street around 2:07 p.m. and initiated a pursuit when Bush failed to stop. The chase, involving high speeds exceeding 100 mph, continued through Marshall County for approximately 13.5 miles.

Sergeant Matthew Brown of the Marshall County Sheriff's Department joined the pursuit near State Road 10 and Sycamore Road. The pursuit concluded when officers boxed in Bush's vehicle at the First National Bank of Monterey's parking lot on North Lake Shore Drive. Sergeant Brown positioned his patrol car in front of Bush's vehicle and exited to issue commands. Bush ignored the commands and accelerated, pinning Sergeant Brown between the vehicles. In response, Sergeant Brown fired three shots from his service weapon, striking Bush.

After being shot, Bush's vehicle traveled several hundred feet before colliding with an unoccupied pickup truck in a nearby parking lot. Officers administered first aid until paramedics arrived, but Bush was pronounced dead at the scene by the Marshall County Coroner.

An autopsy revealed Bush had a blood alcohol content of .23%, nearly three times the legal limit, and the presence of cannabinoids and methanol in his system. The Marshall County Prosecutor's Office determined Sergeant Brown's use of deadly force was justified.

On November 28, 2021, Winston-Salem Police officers responded to a disturbance involving weapons at a residence on Bell West Drive in Kernersville, North Carolina. Residents reported that 33-year-old Adam Brent McKnight had assaulted a family member and posed a threat to others. Officers encountered McKnight at the back of the property near a pool house. McKnight emerged holding a machete and reportedly refused repeated commands to drop the weapon. Officers deployed Tasers in an attempt to subdue him. Despite this, McKnight continued advancing toward Officer R.T. Phillips. When McKnight got within a few feet, Officer Phillips discharged his firearm, fatally striking him. McKnight was pronounced dead at the scene.

On 11/28/2021, officers from the Seekonk Police Department responded to a report of a break-in at a local business. They later located Jeffrey W. Groulx inside his car in a nearby parking lot. Upon spotting the officers, Groulx fled the scene, leading to a crash at the intersection of US-6 and School St. After the crash, Groulx allegedly opened fire on the officers with an AK-47 rifle. The officers returned fire, resulting in Groulx's death.

The Kalamazoo Public Safety officers responded to the scene approximately two minutes after the initial 911 call was made. Upon arrival, they encountered an active shooter situation. Responding officers, engaged in a shootout with Oliver and Sean and killed him. The three others sustained non-life-threatening injuries. The shooting led to a temporary suspension of Metro bus services for the day. Investigations into the incident, including Oliver's motives and whether he knew any of the victims, were ongoing at the time. Oliver had a history of mental health issues, with his family indicating he was seeking help for these problems prior to the incident. The Kalamazoo County prosecutor later ruled that the officers' actions were in self-defense, and no criminal charges were filed against them. Video evidence from body cameras, bus cameras, and the transportation center's security cameras captured much of the incident. On November 26, 2021, at approximately 2:00 p.m., deputies from the Bernalillo County Sheriff's Office responded to a report of a hit-and-run crash at the intersection of Edith Blvd NE and Montano Rd NW in North Valley, Albuquerque. Deputy Ronald Perez, a 10-year veteran of the department, was among the responding officers. The suspect, identified as 18-year-old Elijah Richie, fled the scene on foot after crashing his red SUV into a chain-link fence at Edith Blvd and Nikanda Road. Surveillance footage showed Richie attempting to flee the scene, armed with an AR-15 style pistol and carrying a backpack containing additional firearms. Deputy Perez pursued Richie and, after verbal commands to drop the weapon, fired a single shot, fatally wounding Richie. Richie was pronounced dead at a local hospital shortly after the incident. The shooting was captured on body-worn cameras and surveillance footage. The incident is under investigation by the Multi-Agency Task Force, a collaborative team that investigates officer-involved shootings.

On November 26, 2021, at approximately 2:30 p.m., a New Mexico State Police officer arrived at a Starbucks on Promenade Boulevard in Santa Fe and was informed by employees that the store had just been robbed by a man and a woman with firearms. The suspects fled in a black Kia without a license plate. A pursuit ensued, with the suspects traveling south on Richards Avenue, then north on Interstate 25, and subsequently south on U.S. 285. During the chase, the female passenger, identified as Christean "Christy" Ann Dimas, fired shots at officers. Officers from the New Mexico State Police, Santa Fe County Sheriff's Office, and Torrance County Sheriff's Office returned fire. The vehicle crashed near Clines Corners, and the driver, Jacob Montoya, was apprehended. Dimas was found deceased in the passenger seat. Montoya was airlifted to a nearby trauma center for gunshot wounds and later booked into the Torrance County Detention Facility on multiple charges, including assaulting a peace officer and shooting from a motor vehicle. No officers were injured during the incident.

A motorcyclist was killed after officers pursued him across multiple Houston-area cities, reaching speeds in excess of 100 mph. The pursuit ended around 9:45 p.m. when the motorcyclist entered Dixie Farm Road, turned into oncoming traffic, and hit a curb, causing him to lose control of the motorcycle and crash, resulting in his death.

Around 11:30 a.m., officers responded to a call about a man with a gun. Patrol officers found a woman with a fatal gunshot wound inside a neighbor's garage. On November 24, 2021, while following up on a tip, Nebraska State Patrol officers spotted a vehicle with German "Pee-Wee" Pedrazo and a woman inside. When police confronted Pedrazo, he allegedly reached for a gun. A trooper shot and killed him. No BWC was provided. Pedrazo died 4 days later from injuries.

Rockwall police attempted to stop a white Chrysler 300 for the violation of a stolen license plate. The pursuit continued into the city of Fate, and Fate Police Department took over the pursuit. The vehicle traveled into the median of the interstate and lost control, striking a culvert. The driver of the Chrysler was killed, and the passenger was injured.

On November 23, 2021, at approximately 2:35 p.m., Pasadena Police Department officers were called to a report of a shooting at a residence where 70-year-old GevorkMinissian had been shot. Devin Edward Hall fled the scene on foot and allegedly attempted to carjack a woman by jumping into her vehicle. Hall was unable to steal the car and fled on foot. Police engaged him in a shootout.

Hall ran onto Sunnyslope Avenue, where he allegedly attempted to carjack a second woman but again failed to take the vehicle. He reportedly held the woman at gunpoint until he was cornered by officers. Police engaged him in a shootout and he was shot and killed.

On November 23, 2021, at approximately 9:00 a.m., a Vermilion Parish Sheriff's Deputy conducted a traffic stop on a motorcycle near the intersection of LA-700 and Shelly Road in Kaplan, Louisiana, due to the rider not wearing a helmet. The motorcyclist, identified as 43-year-old Kelly Vaughn (also known as Kelly Griffin), did not provide identification and reportedly gave false information regarding his identity. Police alleged that he retrieved a handgun from his saddlebag; however, they admitted that he did not threaten them. Officers drew their firearms, and Vaughn fled on foot into a dense grassy field. Law enforcement established a perimeter, and the Vermilion Parish Sheriff's Office Special Response Team, along with Louisiana State Police, deployed aerial drones to locate him. Negotiators demanded his surrender, but Vaughn responded by firing shots at the drones and stated he would end his life rather than be taken to jail. At approximately 4:24 p.m., many hours after the standoff began, Vaughn died from a self-inflicted gunshot wound during the SWAT standoff. It is unknown whether he had committed any crimes, as police did not comment further.

The driver, 31-year-old Blake Edward Thacker, was wanted on charges from Oregon. During the chase, Thacker's vehicle collided with another car, causing their vehicle to roll over. On 11/22/2021, U.S. Marshals were attempting to serve an arrest warrant in the 700 block of N Sunset Avenue in West Covina, California. According to law enforcement, the individual ran from officers and allegedly produced a firearm. A West Covina police officer assigned to the U.S. Marshals Pacific Southwest Regional Fugitive Task Force opened fire, killing the individual.

On November 22, 2021, Warwick Police Department officers responded to a welfare check at the Lockwood Condominiums, located at 3524 West Shore Road in Warwick, Rhode Island. The call was initiated by a family member concerned about Frank J. Nappa Jr., who was reportedly experiencing a mental health crisis and armed with a loaded firearm. Nappa exited the condominium multiple times, each time holding a rifle and pointing it at the officers before retreating inside. Despite repeated commands to disarm, Nappa emerged a third time, advancing toward the officers with the rifle aimed in their direction. Officer Justin Martin open fire, striking Nappa. Immediate medical assistance was provided, and Nappa was transported to Rhode Island Hospital, where he was pronounced dead. Subsequent investigations by the Rhode Island Attorney General's Office and the Rhode Island State Police concluded that the use of lethal force was legally justified.

The officer initiated a traffic stop, and the driver, identified as 28-year-old Isiah Smith, initially complied by stopping the vehicle.

As the officer approached on foot, Smith drove away, prompting a police pursuit. During the pursuit, another officer discovered that the tags on the vehicle did not belong to the Acura Smith was driving. On November 21, 2021, at approximately 10:00 p.m., officers from the Louisiana State Police and the East Jefferson Levee District Police Department were directing traffic at the intersection of Airline Highway and Severn Avenue in Metairie, Louisiana, due to traffic signal repairs. Several concerned motorists alerted the officers to a man walking along Airline Highway near Causeway Boulevard, armed with a firearm and a knife. The individual was identified as 52-year-old Jabari Farafiai Asante-Chioke of LaPlace. He fled on foot when police demanded that he stop. A trooper fired a Taser. Asante-Chioke turned and pointed a gun. A state trooper and two East Jefferson Levee District officers all opened fire at the same time and killed him. He was pronounced dead on the highway.

Maurice Monk, a 45-year-old Black man and security guard, died on November 15, 2021, while in custody at Santa Rita Jail in Dublin, California. He was incarcerated in early October after missing a court hearing related to a prior arrest. Monk suffered from diabetes and high blood pressure and was unable to receive his necessary medication despite his sister, Elvira Monk, making repeated attempts to provide it. Jail staff failed to process the prescription information. Two years after his death, body camera footage revealed that Monk had been left unattended for days before deputies discovered he had died. The Alameda County Sheriff’s Office is under scrutiny for the handling of his medical care.

Monk, who had mental health and other healthcare needs, had been left unchecked in his cell for several days. An internal investigation revealed that deputies and medical staff failed to conduct required welfare checks, and some allegedly falsified records to indicate checks had been performed. In November 2024, Alameda County District Attorney Pamela Price filed felony charges against seven sheriff's deputies, two former deputies, and two medical staff members for dependent adult abuse and neglect in connection with Monk's death. Three of the defendants were also charged with falsifying documents. Monk's family previously settled a lawsuit against the county for $7 million.

On November 14, 2021, at approximately 9:35 p.m., Hollywood Police Department officers pursued 16-year-old Christopher Walls, who was possibly involved in a carjacking a black 2018 Alfa Romeo and fleeing in the vehicle. The chase ended when Walls crashed the car near Northwest 62nd Street and Northwest 27th Avenue in Miami, Florida. Witnesses to the arrest alleged that police used excessive force, stating that Walls was beaten during the encounter. After being handed over to Miami-Dade Police custody, Walls became unconscious and later died. Miami PD did not disclose the names of the officers who beat him to death.

Carlos Ortega, 38, entered the barbershop and shot Jeffers while he was cutting the hair of off-duty Baltimore Police Sergeant David Burch. In response, Sergeant Burch retrieved his personal firearm and shot Ortega six times, killing him.

On November 11, 2021, at approximately 4 p.m., NYPD officers pulled over 65-year-old Brian Astarita for allegedly speeding in a gray Jeep Cherokee near Bay 8th Street in Brooklyn, New York. As the officer approached the vehicle, Astarita fled, prompting a pursuit. During the chase, Astarita reportedly rammed the patrol car after it stopped in front of his Jeep.

Officers pursued Elliot Lewis in an hour-long, high-speed chase across the Houston area. He reportedly made phone calls to dispatch during the chase, which started near the Texas Medical Center/OST area about 11 a.m. Police cornered him in a parking lot after intentionally crashing into his vehicle. Officers were investigating a stabbing that occurred about noon on Las Vegas Trail where the victim's gray truck was also stolen. At about 1:45 p.m., officers located the man suspected in the stabbing. Chassity Brooks crashed the stabbing victim's truck into the Taz game room, killing 45-year-old Jeffrey Mazurowski inside. Officers were nearby and began following Brooks. About a mile down the road, he allegedly crashed into a woman sitting at a bus stop, Lailani Snell, who was killed. Police arrested Brooks.

On November 8, 2021, at around 10: 45 a.m., a standoff began at 1523 Washington Avenue in Kalamazoo when police from Kalamazoo Public Safety and the Kalamazoo County Sheriff's Office attempted to arrest Alex Rawls, a 35-year-old male, on charges related to a shooting incident on November 1 where he allegedly shot his ex-girlfriend's sister five times. The standoff lasted approximately 16 hours, ending around 3 a.m. on November 9, 2021. During this time, Rawls fired multiple shots at law enforcement, although no officers were injured. Officers returned fire, and Rawls was struck by several bullets. After the standoff concluded, Rawls was found deceased on the second floor of the residence. An autopsy later confirmed that while Rawls was hit by police gunfire, the fatal shot was self-inflicted, though police provided no proof of this assertion at the time. The incident drew significant attention due to the length of the standoff, the use of 'directed destruction' tactics by police to view inside the home, and the subsequent demolition of the property. Rawls' family has questioned the circumstances of his death, with his sister stating that he never spoke of suicide and that the system had failed him since childhood. The investigation was handed over to Michigan State Police, who placed the involved officers on administrative leave pending review. On 11/09/2021, the Brooklyn Park Police Department responded to a call for a stolen truck. Officers saw a man driving the truck. The truck drove through a ditch onto Highway 610, losing a construction trailer. It then dragged fencing for miles onto Highway 252. Police pursued the truck. At the intersection of MN-252 and 66th Avenue N, police escalated a chase which ended when he crashed Into a fence and a light pole and died.
