On October 10, 2021, at approximately 6 p.m., Brunswick County Sheriff's Office deputies responded to a domestic disturbance at 8627 Wanets Landing Road in Leland, North Carolina. Linda Beck, the caller, reported that her husband, 57-year-old Michael Robert Beck, had fired a firearm in her direction twice and expressed concerns about his volatility. Upon arrival, deputies located Beck on the back deck of the residence. He reportedly produced a firearm and pointed it toward the deputies. Despite repeated commands to drop the weapon, Beck allegedly failed to comply. Two deputies discharged their firearms, fatally striking him.

The Brunswick County District Attorney's Office, in coordination with the North Carolina State Bureau of Investigation, reviewed the case and determined that the deputies were legally justified in their use of deadly force.

On October 9, 2021, at approximately 2:46 a.m., Maryland Transportation Authority Police Officer Theodore Jeremenko observed a silver Chevrolet Monte Carlo commit a traffic violation near the intersection of I-395 and Conway Street in Baltimore, Maryland. Suspecting the driver might be impaired, Officer Jeremenko initiated a pursuit.

The driver, identified as 26-year-old Jawuan James Ginyard, initially pulled over on I-95 South but then fled as Officer Jeremenko approached his vehicle. The officer chased him onto I-695, with Ginyard reaching speeds exceeding 100 mph and weaving through traffic. Upon taking the Wilkens Avenue exit ramp, Ginyard lost control of his vehicle, which struck the median, causing him to be ejected from the car. Officer Jeremenko rendered first aid until paramedics arrived, but Ginyard was pronounced dead at the scene.

Officers were responding to a DWI call a block away when they heard gunshots in the entertainment district at around 1 a.m. The officers arrived to find two groups of men shooting at one another. An officer began firing at the men. Michael "Mikey" Carothers was killed. It was not immediately known whether the teen was killed by officer-fired bullets or from the other group involved in the shooting.

At approximately 4:30 a.m. on October 9, 2021, a Middlesboro police officer shot and killed 35-year-old Timothy Houston. Houston was reportedly wanted in Claiborne County, Tennessee, on charges stemming from a previous shooting. The incident occurred in Middlesboro, Kentucky. On October 9, 2021, at approximately 8:00 p.m., officers from the Las Cruces Police Department initiated a traffic stop near the intersection of 5th Street and Wilson Avenue. During the stop, the driver, identified as 35-year-old Gustavo Angel Esparza, opened fire on the officers before fleeing the scene. On October 8, 2021, Officer Dustin Broughton of the Mexico Police Department responded to an unknown incident at 1167 Main Street in Dixfield, Maine, involving 29-year-old Matthew Allen Marston. On 10/08/2021, around midnight, Tulsa Police officers saw Tommy Carmichael, 46, driving a white truck in West Tulsa. Carmichael was a person of interest in the death of his mother, Carol Longacre, who had been found dead in her bathtub earlier that day. Officers followed Carmichael as he drove toward the home where Longacre’s body was discovered.
Carmichael arrived at the house near South Utica Avenue and East 61st Street and tried to enter the property. Before officers could apprehend him, Carmichael shot himself and was pronounced dead at the home.

On 10/08/2021, the Pennsylvania State Police and Coaldale Police Department were involved in a standoff with 47-year-old Richard Lee Sweet in Coaldale, Pennsylvania. Sweet was wanted in connection with the fatal shooting of his girlfriend, Joan Cressman, earlier that day in East Allentown. After the shooting, Sweet fled to his home on the 100 block of Fisher Avenue in Coaldale. Police escalated a large standoff that lasted over four hours. On October 8, 2021, at approximately 8:51 p.m., Winston-Salem Police officers responded to a 911 call reporting that 46-year-old Gilbert Lee Collison Jr. had broken into a residence on South Main Street. The caller reported that Collison was armed with a shotgun and had threatened to shoot any law enforcement officers who attempted to intervene. Officers learned that Collison had previously pointed the shotgun at his girlfriend during an argument.

The Winston-Salem Police Department, along with SWAT and crisis negotiators, surrounded the residence and made repeated attempts to communicate with Collison. Despite these efforts, Collison fired shots at the officers. At approximately 2:30 a.m., Collison appeared in the front doorway with the shotgun. Officers used a less-lethal impact baton, striking him in the leg, but Collison responded by firing on the officers. Three officers returned fire, fatally striking Collison.

On October 7, 2021, at approximately 9:06 a.m., Cleveland Police Department officers responded to a mental disturbance call at a residence on the 700 block of Springhill Drive NE. Upon arrival, three officers encountered 71-year-old Lloyd Grant McClung Jr., who was armed with a shotgun. Despite repeated verbal commands to drop the weapon, McClung advanced toward the officers and fired a round from his shotgun. Two officers opened fire, striking McClung. McClung was transported to a local hospital, where he was pronounced dead.

On October 6, 2021, at approximately 6:00 a.m., correctional officers with the Hardeman County Sheriff's Office were outside the Hardeman County Criminal Justice Complex when they were allegedly threatened by an armed individual. Three officers from the Bolivar Police Department responded, but the man had already fled. Shortly thereafter, the individual returned, exited his vehicle while still armed, and confronted the officers. The officers shot and killed 43-year-old Andra Davon Murphy.

Additional Information: The family of Andra Davon Murphy has retained attorney Ben Crump, asserting that Murphy was not the individual who initially threatened the correctional officers and is seeking the release of video footage to clarify the events leading to his death. On October 4, 2021, shortly before 11:30 a.m., Terrebonne Parish Sheriff's deputies responded to a disturbance at a business on Tunnel Boulevard in Houma, Louisiana, involving 45-year-old Shane E. Hartman, who was reportedly seen with a knife. Deputies took the knife, had him evaluated by medical personnel, and released him. Later, deputies received a second complaint that he was impeding traffic. Hartman asked to be taken to a business within the city limits. When police drove him, he refused to exit the patrol vehicle. At approximately 10:30 p.m., an officer with the Houston Police Department spotted a man who was driving a white Infiniti G37, doing doughnuts in a parking lot. When the officer activated his sirens in an attempt to conduct a traffic stop, police said he fled at high speed down the residential street. He then allegedly hit three valets, 22-year-old Eric Orduna, 23-year-old Nick Rodriguez, and 18-year-old Fnan Measho, before flipping the vehicle and crashing into a fence.

Around 2 a.m., a QuikTrip clerk called 911 to report that a woman had entered the store, stating her boyfriend, Jess Jackson, had threatened her, made suicidal statements, and possibly fired a gun at a home on North Terrace Drive. When officers arrived at the QuikTrip, the woman was no longer present. Officers proceeded to the residence on North Terrace Drive, where a resident confirmed Jackson was staying due to relationship issues and allowed officers entry. Officers found Jackson in bed and ordered him to get out and show his hands. Jackson ignored commands and reached toward the floor, producing a handgun from under the blankets. An officer then fired four shots, fatally wounding Jackson.

On 10/03/2021, Paula Kathern Kennedy, 58, was killed in a police chase crash at the intersection of Noe Bixby Road and Chatterton Road in Blacklick Estates, Ohio. The incident began when Madison Township Police pursued an allegedly stolen 2022 Mercedes SL at approximately 8:44 a.m. The driver of the stolen vehicle, identified as a 15-year-old boy, did not stop. Police escalated a high-speed chase through a red light at the intersection, where the stolen vehicle crashed head-on into Paula Kennedy's 2019 Nissan Rogue. Paula Kennedy died from her injuries. The teenage driver was seriously injured and hospitalized.

On October 3, 2021, at about 7:58 a.m., Nevada Highway Patrol received a report of domestic battery occurring inside a gray Toyota Corolla, with witnesses describing a man hitting a woman. Troopers found the vehicle around 8:19 a.m. near Lake Mead Blvd. by mile marker 14. At approximately 8:26 a.m., officers shot and killed Adrian Zarate-Cervantes. Details were not disclosed.

On October 2, 2021, at approximately 2:00 a.m., 58-year-old Wade Anthony Adams Sr. called 911, stating that he was armed with a shotgun, consuming alcohol and marijuana, and expressing suicidal intentions, indicating that officers would have to kill him. Westwego Police officers located Adams in a church parking lot, but he refused to communicate and drove away, leading to a two-hour low-speed pursuit around the city. Officers eventually boxed in Adams' vehicle in the 400 block of Fourth Street. As they approached, Adams reportedly reached for his shotgun. Officers opened fire and killed him. Adams was pronounced dead at the scene.

On October 2, 2021, at approximately 8:30 p.m., the Lane County Sheriff's Office responded to a report of a domestic dispute involving a firearm at a residence in the 87000 block of Rice Road in Mapleton, Oregon. The caller reported that 28-year-old Jazmine Thein had fired a gun at her brother, who fled to a neighbor's home for safety.
Deputies arrived on the scene and confirmed Thein was still inside the residence. Shortly after 6:00 a.m., deputies discovered Thein deceased in a bedroom from what appeared to be a self-inflicted gunshot wound.

On 10/01/2021, the Pittsburgh Police Department and the Allegheny County Police Department responded to a situation involving Khalil Singletary. Officers engaged in a five-hour standoff in Pittsburgh's Hill District as they attempted to apprehend Singletary, who was wanted for questioning in a homicide. During the standoff, Singletary reportedly exchanged gunfire with officers. He was found deceased in the home shortly before 6 p.m. after the standoff began around 12:30 p.m.

On October 1, 2021, Jason Metcalfe, 41, was accused of a fatal shooting in the Lake Lorelei community of Brown County, Ohio. Metcalfe fled the location in a stolen 2016 Chevrolet pickup truck. Multiple law enforcement agencies, including Clinton County deputies and the Ohio State Highway Patrol, escalated a high-speed chase through several counties. The chase ended in Clinton County when officers deployed stop sticks near the intersection of OH-134 and Patch Road in Clark Township. Metcalfe allegedly exited the vehicle with a weapon. Officers engaged him in a shootout during which he was killed. The Montgomery County Coroner's Office later determined that the gunshot wound was self-inflicted.

On October 1, 2021, officers from multiple agencies, including the Hamilton County Sheriff's Office, Chattanooga Police Department, U.S. Marshals Service, Soddy-Daisy Police Department, Tennessee Highway Patrol, and Tennessee Bureau of Investigation, were searching for 27-year-old James Michael Farris, who had been added to the TBI Most Wanted list earlier that day. Farris was wanted in connection with the murder of Frederick Williams, who was shot on September 29, 2021. Officers located Farris in a wooded area behind the 9700 block of Berry Meadow Way in Soddy-Daisy. He reportedly had a firearm and pointed it at officers, prompting a Hamilton County deputy to fire his weapon, striking Farris. Farris was transported to a Chattanooga hospital, where he died from his injuries. No officers were injured during the incident.

On October 1, 2021, at approximately 11:00 p.m., a Rogersville police officer spotted a truck on Main Street that was the subject of a "Be On the Lookout" (BOLO) alert from Hamblen County. The officer attempted to stop the vehicle, but the driver, 32-year-old Buster Lee Carpenter, made a U-turn and crashed into the side of a house. As the officer approached the truck, Carpenter reversed, striking the officer's vehicle. Carpenter then drove the truck toward the officer, who fired shots into the vehicle, striking Carpenter. Carpenter was transported to a local hospital, where he was pronounced deceased. The officer was not injured during the incident.

The Tennessee Bureau of Investigation (TBI) is conducting an independent investigation into the circumstances leading to the shooting, including collecting evidence and conducting interviews. Findings will be shared with the District Attorney General for further review and consideration. The TBI does not identify the officers involved in these types of incidents and refers questions of that nature to the respective department.

On 10/01/2021, around 11: 45 p.m., a Lakeville police officer observed a motorcycle driving recklessly on Main Street, exceeding the speed limit by traveling at 68 miles per hour in a 40 miles per hour zone. The officer attempted to initiate a traffic stop, but the driver did not comply. After a pursuit, the officer “called off the chase.”. However, shortly after, while still continuing to follow at a distance, the officer found the motorcycle had crashed into a telephone pole just over the Middleboro town line, resulting in the driver's death.

Deputies received a call for a suspicious person outside of a convenience store, which led to a short pursuit around 8 a.m. A deputy who had stopped at the store to take a break was told by a store clerk that the man, Nicholas Norris, who was outside, had been scaring customers over the past few days. Norris fled. A deputy reported that Norris was driving in circles and driving erratically on Potranco Road. The deputies reengaged a pursuit because it was rush hour. Deputy Joseph Trevino shot at the vehicle, killing Norris.
