On April 23, 2021, Yellowstone County Sheriff's Office deputies, alongside a SWAT team, responded to a standoff situation involving 26-year-old Michael Lee McClure in Billings, Montana. The incident began in the late afternoon after McClure barricaded himself inside a residence on Ridgewood Lane South. Law enforcement initiated a standoff that lasted approximately nine hours.Deputies arrived at the scene following reports of an armed individual threatening others. McClure refused to exit the residence or surrender, prompting the deployment of the Yellowstone County SWAT team.
A tactical robot was deployed and multiple drones equipped with thermal imaging and cameras were deployed. SWAT officers fired tear gas and other chemical irritants into the residence to encourage McClure to exit and to limit his ability to maneuver within the house.At approximately 11:00 p.m., SWAT officers breached the residence using a combination of ballistic shields, breaching tools, and tactical flashbangs to disorient McClure. Upon reaching the attic, they encountered McClure, who reportedly charged at officers while attempting to flee.Police engaged in a shootout with him and killed him.

Witnesses reported the driver engaging in dangerous behaviors, including speeding, swerving between lanes, making sudden U-turns, and speeding through town at 60 mph.
The Oregon State Police and Bandon Police Department jointly escalated a pursuit. An Oregon State Police trooper located the vehicle near mile marker 247 on U.S. Highway 101, near Belloni Boys Ranch. A minor collision occurred between the trooper’s vehicle and the red Mitsubishi, prompting the driver to flee southbound on the highway.
The driver then crossed over the double yellow line and collided head-on with a vehicle in the opposite lane.

On 04/21/2021, an officer from the Wyoming Department of Public Safety noticed a vehicle without a license plate near 44th Street SW and S. Division Avenue in Grand Rapids, which matched the description of a stolen car reported earlier that evening. The Wyoming Department of Public Safety is under the city of Wyoming, Michigan, not the state of Wyoming. The officer attempted to pursue Shynia Jones, who was driving the vehicle, but she did not stop. Shortly after, another officer spotted Jones' car and escalated a chase to between 80 and 90 mph. The officer began a pursuit but allegedly “called off the chase” moments before she crashed into another vehicle, then a boulevard median at high speed, became airborne, lost control, and collided with a light pole on the east side of the Street, killing her.

On April 21, 2021, deputies from the Pasquotank County Sheriff's Office shot and killed Andrew Brown Jr., 42, in the 400 block of Perry Street in Elizabeth City, North Carolina. Deputies were attempting to serve drug-related warrants when Brown allegedly tried to escape by driving in the direction of officers.

Brown was fatally shot, reportedly in the head, as he attempted to flee in his vehicle. The incident prompted protests and widespread public scrutiny. The family and community believe that Brown was trying to escape, and that the deputies violated the use of force policy by shooting at a moving vehicle. Despite the release of body camera footage, which showed Brown driving toward deputies, the family maintains their claim that the shooting was unjustified.
At the time of the shooting, the deputies were reinstated after an investigation, with one officer later retiring. Brown's family reached a $3 million settlement with the Pasquotank County Sheriff's Office but continues to seek full accountability. On 04/21/2021, around 12: 05 a.m., Worcester Police Department officers encountered Phet Gouvonvong at 265 Grafton St in Worcester. Gouvonvong was wearing body armor, appeared to be armed with what looked like an assault rifle, and had an explosive device with wires attached, along with carrying a backpack. During the encounter, Gouvonvong reportedly became agitated and made a move toward the police. Police shot and killed him.

On 04/20/2021, officers from the Detroit Police Department responded to an incident at E 8 Mile Road and HannaStreet in Detroit involving a man in a mental health crisis. During the encounter, the man reportedly stabbed an officer in the leg with a knife. In the ensuing chaos, another officer accidentally shot the injured officer in the hand. On 04/20/2021, a 22-year-old male driving a Dodge Charger was allegedly chased at high speed by officers of the Grosse Pointe Park Police Department when he lost control of his vehicle at the intersection of ConnerStreet and E Jefferson Avenue in Detroit, landed under the trailer of a semi-truck, killing him. On April 20, 2021, Columbus Police Officer Nicholas Reardon responded to a report of a fight outside a residence on the 3100 block of Legion Lane in Columbus, Ohio. Upon arriving at the scene, Reardon observed 16-year-old Ma'Khia Bryant holding a knife and charging toward another individual.

Reardon fired four shots, striking and killing Bryant. The incident was captured on body camera footage, which showed the moments leading up to and including the shooting.

On 04/18/2021, an incident involving five people being shot and wounded occurred in Detroit, Michigan. A person who was subsequently struck by a car nearby and died was believed to have been involved in this shooting. Following the initial incident, police initiated a pursuit of a black GMC driven by a 27-year-old male man traveling north on Gratiot Avenue. The chase continued until the driver reached 8 Mile Road, where he exited his vehicle at a strip mall. While fleeing on foot, the man allegedly fired shots at pursuing officers. Police opened fire, killing him. This event is under internal self-investigation by the Detroit Police Department.

On 04/18/2021, officers from the Burnsville Police Department found Bradley Olsen driving a vehicle with stolen plates when he subsequently crashed. Officers soon received a report that he was involved in a carjacking. Police pursued the carjacked vehicle, and Olsen allegedly fired at pursuing officers before crashing the vehicle and continuing to shoot at police, who then shot and killed him. Officers Dan Anderson, Brent Murray, Paul Oelrich, and Chris Walswick fired their guns.

On April 18, 2021, at approximately 6 p.m., Wayne County Sheriff's deputies attempted to serve outstanding warrants on Donnie Morris Butts Jr., 53, in Goldsboro, North Carolina. Butts fled in a pickup truck, leading deputies on a pursuit. During the chase, Butts' vehicle collided with a deputy's cruiser. A second deputy continued the pursuit. Butts lost control of his truck, ran off the road, and collided with a parked big rig. Butts died at the crash site.

On April 16, 2021, Albuquerque Police Department officers responded to a domestic dispute at a residence on Coyote Canyon Place NW in Albuquerque, New Mexico. Juan James Cordova, 51, was reportedly armed and had fired a shot inside the house and was in mental health crisis per his girlfriend. Officers spent approximately an hour attempting to negotiate with Cordova, urging him to surrender peacefully.

Despite negotiations, Cordova remained uncooperative, holding a firearm and making statements suggesting he intended to provoke a lethal response from the police. At one point, he fired a round, which passed through a window and struck a neighbor's house. When Cordova allegedly pointed his firearm at the officers, Officer Bryce Willsey opened fire and killed him.

Officer Willsey, a member of the Albuquerque Police Department since 2015, had been involved in three prior officer-involved shootings before this incident. In October 2023, Cordova's family filed a lawsuit against the city of Albuquerque and Officer Willsey, alleging excessive force and claiming that Cordova was experiencing a mental health crisis and his death had been avoidable.

On April 15, 2021, at approximately 11:30 p.m., Roswell Police Department officers assisted the Chaves County Sheriff's Office during a pursuit involving a white Ford pickup truck that had fled from a deputy during an attempted traffic stop. Roswell officers deployed two sets of tire deflation devices and missed.
The vehicle then entered the parking lot of a bank at 1901 N Main St in Roswell, New Mexico. The driver, identified as 22-year-old Victor Ivan Barron, exited the truck holding a firearm and attempted to run away on foot. Officers Danny Flott and Abrem Atencio, both with the Roswell Police Department, along with Chaves County Sheriff’s Deputy Samuel Lueras, each discharged their firearms, striking and killing Barron.

On April 15, 2021, police conducted a welfare check at 1:16 p.m. where they discovered a 64-year-old woman deceased from strangulation and multiple blunt force injuries. Two individuals, Henry Porter, 22, and Erika Miller, 28, were identified for questioning in connection with this death. Later that evening, around 10:30 p.m., the Gallatin County Sheriff's Office, along with other agencies, pursued the vehicle associated with these individuals. After the vehicle was stopped near the intersection of Highway 287 and Highway 191, Porter and Miller fled on foot into a wooded area. Deputies arrived at the scene and made contact with Wood.

On April 13, 2021, Pennsylvania State Police responded to a domestic violence call in Saltlick Township, Fayette County. A woman had fled the residence and reported being assaulted and strangled by her husband, Keith Robert Tafoya. On 04/11/2021, officers from the Brooklyn Center Police Department pulled over Daunte Wright for an alleged traffic violation related to expired registration tags. At around 11:01 p.m., officers located a vehicle matching this description on Harps Street near Fourth Street, with Guillermo "Memo" Amezcua, 48, seated in the driver’s seat. Officers issued multiple commands over approximately five minutes for Amezcua to show his hands. Police alleged that Amezcua was uncooperative and observed manipulating an unknown object.
