Officers responded to a ShotSpotter activation on Lyell Avenue around 4:21 a.m. Surveillance cameras captured a vehicle leaving the location. The vehicle was later spotted on Glasser Street, where officers attempted a traffic stop. During the stop, officers claimed that Mark D. Gaskill was holding a handgun. Two Rochester police officers then shot and killed Gaskill.

On 05/14/2021, at approximately 3:25 p.m., deputies from the Pennington County Sheriff's Office responded to a call about an intoxicated and out-of-control family member at a residence on Radar Hill Road. Upon arrival, deputies confronted a man, later identified as Ty Quinton Stilwell, who was carrying a long gun. Deputies ordered him to drop the weapon, but Stilwell reportedly leveled the gun in their direction and ignored their commands. In response to the perceived threat, the deputies used deadly force, shooting and killing Stilwell.

Adrian and Hector Murillo were being followed by undercover officers, who attempted to stop them in Ellis County. The brothers fled, leading to a police pursuit. During the chase, one or both men allegedly fired at the pursuing officers. A constable from Hill County became involved in the pursuit and was shot and wounded. The vehicle driven by the Murillos crashed after being disabled by an officer. Adrian and Hector continued exchanging gunfire with the officers. Eventually, the two men surrendered. Adrian Murillo was killed in the exchange.

On May 12, 2021, around 5 p.m., a 52-year-old man was reportedly barricaded inside a vehicle near 173rd Street South and A Street South in Spanaway, Washington. During the standoff, the man allegedly fired a shot at a deputy, though no injuries were reported. Shortly after, the man reportedly shot and killed himself.

Troopers were dispatched to locate Philip Stanley Watros, who was reportedly armed and making homicidal and suicidal threats to an estranged girlfriend. Officers located him and his vehicle at Mexico Community Park. Due to the nature of the threats and concerns for public safety, officers established a perimeter around the area. After a 12-hour standoff, Watros reportedly shot and killed himself with a rifle.

On May 10, 2021, at approximately 2:00 a.m., a Louisiana State Police trooper observed a Ford Explorer traveling northbound on U.S. Highway 167 near LA Highway 749 that was driving too fast. The trooper escalated pursuit; the driver did not stop. The trooper allegedly "called off the chase" due to inclement weather and high speeds, just before the Explorer veered off the roadway, struck a parked vehicle, overturned, and hit a utility pole. The Explorer was occupied by four juveniles, including 15-year-old passenger Trashawn Blake Harrison, who was ejected from the vehicle and died. Three other juveniles were injured.

On May 10, 2021, Shakeela Imonai Johnson was reportedly fleeing from a domestic incident in Henrico County. On May 10, 2021, Deputy James T. Webster, 51, was off duty but driving his Jefferson County Sheriff's Department-issued 2016 Ford Explorer westbound on Clifty Drive in Madison, Indiana. Officers responded to a welfare check at the Spanish Trails Inn around 9:45 p.m. on May 10, 2021, after a man was reportedly refusing to let a woman leave. Upon arrival, officers determined Kalon J. Horton might have been under the influence of an unknown substance and needed medical attention. He was taken into custody without a warrant and transferred to a medical center for a mental evaluation. Horton was released by the hospital overnight. Early the next morning, officers were called to a shooting in progress at an apartment where a woman and two children were present. Officers arrived and announced themselves, requesting the man to come outside. After receiving no response, they entered the apartment, where they shot and killed Horton.

On May 9, 2021, Albuquerque Police Department officers were working near Iliff and Coors in Albuquerque, New Mexico, when they were flagged down regarding an individual reportedly throwing rocks at vehicles in a hotel parking lot. On 05/09/2021, officers from the Baltimore County Police Department and firefighters responded to 911 calls about a fire and an armed man at the 7500 block of Maury Road in Windsor Mill at around 6: 40 a.m. Upon arrival, they found Everton Garfield Brown outside the burning residences. On May 7, 2021, at approximately 12:30 a.m., Providence Police officers responded to reports of a man screaming in the street near 345 Angell Street, possibly under the influence of narcotics. Upon arrival, they encountered 34-year-old Joseph L. Ventre Jr., who was rolling on the ground and groaning unintelligibly. Officers attempted to calm Ventre and requested assistance from the Providence Fire Department. Due to his erratic behavior and for the safety of all involved, officers decided to restrain Ventre. Body camera footage shows officers holding Ventredown on his stomach for approximately 90 seconds while handcuffing him. Shortly thereafter, Ventre became unresponsive. Emergency responders initiated CPR and transported him to Rhode Island Hospital, where he was pronounced dead at 1:41 a.m. The Rhode Island Medical Examiner later determined that Ventre's death was due to the combined toxic effects of methamphetamine and methadone during police restraint.

On May 7, 2021, law enforcement officers from the Carteret County Sheriff's Office, Morehead City Police Department, and North Carolina State Patrol pursued Richard Kelly Murphrey, 35, who was wanted on several outstanding charges such as carrying a concealed weapon, and numerous drug-related offenses.

Initial contact was made with the man via phone, during which a single gunshot was heard, and communication was subsequently lost. On May 6, 2021, at approximately 3:54 a.m., officers from the Casper Police Department attempted to conduct a traffic stop on a vehicle. As officers approached, the driver exited the vehicle, and the passenger moved into the driver's seat and allegedly attempted to flee. One officer contacted the driver, while the other officer tried to stop the fleeing vehicle. The vehicle entered I-25, heading into oncoming traffic lanes. The officer, still inside the vehicle with the driver, continued to order the driver to stop. At this point, the officer shot and killed the driver, reportedly stopping the vehicle along the side of the Interstate.

On May 5, 2021, Winston-Salem Police officers responded to reports of a violent altercation at a residence in the 500 block of Lockland Avenue. Edwin "Joey" Joseph Castillo Jr., 34, was involved in chaotic interactions with housemates and police.
Castillo allegedly struck his roommate in the head with a hammer and fired shots at officers. Police opened fire, killing Castillo during the encounter. The Winston-Salem Police Department is investigating the incident and the circumstances leading to the use of deadly force.

On May 3, 2021, 32-year-old Brandon Lee Shockley Sr. was pursued by law enforcement officers from Indiana into Illinois. Shockley was wanted for attempted homicide.

The pursuit began on I-70 near the Indiana-Illinois state line, where Indiana State Police deployed stop sticks, causing Shockley's vehicle to suffer a flat tire. Despite this, he continued driving into Clark County, Illinois, eventually veering off the road and coming to a stop in a ditch near milepost 153.

Officers reported hearing a gunshot from inside the vehicle. In response, a Vigo County Sheriff's Deputy and a West Terre Haute Police Officer fired their weapons at the car. Officers found Shockley deceased in the driver's seat with a firearm present. An autopsy later determined that Shockley died from a self-inflicted gunshot wound.

At around 2:55 a.m., Ford County dispatch sent a be-on-the-lookout for a person wanted in Garden City who allegedly was involved in a stabbing. Gray County deputies located Luis Salgado's vehicle at around 3:17 a.m. The deputies attempted to stop the vehicle, but Luis Salgado continued driving. Deputies escalated a chase into Ford County, where Ford County deputies joined the chase at 3:24 a.m. Spike strips were deployed west of Dodge City, deflating one of the tires. Salgado continued driving east into Dodge City before veering off the roadway, striking a power pole, and rolling several times. He was ejected and died at the crash site.

On 05/02/2021, David Earle Hartenstein, 39, of Fitchburg, Massachusetts, was traveling northbound in the southbound travel lanes on NH-101. Police attempts to stop the wrong-way vehicle, including the use of tire deflation devices, were unsuccessful. Officers from multiple agencies set up rolling roadblocks to prevent other drivers from encountering the vehicle. Near the Route 101 split in Manchester, Hartenstein veered off the roadway and into the woods. The crash resulted in his death.

On May 2, 2021, at 6:18 p.m., Hamilton Township Police officers responded to a residence on the 1200 block of Adams Road in Loveland, Ohio, following a 911 call from a neighbor. The call reported that Anthony Williams, a 58-year-old male, was intoxicated, threatening suicide, and discharging a firearm in his home. On 05/02/2021, around 2 a.m., Joshua Day was crossing North Point Boulevard in Dundalk when he was initially struck by a car. The driver of this car stopped at the scene. Shortly after, Berlynn Matthews, 21, stopped to assist Day. Tragically, off-duty Officer William Collazo-Brown on a motorcycle then struck both individuals, leading to their deaths. Collazo-Brown was subsequently arrested and charged with DUI-related offenses.He was found guilty of criminally negligent manslaughter, driving under the influence, homicide with a motor vehicle, and negligently operating a vehicle under the influence.

He was sentenced to 18 months in jail with additional probation conditions, including home detention, after his release from incarceration.

On May 1, 2021, around 6 p.m., the Porter County Sheriff's Office responded to a 911 call regarding a disturbance at a residence located at 716 Long Run Road in South Haven, Indiana. After several hours of attempting to communicate with a man inside the home without success, officers entered the residence. Inside a bedroom, they encountered Alexander T. Tuzinski, who was reportedly armed with a handgun. Officers shot and killed Tuzinski during the confrontation.

Around 10:50 p.m. on May 1, 2021, officers in an unmarked patrol car attempted to stop Jose Astrain Jr. for an unspecified traffic violation. Astrain refused to stop and sped away at high speed. The officers turned off their emergency lights and did not pursue him. Astrain lost control of his Ford Fusion, fishtailed into oncoming traffic, and crashed into a pickup truck, resulting in his death.

On May 1, 2021, at approximately 7:27 p.m., dispatchers received a report of shots fired at the Oneida Casino in Green Bay, Wisconsin. Bruce K. Pofahl, a former restaurant employee, allegedly approached a waiter station and shot and killed 32-year-old Ian J. Simpson and 35-year-old Jacob T. Bartel. Pofahl then left the restaurant and shot and wounded 28-year-old Dan Mulligan. Green Bay police officers encountered Pofahl near the first floor of the parking ramp east of the Radisson Hotel. Officers Brian Jordan, Makayla Wolfe, and Ben Snyder confronted Pofahl, who was armed, and shot and killed him during the encounter.

Officers from the Miramar Police Department attempted to stop a black Mercedes sedan. The driver, Everette Orette Brown, continued driving and headed south on Southwest 64th Avenue, where the Mercedes struck a pedestrian. Officers pursued the car until it crashed into multiple light poles and a fire hydrant, overturning around 9 p.m. Brown was killed in the crash.

On April 28, 2021, at approximately 5:16 a.m., officers from the Lake Charles Police Department responded to reports of gunfire at a hotel located at 1000 North Lakeshore Drive in Lake Charles, Louisiana. Upon arrival, officers determined that shots had been fired from a hotel room toward Interstate 10, prompting the closure of the highway and diversion of traffic to Interstate 210 for safety reasons.

The individual inside the hotel room, identified as 44-year-old Kyner Gene Rollins of Los Angeles, California, had locked himself in the room. SWAT was called and forced entry into the room. They found Rollins dead from a gunshot wound. The circumstances surrounding his death and who killed him were not disclosed by police.
