On April 14, 2021, Aroostook County Sheriff's deputies responded to a 911 call at approximately 2:58 a.m. that reported a man, later identified as Jacob "Jake" Wood, holding a knife to a woman’s throat at a residence on Scovil Street in Mars Hill, Maine. Deputies arrived at the scene and made contact with Wood.

Despite repeated commands to drop the knife, Wood did not comply. He was reportedly holding a knife to the woman’s throat, and the situation quickly escalated. Deputy Isaac Ward, fearing for the woman’s safety and the potential for further escalation, discharged his firearm, fatally wounding Wood.

The woman was unharmed, and after the shooting, she was able to safely exit the residence. Following the incident, the Maine Attorney General's Office conducted an investigation and determined that the use of deadly force was justified under the circumstances, as Wood did not comply with law enforcement commands and posed a potential threat to both officers and the woman involved.

Around 12:40 a.m., deputies responded to reports of a man, Marcelo Garcia, suffering from a mental health crisis. Family members stated that Garcia had schizophrenia and bipolar disorder. On April 13, 2021, Pennsylvania State Police responded to a domestic violence call in Saltlick Township, Fayette County. A woman had fled the residence and reported being assaulted and strangled by her husband, Keith Robert Tafoya. On 04/11/2021, officers from the Brooklyn Center Police Department pulled over Daunte Wright for an alleged traffic violation related to expired registration tags. On April 10, 2021, at approximately 7:00 p.m., deputies from the Jefferson Parish Sheriff's Office responded to a disturbance call in the 6500 block of Park Manor Drive in Metairie, Louisiana. A woman reported that her boyfriend, 30-year-old Joshua Alexander Mitchell, had sexually abused a child and threatened to kill her. She also informed deputies that Mitchell was armed.

At around 11:01 p.m., officers located a vehicle matching this description on Harps Street near Fourth Street, with Guillermo "Memo" Amezcua, 48, seated in the driver’s seat. Officers issued multiple commands over approximately five minutes for Amezcua to show his hands. Police alleged that Amezcua was uncooperative and observed manipulating an unknown object.

To gain compliance, an officer deployed three less-lethal stun bags aimed at the rear window area of the vehicle, but these attempts were unsuccessful. At approximately 11:06 p.m., police reported observing a muzzle flash consistent with a gunshot from inside the vehicle. Officers opened fire, fatally striking Amezcua, who was pronounced dead at the scene.

The Los Angeles County Sheriff's Department investigated the officer-involved shooting. According to an autopsy report dated 04/14/2021, Amezcua sustained five gunshot wounds, with three being fatal. Two men, a woman, and a child were parked at the entrance of an apartment complex when officers stopped to investigate. On April 7, 2021, around 6:45 p.m., Philadelphia Police officers conducted a traffic stop on a blue Kia Optima in the 1500 block of West Somerville Avenue after it failed to stop at a stop sign. On 04/07/2021, at approximately 5 a.m., officers from the U.S. Pentagon Force Protection Agency responded to a report of shots fired in the parking lot of a condominium complex located at 7333 New Hampshire Avenue in Takoma Park. An off-duty officer, David Hall Dixon, approached them, stating he suspected a car was being burglarized. He engaged with Dominique Williams, 32, and James Lionel Johnson, 38, who did not comply with his commands. Both Williams and Johnson attempted to flee in a vehicle, at which point Dixon shot and killed them. Following the incident, Dixon was charged with second-degree murder.

On April 6, 2021, Oneonta Police Department Sergeant Ralph Pajerski and an unidentified partner responded to a domestic incident at 48 River Street in Oneonta, New York. Upon arrival, they encountered Tyler R. Green, 23, who was allegedly threatening his girlfriend with a knife while holding their two-year-old son. Despite officers' commands to drop the weapon, Green reportedly cut his girlfriend's leg and swung the knife toward the child. In response, Sergeant Pajerski fired two shots, striking Green. Green was transported to Albany Medical Center, where he was pronounced dead.

On 04/05/2021, around 3 p.m., a crash occurred at Union Road and Oak Road in Buena Vista, New Jersey. Franklin Township police officers responded to the scene along with local Buena Vista authorities. During the response, Roy Kenneth Jackel Jr., 41, allegedly entered a Franklin Township police cruiser and drove away. Officers pursued the stolen vehicle until it crashed. On 04/04/2021, Michigan State Police troopers were conducting a welfare check at around 8 p.m. when they discovered a 62-year-old woman dead from an apparent homicide in her home in Lake Township. The house was empty except for the victim at the time of discovery. Through investigation, troopers identified a man, Adam Briggs, a 30-year-old relative of the victim. A broadcast alert was issued for Briggs. Shortly after 1 a.m., the Cadillac Police Department spotted the vehicle associated with Briggs and attempted a traffic stop. Briggs did not stop. Police escalated a with multiple agencies. During the chase, Briggs allegedly fired shots at the pursuing officers. No one was hit. Missaukee County Sheriff's deputies used stop sticks to disable Briggs' vehicle. Briggs then ran away on foot. His body was found shortly after 6 a.m. with a self-inflicted gunshot wound, confirming his death as a suicide.

On April 4, 2021, Michael Ray Wagner, a 47-year-old white male from Geneva, Indiana, was killed during a police chase in Shelbyville, Kentucky. After reports that Wagner had stopped his vehicle on the roadway and pointed a gun at another motorist, officers located him. He drove eastbound on Frankfort Road (US-60), and police escalated a high-speed chase. Approximately two to three miles into the pursuit, Wagner lost control of his 1999 Pontiac Firebird near the intersection of US-60 and Ski Area Road. He crashed, was ejected, and was pronounced deceased at the scene by the Shelby County Coroner.

Gabriel Casso, 21, was running toward New York City Police officers while being pursued by two unidentified men near East 166th Street and Findlay Avenue in the Bronx on April 3, 2021. Police opened fire during the encounter, resulting in Casso being fatally shot. Around 3:30 a.m., Corvallis Police responded to a call from the Days Inn motel staff reporting a man acting aggressively with a knife. Officers encountered Jeffrey W. Appelt, 32, who reportedly had a knife. An officer fired a Taser, but Appelt retreated toward the motel entrance. A second officer attempted to take him into custody, but Appelt evaded the officer. As a third officer arrived, Appelt continued approaching while holding the knife. Officer Daniel Granillo then shot and killed Appelt.
