Patrick Enos Jr.
Age :28
Gender :Male
Race : Native Pacific Islander
Date : 12/20/2021
Location : Highway 11 near Kipimana Street
City : Keaʻau
County : Hawai‘i
State : Hawaii
Agency : Hawaiʻi Police Department
Officer(s) : Not Disclosed
Cause of Death : Vehicle
Event : Killed in crash during alleged police pursuit
OPEN RECORDS SUMMARY :
.
REASON WHY PURSUIT WAS INITIATED:
The precise reason police allegedly initiated the pursuit has not been publicly disclosed. Enos’s sister said he had an outstanding bench warrant after missing a court date.
.
EVENT NARRATIVE:
On 12/20/2021, 28-year-old Patrick Enos Jr. was driving on Highway 11 in the Keaʻau area of Hawaiʻi Island when, according to his family and at least one witness who later spoke with them, Hawaiʻi Police Department officers allegedly pursued his vehicle. The existence and circumstances of the alleged pursuit were not disclosed in the original public police account of the fatal crash.
.
According to the publicly released police account, Enos was speeding southbound on Highway 11 when he entered the intersection near Kipimana Street against a red traffic signal and collided with another vehicle. Enos died in the crash. Nearly four years later, reporting by Hawaii News Now revealed that his family had been told by at least one witness that police were chasing Enos before the collision. The family maintains that the alleged pursuit was a major contributing factor in his death.
.
The original public reporting therefore described Enos as a speeding driver who ran a red light and crashed, without disclosing the family's allegation that his driving occurred during an active police pursuit. By November 2025, the Hawaiʻi Police Department declined to answer questions about the alleged pursuit because litigation was ongoing, while Enos's attorney said investigators had provided the family and legal team with few details about what police did before the crash.
.
Enos was a father of four. His family described him as someone who enjoyed hunting, fishing and repairing cars and who routinely helped other people even when he and his own family were struggling.
.
TOTAL FATALITIES/INJURIES:
1 Fatality. Injuries to the driver of the other vehicle were not established in the available reporting.
.
EYEWITNESS OR FAMILY STATEMENTS:
Enos's family said at least one witness told them that police were pursuing him before the fatal collision. His older sister, Patty Paiva, acknowledged that Enos should not have driven away from police but disputed the necessity of continuing a dangerous chase. The family questioned whether officers should have terminated the pursuit before it reached the circumstances that resulted in the fatal crash.
.
Attorney Eric Seitz, representing the family, said the legal team still lacked critical information concerning why officers allegedly pursued Enos and why they continued the chase. Seitz specifically raised the issue of police pursuit policy, stating that officers should terminate a pursuit when continuing it creates excessive danger.
.
DISPOSITION:
The circumstances and police conduct preceding Enos's death remained under investigation and litigation as of November 2025. Hawaiʻi Police Department declined to discuss the case because of the pending lawsuit. The Hawaiʻi Attorney General's Office had also been asked for information concerning its investigation, but no final public determination concerning police responsibility was reported.
.
LITIGATION:
Enos's family filed litigation alleging that police conduct contributed to his fatal crash. Attorney Eric Seitz represented the family. As of November 2025, the lawsuit remained pending, with Seitz stating that the family wanted the matter moved toward trial or settlement and sought evidence necessary to determine whether police were legally responsible for Enos's death.

Linda Jozefiak
Age :58
Gender :Female
Race : White
Date : 12/20/2021
Location : Cass Avenue and South Rose Street
City : Mt. Clemens
County : Macomb
State : Michigan
Agency : Macomb Auto Theft Squad / Macomb County Sheriff’s Office
Officer(s) : Unknown
Cause of Death : Vehicle
Event : Bystander killed by police chase crash
OPEN RECORDS SUMMARY :
.
REASON WHY PURSUIT WAS INITIATED:
Macomb Auto Theft Squad officers located a 2014 Dodge Ram pickup that had been reported stolen earlier that morning in Fraser. Officers established surveillance and attempted to arrest 41-year-old Vertese Steven Woods when he entered the truck.
.
EVENT NARRATIVE:
On 12/20/2021, at approximately 1:23 p.m., Macomb Auto Theft Squad officers were conducting surveillance of the stolen Dodge Ram parked on Main Street in Mount Clemens. When Woods entered the truck and started it, an officer moved an undercover police vehicle behind the truck to block it while another officer activated emergency lights, exited and ordered Woods to stop. Woods reversed the pickup into the undercover police vehicle, pushed it backward, drove over a curb and struck a parking meter before continuing away at high speed.
.
As Woods drove over the curb, a pedestrian had to move out of the truck’s path and several people sitting at a picnic table or bench were also narrowly missed. Police lost sight of the truck and began searching the surrounding area. Authorities subsequently received reports that the pickup was being driven recklessly westbound on Hubbard Avenue.
.
At approximately 1:26 p.m., only about three minutes after police attempted to arrest Woods, 911 callers reported a rollover crash at Cass Avenue and South Rose Street. Woods was traveling at approximately 80 mph when he ran a stop sign and struck the driver’s side of a Ford Fusion driven eastbound on Cass Avenue by 58-year-old Linda Jozefiak. The impact pushed Jozefiak’s car southeast, where it struck a curb and rolled onto its passenger side. The Dodge Ram also rolled, struck a utility pole and came to rest on its driver’s side.
.
Jozefiak, a Harrison Township resident, was transported to McLaren Macomb Hospital, where she died from her injuries. Woods climbed from the crashed pickup and ran from the scene. Prosecutors alleged that he discarded his jacket and stole a child’s bicycle while attempting to escape before police arrested him.
.
Jozefiak was remembered by people who knew her as a thoughtful and generous woman who regularly helped others. Harrison Township firefighters regarded her as part of their extended family, and her family requested memorial donations to the Macomb County Food Program.
.
TOTAL FATALITIES/INJURIES:
1 Fatality. No additional crash injuries were reported.
.
DISPOSITION:
Woods was initially charged with 16 counts, including second-degree murder and first-degree fleeing police. Following a four-day jury trial, on 10/06/2023 a Macomb County jury convicted Woods of involuntary manslaughter, first-degree fleeing and eluding, operating with a suspended license causing death, receiving and concealing a stolen vehicle, failure to stop at the scene of an accident causing death, two counts of assault with a dangerous weapon, moving violation causing death, malicious destruction of property and larceny.
.
Woods was sentenced to concurrent prison terms of 10 to 15 years for involuntary manslaughter, first-degree fleeing and eluding, operating with a suspended license causing death and failure to stop at the scene of an accident causing death, with additional concurrent sentences on the remaining convictions. On 02/26/2026, the Michigan Court of Appeals affirmed his convictions and sentences.
.

Chatuma M. Crawford III
Age :20
Gender :Male
Race : Black
Date : 12/17/2021
Location : Northern Boulevard just south of Totman Road
City : Cicero
County : Onondaga
State : New York
Agency : Town of Cicero Police Department
Officer(s) : Michael Aregano
Cause of Death : Vehicle
Event : Pedestrian bystander killed by off-duty police officer vehicle
OPEN RECORDS SUMMARY :
.
REASON WHY POLICE VEHICLE WAS IN MOTION:
No police activity was involved. Officer Michael Aregano was off duty and driving his fiancée’s personal Toyota Highlander to dinner in Syracuse. Although his final patrol shift had occurred on 10/28/2021, he remained employed as a Cicero Police Department patrol officer until his retirement on 12/31/2021.
.
EVENT NARRATIVE:
On 12/17/2021, at approximately 7:08 p.m., 20-year-old Chatuma M. Crawford III was on Northern Boulevard near Totman Road in Cicero. Crawford and his girlfriend had attended a birthday party together earlier that evening. After an argument, Crawford left their vehicle and walked south along Northern Boulevard. According to his girlfriend, Crawford threw his cellphone several times while walking; the final time, it traveled across the southbound lanes, and Crawford entered the roadway to retrieve it.
.
Aregano was driving south in the left lane of Northern Boulevard with his fiancée as a passenger. The roadway was dark and had no streetlights. Aregano told investigators that he noticed a stopped vehicle on the shoulder but did not see Crawford before striking him with the SUV. A motorist traveling directly behind Aregano estimated that both vehicles were traveling approximately 50 mph; the speed limit at the collision site was 55 mph.
.
Aregano struck Crawford in the southbound left lane just south of Totman Road. Crawford's girlfriend said she had seen Crawford near the yellow line by the median immediately before the collision. She found him unconscious and unresponsive after the impact. Aregano stopped, called 911 and reported that he had struck a person. Emergency responders attempted lifesaving measures, but Crawford died at the scene.
.
The New York State Police reconstructed the collision. Investigators found no evidence that Aregano was speeding, impaired or distracted. The reconstruction and witness evidence indicated that darkness, the absence of street lighting and Crawford's dark clothing substantially limited visibility. The medical examiner concluded that Crawford was probably bent over when he was struck, consistent with the account that he was attempting to retrieve his cellphone.
.
TOTAL FATALITIES/INJURIES:
1 Fatality, 0 reported injuries.
.
DISPOSITION:
The New York Attorney General's Office of Special Investigation investigated Aregano's role in Crawford's death because New York law requires review when a police officer, including an off-duty officer, may have caused a person's death. OSI reviewed eyewitness accounts, physical evidence, crash reconstruction, medical evidence and other investigative material. On 12/29/2022, OSI concluded that the evidence did not establish criminal conduct by Aregano and declined to file criminal charges.
.
LITIGATION:
None found.
