Officers and deputies spotted a car traveling along the Appalachian Highway that was the subject of a BOLO (Be On the Lookout) in connection to a robbery that occurred about 7 p.m. in Jacksboro. Officers escalated a pursuit onto the freeway, I-75, and chased the vehicle at high speed until the driver crashed into several vehicles stopped in a construction zone, including a cruiser driven by a trooper with the Tennessee Highway Patrol who was parked behind a disabled vehicle on the highway. During reported efforts to take the driver into custody, a Caryville officer and a Campbell County deputy shot and killed him.

On October 21, 2021, Memphis and Bartlett police attempted to stop a teenage driver with active warrants for aggravated assault. The driver briefly stopped but then hit a Bartlett squad car while fleeing. Memphis police pursued the vehicle but alleged that they lost sight of him on Elvis Presley Blvd. near Winchester. As the driver approached Raines Road from Elvis Presley Blvd., he crashed into another vehicle driven by bystander Marvin Honey who was on his way to the grocery store, and after the massive crash was transported to the hospital where he died from his injuries.

On October 21, 2021, at approximately 9:30 a.m., FBI agents from the Portland Field Office executed a search warrant at a residence in the 6100 block of Northeast 14th Avenue in Portland, Oregon, as part of a federal child exploitation investigation. During the operation, a male occupant of the home barricaded himself inside. The FBI was assisted by the Portland Police Bureau's Special Emergency Reaction Team (SERT), Crisis Negotiation Team, Explosives Disposal Unit, and patrol officers from the North Precinct. After entering the residence, officers discovered the individual deceased. No further details have been released.

On 10/20/2021, officers assigned to a fugitive apprehension task force with the Colorado Department of Corrections attempted to arrest Aaron Leo Lang in Aurora, Colorado for an alleged parole violation. During the attempted arrest near South Parker Road, officers reported that Lang appeared to raise or aim a weapon toward them. Officers opened fire, striking Lang. He was pronounced dead at the scene. The shooting was referred for independent investigation and remained under review at the time of reporting.

On 10/20/2021, Atlanta police responded around 3:14 a.m. to reports of gunfire at the Atlantic House Midtown apartments on West Peachtree Street. Police and investigators reported that Jarvis Jarrette shot and killed 31 year old Courtney Cox inside an apartment before officers reached the unit. Officers traced the gunfire to an upper floor and encountered Jarrette on a balcony. Investigators said Jarrette fired a rifle toward responding officers. Officers opened fire, and Jarrette was found dead on the balcony. The Georgia Bureau of Investigation investigated the police shooting, and Atlanta police investigated Cox’s death.

On October 20, 2021, officers from the Syracuse Police Department responded to a residence at 216 Ulster Street in Syracuse, New York, following reports of a disturbance. attempted to communicate with Allison Lakie, 33, who was inside the home.

Around 2:30 p.m., dispatch received a call from a passerby who said an SUV was stopped in the middle of the road, and the driver appeared to be slumped over the wheel. Two Arlington officers responded to the call. One of the officers tried to talk to the driver and told him to stay where he was. The driver fled. The driver stopped briefly, at which point one of the officers told him he was under arrest and that he should shut off his vehicle. The driver again fled. The officers again pursued. The driver drove to a cul-de-sac and made a U-turn and allegedly began driving in the direction of one of the officers, who had stopped, exited his vehicle, and was standing near his open door. As the vehicle moved towards him, the officer shot and killed him. Officer Robert Phillips was fired from the agency two days later for violations of policy.

On 10/20/2021, Pennsylvania State Police and Pocono Township police pursued Betty Jane Tibaldi, 54, after officers attempted to contact her outside a closed business and she drove away. Police disabled the vehicle with stop sticks along PA-611 near Shine Hill Road, leading to an hours-long armed standoff involving negotiators and the state police Special Emergency Response Team. Tibaldi remained inside the vehicle with her husband while displaying signs of severe mental distress. After her husband surrendered and left a handgun in the vehicle, Tibaldi opened the door and fired a shot toward troopers. Police returned fire, striking and killing her at the scene. The Monroe County District Attorney’s Office conducted the investigation.

At about 5:30 p.m., police received information that a person wanted on several warrants, including grand theft, auto theft, aggravated fleeing and eluding, and driving with a suspended license, was in the area. Officers arrived and found the person inside his vehicle. Police blocked the vehicle, at which point the person drew a handgun. An officer fired a single shot, missing him. After the shot was fired, the person pointed the gun at his head and shot and killed himself.

On October 20, 2021, around 11:37 p.m., a Missoula Police Department officer observed a white Honda Civic speeding at approximately 60 to 70 mph in a 25-mph zone. An attempt to stop the vehicle led to a pursuit after the driver, Kemari D. Parks, aged 20, made a quick turn and initially evaded the officer. The vehicle was later spotted, and as police approached, Parks accelerated through a parking lot, with a second person, J.M., in the passenger seat. Police continue to chase including into oncoming traffic and through red lights, until Parks crashed into a parked car, leading to further collisions. J.M., the passenger, died in the crash.

Officers from the Falmouth Police Department responded to an incident at the intersection of Middle Road and Lunt Road in Falmouth, Maine. On October 19, 2021, at approximately 11:40 p.m., Suffolk County Police officers in a marked police cruiser attempted a traffic stop on a 2004 Toyota Tacoma with stolen license plates. The alleged driver, Robert Hengeveld, 50, failed to comply and fled from officers, leading to a pursuit.

During the chase, the Toyota Tacoma collided with a Honda at the intersection of North Ocean Avenue and Expressway Drive South in Holtsville, New York. The driver of the Honda, 19-year-old Starlin Manuel Diaz Felipe, was killed in the crash. Hengeveld was arrested and charged with reckless driving, unlawfully fleeing a police officer, criminal possession of stolen property, and multiple bench warrants.

On 10/19/2021, Georgia State Patrol troopers attempted to stop Shavoz Levon Seals near Abernathy Road NW and Brandon Mill Road while police were trying to arrest him on active warrants tied to a murder investigation. Seals did not stop and continued driving. Troopers performed a PIT maneuver near Brandon Mill Road and Brandon Ridge Drive to stop the vehicle. Authorities reported Seals then shot himself inside the vehicle. Police transported him to a hospital where he died, and investigators reported finding a firearm in the vehicle.

The vehicle, occupied by Betty Jane Tibaldi and her husband, Nello Tibaldi, did not stop. Police escalated a chase, deploying spike strips that disabled the vehicle on Route 611 near Shine Hill Rd. The couple refused to exit the vehicle, and officers learned they had a gun. The Pennsylvania State Police Special Emergency Response Team (SERT) was called to assist. On October 19, 2021, Indiana State Troopers and Evansville Police responded to a residence at 1801 Stinson Avenue following a report of a woman discovering a man’s body inside the home. Upon arrival, officers found 36-year-old Heidi Carter standing outside near a vehicle. Carter informed officers there were additional individuals inside the home and stated that she was armed.

Police took Carter into custody and surrounded the residence, ordering all occupants to exit. Carey Hammond, 46, reportedly rushed out of the home holding what appeared to be a firearm. Officers opened fire, shooting and killing Hammond.

Around 7 p.m., deputies were searching for a homicide suspect when they began speaking with a man unrelated to the case. The man reached inside his waistband, prompting a scuffle between him and a deputy. During the struggle, the man retrieved a weapon and shot the deputy in the leg. The deputy then shot and killed the man.

On October 19, 2021, officers from the Reno Police Department's repeat offenders’ program were searching for Richard "Ricky" Thomas, who had an outstanding felony warrant. They located his red truck at a gas station on Golden Valley Drive. As the truck began to move, three unmarked police vehicles surrounded it. Officers attempted to remove Thomas from the vehicle, but he allegedly refused to comply and tried to drive away. During this attempt, one officer was pinned by the driver's door, and another was caught by the closing passenger door as the truck moved off. Both officers involved opened fire, killing Thomas.

On 10/18/2021, Metropolitan Police Department officers went to an apartment building on Congress Street Southeast in Washington, DC to serve a temporary protection order on Deandre Johnson and address a prior reported assault. Officers made contact with Johnson inside the building and attempted to place him under arrest. During the attempt to handcuff him, a physical struggle occurred. Police stated they believed Johnson was attempting to gain control of an officer’s firearm. Officers opened fire and shot Johnson. He was pronounced dead at the scene. On October 18, 2021, a Rochester Police Department officer observed a stolen vehicle on Bartlett Street in Rochester, New York. The officer reportedly did not activate emergency lights or engage in a pursuit, but the vehicle left the area.

On 10/18/2021, Pennsylvania State Police responded to a welfare check at a residence on the 200 block of Burket Drive in Bloomfield Township where Jacob Curry, 44, barricaded himself inside and fired a gun toward troopers. Police established a perimeter and deployed the Special Emergency Response Team as the situation escalated into an armed standoff. Reporting states Curry continued firing from the residence, and troopers opened fire, striking him. He died at the scene. The Bedford County District Attorney and coroner assisted with the response and the incident was reviewed by investigators.

On October 18, 2021, Shawn Delemore Dempsey was pursued by Mingo County Sheriff's deputies following the discovery of his parents, Shawn Dempsey, 52, and Karen Sue Dempsey, 52, who were found dead. On 10/17/2021, Springfield police officers encountered Douglas Knakmuhs at a gas station in Springfield, Missouri while attempting to make contact in connection with outstanding warrants. Police reported that gunfire occurred and Knakmuhs left the location. Officers later located him at a nearby residence where he entered the home and occupants were inside. A tactical response was initiated. During the police entry, officers encountered Knakmuhs inside the residence and a confrontation occurred. Police opened fire and fatally shot Knakmuhs at the scene. On 10/17/2021, Fort Smith police officers responded to reports of a fight involving weapons at a residence on Tilles Avenue in Fort Smith, Arkansas. Upon arrival, an officer encountered Christofer Conner and observed him striking a child with a brick. The officer ordered Conner to stop, and Conner initially complied. When the officer attempted to place Conner in handcuffs, police reported that Conner produced a sharp object and injured the officer. The officer opened fire, striking Conner. At approximately 2:52 p.m. on October 17, 2021, a Kentucky State Police trooper observed a Yamaha motorcycle traveling eastbound on the Western Kentucky Parkway near the 132-mile marker. The trooper initiated a traffic stop, but the motorcyclist, later identified as 36-year-old Michael C. Beaver, did not stop and continued eastbound. Police escalated a chase. Beaver entered I-65 northbound at the 91-mile marker and continued for approximately 3 miles with police in hot pursuit. He exited I-65 at the 94-mile marker, lost control of the motorcycle on the offramp, and crashed. He was transported to a local hospital, where he was pronounced deceased. Deputies with the Clark County Sheriff’s Office located Kfin Karuo’s white Ford Expedition in a parking lot and attempted to arrest him for allegedly pointing a gun at a man on September 29. Karuo drove away, and deputies escalated a high-speed pursuit reaching 70 to 80 mph. Deputy Forrest Gonzalez performed a PIT maneuver, spinning Karuo’s SUV onto a berm. As Karuo began to climb out the driver’s window, Gonzalez saw a handgun in Karuo’s hand and ordered him to drop the weapon. Karuo pointed the gun at Gonzalez, who opened fire while backing away. Karuo ran toward a mobile home park, then turned and again pointed the gun at Gonzalez. Gonzalez fired more rounds. Deputy David Delin, who had arrived at the scene, also opened fire when Karuo appeared to square off. Deputies later found Karuo dead in a driveway with a loaded 9mm Glock in his hand. Gonzalez fired 10 rounds, and Delin fired 8. At least seven bullets struck a nearby occupied home, but no civilians were injured. The fatal shot struck Karuo in the torso. Deputy Forrest Gonzalez had previously been involved in two other fatal shootings.

About 4 a.m., officers responded to a report of shots fired. Officers arrived at the location and began to canvass the vicinity for a maroon sport utility vehicle firing a weapon. An officer spotted a maroon Lincoln SUV with paper plates and conducted a traffic stop along with assisting officers. Guillermo Gonzalez-Gonzalez was the lone occupant. The officers shot and killed Gonzalez. Gonzalez reportedly had two guns with him at the time of the incident: a semiautomatic handgun and a long rifle.

On 10/16/2021, the Minnesota State Patrol responded to an incident where Cody Wayne Pirila allegedly killed his roommate, Skylar Cody Hunter, 31, and roommate's father, Jamie Allen Hunter, 51. After the killings, Pirila took his own life following a police chase.

Joshua Dewayne Hammock allegedly robbed a convenience store in Lee County while displaying a knife around 11:20 p.m. Lee County deputies began searching for him in response to the theft. Around 7:30 a.m., another robbery took place at a 7/11, where Hammock allegedly stole a case of beer and flashed a knife before charging at the store clerk. When two responding deputies found him, Hammock allegedly tried to attack them with the knife, prompting one of the deputies to shoot and kill him.

Alexander King was carrying a pellet gun, which he allegedly pointed at officers. Police stated the pellet gun resembled a rifle, with only a faded orange tip that was not visible from a distance at night. The names of the two officers who shot King, along with five others present who did not fire, are being withheld under Marsy's Law.

On 10/15/2021, Milpitas police officers located Michael Edward Nelson Jr. inside a reported stolen vehicle at the Milpitas Square Shopping Center on Barber Lane in Milpitas, California. Officers approached the vehicle and Nelson exited briefly, looked toward officers, and then re entered the vehicle. Police reported that Nelson then fired a weapon toward officers from inside the vehicle. Officers opened fire, striking Nelson. He was pronounced dead at the scene. The incident was referred for independent investigation.

On October 14, 2021, at approximately 2 p.m., Edgecombe County Sheriff’s Office deputies responded to a residence on McNair Road in Tarboro, North Carolina, after receiving a report that 32-year-old Jermaine Harris was chasing a family member with a knife. Upon arrival, Harris lunged at one deputy and stabbed him twice in the back. A second deputy, who arrived shortly afterward, discharged their firearm, fatally striking Harris. The injured deputy was transported to Vidant Medical Center in Greenville, where they were treated and later released.

At approximately 12:00 p.m. on October 14, 2021, the Trigg County Sheriff's Department responded to a disturbance on South Road in Cadiz, Kentucky, where 48-year-old Harold Floyd Jett allegedly fired a shot at a vehicle during an altercation. Jett drove away. Officers escalated a chase at high speeds that continued into the city of Cadiz. The chase ended at Hilltop Street, where Jett entered the residence of his aunt, 71-year-old Mary Dullenty.

On 10/14/2021, Crestview police officers responded to a residence in the 300 block of Hospital Drive after a neighbor reported hearing calls for help. Officers encountered Calvin Wilks Jr. inside the residence and attempted to have him step outside. When that did not occur, officers attempted to detain him. During the encounter, officers deployed a Taser and physically restrained Wilks. Wilks later became unresponsive and died. A medical examiner later cited restraint and Taser use as contributing factors. One Crestview police officer was charged with manslaughter in connection with Wilks’s death. The charge was later dismissed. Wilks’s family filed a civil lawsuit related to the incident.

The driver continued driving and police chased him through local roads. During the encounter, deputies reported gunfire involving the motorcycle. Deputies opened fire. The motorcycle then crashed, and Robert Allen Maness was pronounced dead at the scene. The South Carolina Law Enforcement Division conducted the investigation. The case later resulted in a civil lawsuit, which was resolved with a financial settlement to Maness’s family.
