On 02/22/2021, at approximately 2:45 a.m., Escondido Police Department officers responded to a domestic violence call in the 1300 block of Morning View Drive. A woman reported an altercation with Anastasio Rosas Jr., the father of her two children, and told police that Rosas had left with a high-powered rifle.
Shortly after 3 a.m., officers located Rosas driving near West Lincoln Avenue and Morning View Drive. Police alleged that Rosas quickly got out of the vehicle and fired at a police car, with a bullet passing through the driver's door and striking a police sergeant who was seated inside. Police did not fire at Rosas during that encounter. Rosas ran from the area.
Police issued a countywide request for assistance and conducted a large-scale search involving Escondido police, other area law-enforcement agencies, a sheriff's helicopter, drones and tactical personnel. Freeway ramps and surrounding streets were closed during the search.
At approximately 7:45 a.m., a maintenance worker reported seeing Rosas inside a vacant apartment in the 700 block of West Lincoln Avenue. As tactical personnel responded, they heard a gunshot from inside the apartment. Police deployed a robot and found Rosas unresponsive with a gunshot wound to the head. He died from a self-inflicted gunshot wound.
A later San Diego County Citizens' Law Enforcement Review Board investigation confirmed that Rosas died from a self-inflicted gunshot wound to the head and reviewed the San Diego County Sheriff's Department's participation in the search and barricade response.
OFFICER BACKGROUND:

NARRATIVE:
On 02/16/2021, 25-year-old Ivan Sandoval was killed when a stolen vehicle he was driving crashed after a San Pablo Police Department pursuit in Contra Costa County.
At approximately 7:59 a.m., San Pablo police were conducting surveillance on an unoccupied stolen vehicle in the parking lot of a business at 2701 El Portal. The vehicle's license plate had been detected by the city's automated license-plate reader system, generating an alert to San Pablo police dispatchers.
Officers watched Sandoval leave the business, enter the vehicle and drive from the parking lot. Police followed the vehicle and attempted a traffic stop by activating their emergency lights. Sandoval continued driving. Officers then activated their sirens, and Sandoval accelerated away.
The pursuit continued north on San Pablo Avenue toward Richmond Parkway. As Sandoval approached the heavily traveled intersection of San Pablo Avenue and Richmond Parkway at high speed, San Pablo police determined that continuing the pursuit was unsafe and terminated it.
Sandoval continued accelerating north through the intersection. He lost control of the vehicle, entered an uncontrolled skid, crossed the center median and traveled into opposing traffic.
At San Pablo Avenue and Kay Road, Sandoval's vehicle collided with an oncoming vehicle. The impact split the stolen vehicle in half and ejected Sandoval. San Pablo police officers attempted CPR and other emergency medical treatment, but paramedics pronounced him dead at the scene.
The driver of the uninvolved vehicle survived the collision.
The Contra Costa County Law Enforcement Involved Fatal Incident protocol was activated following Sandoval's death. Investigators from the Contra Costa County District Attorney's Office responded and conducted an independent criminal investigation. The Contra Costa County Sheriff's Office Forensics Division processed evidence from the scene, and the California Highway Patrol Multidisciplinary Accident Investigation Team investigated the collision.

OPEN RECORDS SUMMARY :
On February 14, 2021, at approximately 3:30 a.m., off-duty Corbin Police Department Sgt. Stephen Meadors was contacted at his residence on Dogwood Cove in Corbin, Kentucky, by a neighbor reporting a prowler. Sgt. Meadors went outside and encountered 19-year-old Phillip N. Davenport. An altercation ensued, during which Sgt. Meadors opened fire, killing Davenport. Davenport was transported to Baptist Health Hospital in Corbin, where he was pronounced deceased by the Whitley County Coroner. The Kentucky State Police investigated the incident. In October 2021, a Whitley County grand jury returned a "no true bill," clearing Sgt. Meadors of any wrongdoing.
