On April 14, 2025, deputies from the Rockingham County Sheriff's Office responded to a call regarding a suicide threat involving a knife at a residence on the 6000 block of Rawley Pike in Hinton, Virginia. Deputies found Kurtis Adam Simmons locked in a bedroom, reportedly burning items. When deputies attempted to engage him, Simmons allegedly charged at them with an extended box cutter blade. Deputies deployed tasers twice without effect before one deputy discharged his firearm, striking Simmons. He was transported to Sentara RMH Medical Center, where he later died. The Virginia State Police conducted the investigation, and the case was reviewed by the Harrisonburg and Rockingham County Commonwealth's Attorney's Office, which announced no charges would be filed.

The chase concluded in the 800 block of 26th Street in the Golden Hill neighborhood, where Cortez's vehicle collided with a police K-9 unit, causing minor injuries to an officer.As officers attempted to box in the vehicle, Cortez revved the engine. In response, SDPD SWAT officers Sgt. Richard Curtis and Acting Sgt. Bryan Shields fired multiple rounds, striking Cortez. The chase escalated, exceeding 100 miles an hour, and continued onto the westbound Riverside 91 Freeway. It ended when the vehicle collided when chased the wrong way with a semi truck near Lincoln Avenue. The impact caused the semi to crash into a wall and ejected the driver, 55-year-old Stephen Palacios of Rancho Cucamonga. Palacios was transported to a local hospital and pronounced dead. A passenger in Palacios’ vehicle also sustained moderate injuries.
