OPEN RECORDS SUMMARY :
On June 19, 2024, officers responded to a report of a woman, Lisa Fordyce-Blair, in a mental health crisis, allegedly threatening a neighbor with a rifle. Following the initial call, officers arrived and attempted to deescalate the situation over the next several hours. A SWAT team was deployed after Fordyce-Blair refused to respond to negotiators' attempts to make contact via phone and PA announcements. The police then fired non-lethal rounds at the house to create a path for a drone to provide video surveillance.

After the SWAT team fired "knock knock" rounds to break a window, the negotiator managed to contact Fordyce-Blair for a brief phone conversation. During this time, she reportedly fired two or three rounds at officers. After the police breached the house, they found Fordyce-Blair deceased from gunshot wounds.

An investigation by the Office of Special Prosecutions later found that Sergeant Jonathan Butler, the officer involved, was legally justified in the shooting. This incident marked the fourth police-related fatality in Anchorage within the span of a month. The incident has since been ruled an officer-involved shooting, and no charges were filed against the officers involved.
