On June 13, 2024, at approximately 10:30 a.m., the Iowa State Patrol observed a vehicle traveling at high speeds on Highway 69 near Lake Mills, Iowa. The driver, later identified as 61-year-old Jessica A. Chandler of Lake Mills, did not stop when troopers initiated a traffic stop.

Troopers escalated a high-speed pursuit on southbound Highway 69, ending when Chandler's vehicle left the roadway near the intersection of Highway 69 and County Road A16, struck a tree, overturned, and came to rest in a ditch. Chandler was pronounced dead at the crash location.

The Iowa State Patrol has stated that the actions leading up to the crash are under investigation. Findings will be reviewed by the Winnebago County Attorney's Office.
