On 09/12/2024, Tempe police responded to a 911 call reporting that Jake R. Carruth, a 44-year-old Native American man, had allegedly pointed a gun at another individual near an apartment complex close to Arizona Mills Mall. Officers arrived on the scene and initiated a standoff, attempting to engage Carruth, who initially spoke to them through a window and refused to open the door.

Police repositioned to a safer location while continuing their efforts to negotiate. Carruth exited his apartment twice during the standoff. On the second occasion, police claimed he carried a handgun and pointed it toward them. Two officers fired their weapons, fatally shooting him. Carruth was pronounced dead at the scene.

The Mesa Police Department is leading the investigation as part of the East Valley Critical Incident Response Team (EVCIRT).
