On September 4, 2021, Sergeant Fernando Chavez of the Ohio State Highway Patrol was completing a traffic citation on the side of U.S. Route 24 in Defiance, Ohio. A small pickup truck, driven by Justin Johnson of Defiance, crashed into Chavez's patrol car and the vehicle he had traffic-stopped.

Steven Robert Johnson, 52, who was riding in the passenger seat of the pickup truck, was pronounced dead at the scene. Justin Johnson sustained life-threatening injuries and was airlifted to a nearby hospital for treatment.

It is not known whether the parked vehicles engaged in the traffic stop were obstructing the roadway. The incident is under investigation by the Ohio State Highway Patrol.
