OPEN RECORDS SUMMARY :
On 01/03/2025, Chicago Police Department officers responded to a domestic disturbance call at the Albany Terrace Apartments, a senior living facility in the 3000 block of West 21st Place in Chicago. Upon arrival, officers encountered 58-year-old Timothy Glaze, who was armed with a knife. It is alleged that Glaze moved toward the officers. They opened fire and killed him. The shooting is under investigation by the Civilian Office of Police Accountability (COPA) An autopsy revealed that Glaze was shot 16 times, with wounds to his chest, abdomen, torso, thighs, buttocks, and arms. Toxicology reports indicated the presence of alcohol and cocaine in his system at the time of death. Glaze was reported to be suffering from multiple forms of cancer, and his partner expressed concerns about his mental state leading up to the incident. Autopsy noted 16 gunshot wounds in his body.
