OPEN RECORDS SUMMARY :
Police in Miami, Oklahoma, responded to reports of a shooting following a vehicle collision. The driver, identified as 37-year-old Cody Ryan Mayfield, allegedly shot another driver before leaving the scene. Officers located Mayfield’s vehicle near Peoria and E Street N.W. When police surrounded a nearby residence, they said Mayfield emerged holding a gun to his head and threatening suicide. Officers deployed pepperballs, but Mayfield reportedly fired at police, who opened fire and killed him. The Oklahoma State Bureau of Investigation is conducting an independent review.
