On February 22, 2025, Officer Andrew Duarte of the West York Borough Police Department responded to a hostage situation at UPMC Memorial Hospital in York, Pennsylvania. The assailant, Diogenes Archangel-Ortiz, entered the hospital's intensive care unit armed with a handgun and zip ties, taking staff members hostage under unknown circumstances. During the confrontation, Archangel-Ortiz shot three hospital employees—a doctor, a nurse, and a custodian—and injured a fourth staff member who fell during the incident. Officer Duarte and other law enforcement officers engaged the gunman, resulting in a shootout that led to the deaths of both Archangel-Ortiz and Officer Duarte. Two additional officers were injured but are expected to recover.

On April 30, 2025, it was announced that he had been killed by friendly fire by another officer.
