On July 29, 2021, around 5:30 p.m., officers from the Mansfield Police Department responded to a 911 hang-up call at an apartment located on 300 Wood Street in Mansfield, Ohio. Upon arrival, officers heard someone yelling for help from inside the residence.
Officers ordered James Haynes, a 31-year-old man armed with a knife, to drop the weapon. Instead, he allegedly moved toward a window. An officer fired a Taser, but Haynes began re-entering the apartment through the broken window. Officers opened fire, then forced entry into the apartment. Inside, they again confronted him while he was still armed and fired additional shots, killing him.
