OPEN RECORDS SUMMARY : On 05/10/2026, Forest Acres police said Cody Johnson was driving erratically near Club Relapse at 5024 Two Notch Road and nearly collided with a patrol vehicle. Police activated emergency lights and attempted a traffic stop as Johnson drove into the club parking lot. Police said Johnson ignored commands to stop and continued driving through an area crowded with vehicles and people. Authorities said Johnson accelerated toward the back of the property and nearly struck a security guard and others in the parking lot. Cpl. Daniel Haddad opened fire, killing Johnson. The Richland County Sheriff’s Department said the shooting remains under investigation and findings will be forwarded to prosecutors for review.
