The Orange County District Attorney's Office responded to the scene and is conducting the investigation into the fatal shooting, consistent with California law governing police shootings. Event : Suicide under police duress during barricade and house fire

OPEN RECORDS SUMMARY :

NARRATIVE:
On 07/21/2026, an unidentified adult man died from an apparent self-inflicted gunshot wound during a Dallas Police Department SWAT barricade and house fire in the 600 block of Delaware Avenue in the Oak Cliff neighborhood of Dallas, Texas.
.
At approximately 6:00 a.m., Dallas police responded to an armed disturbance. Police alleged that the man had threatened a woman and a juvenile with a weapon. The woman and child were able to get away safely.
.
The man barricaded himself inside the residence. According to Dallas police, he subsequently set the house on fire while remaining barricaded inside. SWAT personnel responded as the armed barricade and structure fire continued.
.
Dallas Fire-Rescue was called to the residence at approximately 7:30 a.m. Firefighters could not initially enter the burning house because police had not secured the armed barricade. Fire crews instead operated defensively and worked to prevent the fire from spreading to neighboring residences. The house sustained extensive fire damage.
.
After police were able to resolve the barricade, the man was found dead inside the residence. Dallas police said he had an apparent self-inflicted gunshot wound. He was pronounced dead at the scene.
.
The death occurred during an active police encounter that began with officers responding to an armed domestic disturbance and continued through the man's barricade, the structure fire and SWAT response. Police attributed the fire to the barricaded man. Unlike the Woodland Hills case, I found no reporting indicating that police deployed tear gas or other incendiary-capable chemical agents before this fire.
.
Dallas police continued investigating the circumstances surrounding the incident. Initial reporting did not establish the man's relationship to the woman and child or disclose additional information about what preceded the alleged threats.
.
OFFICER BACKGROUND:
The Dallas Police Department has not publicly identified the patrol officers or SWAT personnel involved in the barricade. No police gunfire was reported in the sources located.

Location : 5720 N. Owensmouth Avenue

City : Woodland Hills

County : Los Angeles

State : California

Agency : Los Angeles Police Department

Officer(s) : Not Disclosed

Cause of Death : Fire

Event : Died during SWAT barricade and structure fire

OPEN RECORDS SUMMARY :

NARRATIVE:
On 07/19/2026, a 46-year-old man died during an hours-long Los Angeles Police Department SWAT barricade and structure fire at a townhome at 5720 N. Owensmouth Avenue in Woodland Hills, California. His remains were found inside the burned residence after SWAT officers were eventually able to secure the scene.
.
At approximately 3:15 p.m., a woman went to LAPD's Topanga Station and reported that her husband had severely beaten and raped her. Officers responded to the couple's townhome to investigate and take the man into custody. The man remained inside and would not come out. Police determined that firearms were legally registered to him, and the encounter developed into a barricade.
.
LAPD SWAT was called to the residence. Police used chemical agents, including tear gas, in an attempt to force the man from the townhome, but he remained barricaded inside.
.
Los Angeles Fire Department personnel were staged nearby specifically because of the ongoing police operation. At 6:41 p.m., LAFD upgraded the incident to a structure fire after heavy smoke was observed coming from the second floor of the middle unit of the three-unit townhome complex. Fire officials simultaneously reported that shots were being fired.
.
Because of the continuing reports of gunfire, firefighters could not safely enter the burning building. Instead, LAFD conducted defensive firefighting from outside while LAPD officers provided armed protection for firefighters operating hose lines around the structure. The fire spread from the middle townhome into an adjacent unit.
.
The origin of the gunfire was not immediately established. LAPD subsequently told NBC Los Angeles that firearms were stored inside the residence and that gunshots were heard during the fire, but investigators could not initially determine whether the barricaded man was firing the weapons or whether ammunition was discharging because of the heat. Police confirmed that LAPD officers did not fire their weapons.
.
The tactical situation continued throughout the night. Approximately 63 firefighters ultimately responded. At 4:33 a.m. on 07/20/2026, LAFD reported that the situation remained static and firefighters were still waiting for SWAT to secure the scene. LAFD Arson investigators and the Los Angeles Department of Building and Safety were called to assist.
.
After authorities were finally able to search the burned townhome, human remains were located inside. Police believed the remains were those of the barricaded man. The woman who had reported the domestic violence was also reported to have sustained injuries.
.
Early news coverage reported that investigators believed the barricaded man may have started the fire. However, the Los Angeles Fire Department subsequently stated that the cause of the fire remained under investigation by LAFD and LAPD. The available reporting therefore does not establish whether the man deliberately set the fire, whether the fire was connected to police-deployed chemical agents, or whether another mechanism caused the ignition.
.
That unresolved sequence is significant because LAPD deployed tear gas during the barricade before the structure became involved in fire. The available evidence does not establish that the police chemical agents caused the fire, but it also does not provide a final arson determination establishing an alternative ignition source. The cause of the fire should therefore remain unresolved pending the LAFD arson investigation.
.
The man's medical cause and manner of death have also not been established in the public sources located. It is unclear whether he died from fire-related injuries, smoke inhalation, gunshot wounds, another injury or some combination of circumstances. Cause of Death therefore remains Pending.
.
OFFICER BACKGROUND:
The LAPD officers and SWAT personnel involved in the barricade have not been publicly identified in the sources located. LAPD incident number associated with the operation was 2653.

City : Glendale

County : Maricopa

State : Arizona

Agency : Glendale Police Department; Peoria Police Department; Phoenix Police Department

Officer(s) : Not disclosed

Cause of Death : Suicide

Event : Suicide under police duress

OPEN RECORDS SUMMARY :

On 07/19/2026 at approximately 2:00 p.m., Glendale Police Department officers responded to the intersection of 67th Avenue and Grand Avenue after receiving reports of a man unconscious behind the wheel of a pickup truck. When officers approached the vehicle, the driver, later identified as John Graff, 33, suddenly accelerated and repeatedly rammed multiple Glendale police patrol vehicles in an effort to escape.
After forcing his way through the patrol vehicles, Graff continued driving away. Rather than engaging in a traditional high-speed pursuit, Glendale police coordinated a tracking operation using undercover officers along with air support from the Phoenix and Peoria police departments. Authorities monitored the vehicle from a distance as it traveled through several West Valley communities.
The incident ended in a residential neighborhood near 94th Avenue and Glendale Avenue. Graff stopped the vehicle, exited while armed with a handgun, and died from a self-inflicted gunshot wound before officers took him into custody. Emergency medical personnel responded, but he was pronounced dead.
Subsequent investigation identified the decedent as John Graff, 33. Glendale police stated Graff had allegedly assaulted his girlfriend and stolen her pickup truck before officers first encountered him. Detectives recovered a handgun from the scene and processed both the truck and the damaged patrol vehicles.
Glendale Police Department detectives conducted the investigation with assistance from participating agencies. The circumstances surrounding the initial assault, vehicle theft, and police response remained under investigation following the incident.

Emergency personnel searched the river and subsequently recovered Watson's body. Authorities publicly identified him as 29-year-old Joshua Miguel Watson of Eau Claire.
.
Watson's obituary gives his full name as Joshua Miguel Watson and his date of birth as 04/06/1997. He was born in Haiti and came to the United States after being adopted at age six. He grew up in Wisconsin and graduated from Eau Claire Memorial High School in 2015.
.
Watson was the father of two sons. His obituary describes his children as an important part of his life and says he enjoyed music, basketball, fishing and spending time with family and friends.
.
The available evidence indicates Watson entered and continued through the river while attempting to get away from police. There is no publicly reported evidence that he intended to die. His death is therefore classified as Drowned while fleeing police, without assigning suicidal intent.
.
OFFICER BACKGROUND:
The Eau Claire Police Department has not publicly identified the officers who responded to the Galloway Street disturbance, followed Watson to the Chippewa River or maintained contact with him immediately before he disappeared beneath the water.

Location : 2100 block of Sacramento Street

City : Vallejo

County : Solano

State : California

Agency : Vallejo Police Department

Officer(s) : Brad Kim

Cause of Death : Vehicle

Event : Motorcycle driver killed by police chase crash

OPEN RECORDS SUMMARY :

On 07/14/2026 at approximately 4:21 a.m., a Vallejo Police Department patrol officer observed a motorcycle traveling at a high rate of speed and driving recklessly near the intersection of Sonoma Boulevard and Tennessee Street. The officer attempted a traffic stop, but the rider, later identified as Marcus Kutterer, 43, continued riding away while the officer pursued with emergency lights and siren activated.
According to the Vallejo Police Department, the pursuit lasted less than one minute before the officer reported terminating it because of the motorcycle's excessive speed and increasing distance from the patrol vehicle. Approximately three minutes later, officers were dispatched to a traffic collision in the 2100 block of Sacramento Street, where they found Kutterer trapped beneath an unoccupied parked vehicle after his motorcycle struck it. Vallejo Fire Department personnel used specialized lifting equipment to free him, but he died at the scene.
Subsequent reporting identified the decedent as Marcus Kutterer, 43, of Fairfield. Surveillance video later obtained by the Vallejo Sun showed the pursuing patrol vehicle continuing past the crash scene with lights and siren activated immediately after the collision rather than stopping. Dispatch recordings released by the outlet also indicated the pursuing officer radioed that the motorcycle had continued toward Valle Vista Avenue, even though the surveillance video indicates the crash had already occurred before that transmission. The video prompted Kutterer's family to publicly question the official account of the pursuit and the officer's actions immediately after the collision.
Kutterer's family held a candlelight vigil at the crash site and publicly called for accountability. His daughter, Ariana Ramirez-Kutterer, questioned why the officer did not stop after the collision despite passing the crash scene. His son, Marcus Daniel Kutterer Jr., also challenged the department's account that the pursuit had ended before the collision. The family consulted civil rights attorney Melissa Nold, who publicly questioned the necessity of initiating a pursuit over the reported driving violations.
The Vallejo Police Department Traffic Division continues to investigate the collision. Officer Brad Kim was identified as the traffic investigator assigned to the crash investigation. As of the latest available reporting, no findings regarding the officer's conduct or the pursuit have been publicly released.

City : Saint Augustine

County : St. John's

State : Florida

Agency : U.S. Immigration and Customs Enforcement (ICE); Homeland Security Investigations (HSI) Officer(s):

Officer(s) : Not disclosed

Cause of Death : Vehicle

Event : Killed during immigration enforcement operation

OPEN RECORDS SUMMARY :

On 07/14/2026, federal agents with U.S. Immigration and Customs Enforcement (ICE) and Homeland Security Investigations (HSI) conducted an immigration enforcement operation at a gas station and convenience store along State Road 16 in St. Augustine, Florida, shortly before 7:00 a.m. Four men were inside a vehicle when they encountered the federal agents. During the encounter, all four left the vehicle and ran from the area on foot. Authorities have not publicly explained why the group was stopped or what specific enforcement action was underway.

According to the Florida Highway Patrol (FHP), Juan Jairo Coronilla Duran, a 28-year-old Mexican national, ran across the eastbound lanes of State Road 16 while attempting to leave the area. As he entered the roadway, he was struck by an eastbound tractor-trailer. The impact caused fatal injuries, and he was pronounced dead at the scene. The tractor-trailer driver immediately stopped and attempted to render aid. The roadway was closed for several hours while investigators documented the scene. FHP assumed responsibility for the traffic fatality investigation, while Homeland Security Investigations also opened an investigation into the circumstances surrounding the operation.

The Department of Homeland Security later confirmed that federal law enforcement had been conducting an operation in St. Johns County and stated that both HSI and the Florida Highway Patrol were investigating the incident. Officials did not release additional details about the purpose of the operation or whether anyone else in the group was detained.

The man's identity was initially withheld but was later released by his family as Juan Jairo Coronilla Duran, who was from Guanajuato, Mexico. His wife, Yésica Alamilla López, traveled from Mexico to Florida to identify him and begin arrangements to return his body home. She said Coronilla Duran was the father of two children, had entered the United States legally on a tourist visa approximately three weeks before his death, and had been in Florida for only two days before the incident occurred. She stated that he had come to the United States seeking work.

The death occurred during a week in which multiple fatal incidents involving ICE operations drew national attention. Immigration advocacy organizations called for an independent investigation into the St. Augustine incident, arguing that aggressive enforcement operations can create panic that contributes to dangerous situations. Federal authorities have not alleged that Coronilla Duran committed any violent offense, and no criminal charges involving him were announced following the incident. As of the latest public reporting, both the Florida Highway Patrol and Homeland Security Investigations continue to investigate the circumstances surrounding his death.

Jose Angel Castillo

Age :43

Gender :Male

Race : Latino/a/e

Date : 07/13/2026

Location : 4000 block of International Boulevard near 40th Avenue

City : Oakland

County : Alameda

State : California

Agency : Oakland Police Department; California Highway Patrol

Officer(s) : Not disclosed

Cause of Death : Gunshot

Event : Killed by police after CHP contact at homeless encampment

OPEN RECORDS SUMMARY :

On 07/13/2026, 43-year-old Jose Angel Castillo was shot and killed by two Oakland Police Department officers in Oakland's Fruitvale neighborhood after an encounter that began when California Highway Patrol officers contacted him near an unhoused encampment. Oakland police released body-camera and surveillance footage of the fatal encounter in August.
.
California Highway Patrol officers initially encountered Castillo near an encampment along International Boulevard. Castillo was carrying two long homemade spear-like weapons consisting of poles with knives or blades attached to their ends. CHP requested assistance from Oakland police.
.
Video released by Oakland police shows a CHP officer ordering Castillo to put the weapons down and telling him that he was being detained. Castillo did not drop the poles and instead walked away from the CHP officers toward International Boulevard.
.
Oakland officers subsequently located Castillo near East 12th Street and 41st Avenue. The body-camera footage shows officers repeatedly ordering him to put down the weapons while Castillo continued walking away. Castillo repeatedly told officers to leave him alone and said that he was leaving.
.
Officers continued following Castillo as he moved through the neighborhood carrying both homemade weapons. Additional officers arrived, including plainclothes personnel. Video shows nearby civilians, including children, moving away or taking cover as the police encounter progressed toward a shopping-center parking lot.
.
Castillo eventually entered the parking lot near International Boulevard and 40th Avenue. Officers continued giving him commands to put down the weapons. The released footage shows Castillo still holding the two bladed poles while officers maintained distance from him.
.
At least two Oakland officers then deployed Tasers at Castillo. The electrical discharge briefly affected him, but it did not end the encounter. Castillo removed a backpack and remained in possession of the homemade weapons.
.
The released video shows Castillo subsequently swinging or raising one of the bladed poles toward officers and moving in their direction. Two Oakland police officers then fired their handguns at him from an estimated distance of approximately 15 to 20 feet.
.
Castillo was struck by the officers' gunfire and fell in the parking lot. Officers initially maintained their positions before approaching him, placing him in handcuffs and beginning emergency medical treatment. Castillo died at the scene.
.
The body-camera and surveillance recordings provide direct visual evidence of substantial portions of the encounter, including Castillo carrying the homemade spears, walking away from police while officers repeatedly ordered him to drop them, the attempted Taser deployments and his movement with one of the weapons immediately before officers opened fire.
.
The released footage also documents that the encounter was prolonged rather than an immediate confrontation. Castillo moved through several locations while officers followed him and repeatedly ordered him to surrender the weapons before the encounter culminated in the shopping-center parking lot.
.
Civil-rights attorney Jim Chanin questioned whether officers had alternatives to deadly force, including whether police could have maintained greater distance and continued attempting to contain Castillo rather than moving toward a confrontation. The shooting has renewed questions about police responses to unhoused people and people experiencing apparent behavioral crises.
.
The two Oakland officers who fired their weapons were placed on paid administrative leave following the shooting. Their identities had not been publicly released as of the August release of the body-camera footage.
.
The fatal shooting is being investigated by the Oakland Police Department's Internal Affairs Division and the Alameda County District Attorney's Office. Oakland's Community Police Review Agency was also notified and is involved in civilian oversight of the incident.
.
Oakland police released the body-camera and surveillance footage approximately one month after Castillo's death pursuant to California's requirements governing public disclosure of recordings of critical incidents involving police.
.
OFFICER BACKGROUND:
The involved officers have not been publicly identified.

Elliot Powers

Age :46

Gender :Male

Race : Black

Date : 07/12/2026

Location : Bus stop in the 9600 block of Lost Knife Road, near Montgomery Village Plaza

City : Gaithersburg

County : Montgomery

State : Maryland

Agency : Montgomery County Police Department; Gaithersburg Police Department

Officer(s) : Michael Wengloski; Andrew Hanko; Robert Bullock; Noah Whelan; Carissa Fortin; Christopher Leach

Cause of Death : Gunshot

Event : Killed by police during armed encounter

OPEN RECORDS SUMMARY :

On 07/12/2026, 46-year-old Elliott Powers of Rockville was shot and killed by six officers from the Montgomery County Police Department and Gaithersburg Police Department following an approximately 20-minute encounter at a bus stop in front of Montgomery Village Plaza in Gaithersburg, Maryland.
.
At approximately 6:45 p.m., police responded after a bus driver reported seeing Powers sitting at the bus stop with a handgun and apparently loading the weapon. Powers had a black backpack with him, and another person was initially sitting at the bus stop nearby.
.
Gaithersburg Police Sgt. Christopher Leach was among the first officers to encounter Powers. Body-camera footage shows Powers reaching into his backpack as officers backed away and took cover behind a police vehicle. Officers said they saw a handgun and repeatedly directed Powers to show his hands and put down the weapon. The civilian sitting near Powers moved away.
.
The encounter then developed into a prolonged standoff. Officers spoke with Powers for more than 10 minutes and attempted to persuade him to put down the gun. One officer told Powers that police wanted to work through the situation and wanted him alive the following day. Powers repeatedly told officers to shoot him.
.
Leach requested a negotiator, although available reporting does not establish whether a trained negotiator reached the scene before police opened fire. Officers also discussed how they could safely take Powers into custody while he remained armed.
.
Police alleged that Powers eventually raised or pointed the handgun toward officers. Body- and dash-camera footage released by the Maryland Attorney General shows Powers apparently raising his hand holding the gun in the officers' direction immediately before police opened fire.
.
Six officers fired: Montgomery County Police Officers Michael Wengloski, Andrew Hanko and Robert Bullock, and Gaithersburg Police Officers Noah Whelan and Carissa Fortin and Sgt. Christopher Leach. After determining that Powers no longer had the gun in his hands, officers approached him and provided medical aid. Powers died at the scene.
.
The Maryland Attorney General's Office subsequently released body-camera and dash-camera recordings from the encounter. The released evidence includes footage from two body cameras and two police vehicle cameras and documents much of the prolonged interaction preceding the shooting.
.
The Maryland Attorney General's Independent Investigations Division is investigating the killing to determine whether the officers' use of deadly force was lawful.
.
OFFICER BACKGROUND:
Montgomery County Police Officer Michael Wengloski was identified as a 12½-year department veteran at the time he shot Powers. Officer Andrew Hanko was an eight-year veteran, and Officer Robert Bullock was a seven-year veteran.
.
Gaithersburg Police Sgt. Christopher Leach was a 14-year department veteran. Officer Carissa Fortin was an 8½-year veteran and Officer Noah Whelan was a five-year veteran. Leach was among the officers who initially confronted Powers and requested a negotiator during the standoff.
.
Further review is needed to determine whether any of the six officers had previous fatal shootings, other shootings, significant use-of-force complaints, discipline or civil litigation before Powers' death.
