On 05/22/2026 at approximately 6:37 p.m., Salt Lake City police officers responded to a residence near 150 West Lucy Avenue after a 911 caller reported that Thomas Nelson was suicidal, had brandished a handgun during an encounter with a neighbor, and had a firearm in his waistband. Officers arrived within minutes, established positions around the residence, and requested additional resources while attempting to contain the situation.
Dispatch advised officers that Nelson had gone back inside the residence. Officers remained behind cover and continued monitoring the home while awaiting additional resources. Several minutes later, Nelson exited through the front door holding a handgun. According to the subsequent body-camera review released by the Salt Lake City Police Department, officers repeatedly ordered him to stop and drop the weapon. Nelson instead raised the handgun while facing officers. Four officers then fired their service weapons, striking him multiple times.
Officers immediately approached Nelson after the shooting, secured the handgun, and began rendering emergency medical aid until Salt Lake City Fire Department personnel arrived. Despite lifesaving efforts, Nelson was pronounced dead at the scene. No officers or civilians were injured during the incident.
Under Utah's officer-involved critical incident protocol, the West Valley City Police Department assumed responsibility for the independent criminal investigation. The four involved officers were placed on paid administrative leave in accordance with department policy while investigators collected body-camera recordings, 911 calls, witness statements, and forensic evidence. The Salt Lake City Police Department later publicly identified the decedent as Thomas Nelson, age 33, when releasing the critical incident briefing and body-camera footage.
