On 06/28/2026, Indiana Borough Police Department officers responded to a domestic disturbance at a residence in the 100 block of North Cherry Avenue in Indiana, Pennsylvania. During the incident, 35-year-old Andrew Wade Herman threatened a responding officer with a shovel before running from the scene. An officer pursued Herman on foot and attempted to take him into custody. During the struggle, Herman stabbed the officer multiple times in the head and face with a knife. The officer then fired, striking Herman. Herman was pronounced dead at the scene at 7:28 p.m. The injured officer was transported to a local hospital with non-life-threatening injuries. The Pennsylvania State Police Troop A Major Case Team is investigating the shooting, and the Indiana County Coroner ruled Herman's death a homicide.
