OPEN RECORDS SUMMARY :

On 08/10/2026, at approximately 12:29 a.m., Las Vegas Metropolitan Police Department officers responded to a residence in the 4500 block of Pigeon Point Court after a woman called 911 reporting that her boyfriend, Jason Simmons, 48, was chasing her with a blowtorch and attempting to set her on fire. During the call, the woman reported that Simmons had shot her and that her teenage daughter was also inside the residence.

When officers entered the house, smoke alarms were sounding and police encountered several small fires that LVMPD alleged Simmons had set. Officers located the woman's teenage daughter in a bedroom suffering from a gunshot wound.

Officers then approached a locked interior door and attempted to force it open. LVMPD said Simmons fired seven rounds through the closed door. One round struck Officer Zaira Venegas-Hernandez in the chest, penetrating multiple layers of her ballistic vest and causing an abrasion. Venegas-Hernandez returned fire as officers withdrew from the doorway.

Police converted the encounter into a barricade response and called SWAT personnel and crisis negotiators. When SWAT subsequently entered the locked room, officers found Simmons dead from an apparent self-inflicted gunshot wound. The available reporting does not indicate that Venegas-Hernandez's return fire struck Simmons.

The teenage girl found by the responding officers also died from her gunshot injuries. LVMPD identified her as the daughter of the woman who called 911. The woman's identity and her daughter's identity had not been publicly released in the reporting reviewed. The surviving woman was hospitalized in critical condition with a gunshot wound and burn injuries.

LVMPD released 911 audio and body-worn-camera footage on 08/12/2026. The footage documents officers entering the smoke-filled residence, locating the wounded girl, attempting to enter the locked room, the ensuing gunfire, Venegas-Hernandez being struck, and officers withdrawing.

OFFICER BACKGROUND:
Zaira Venegas-Hernandez, 36, was identified as the LVMPD officer who exchanged gunfire with Simmons. She has been employed by LVMPD since 2021 and is assigned to the Tourist Safety Division's Spring Valley Area Command. LVMPD placed her on paid administrative leave pending review of the shooting.
