The deputy initiated a pursuit without activating lights and sirens but soon lost sight of the vehicle. Shortly thereafter, the deputy discovered the Tesla had veered off the road, crashed through a fence, struck three large trees, and caught fire in the backyard of a residence in the 7000 block of 128th Street East. The deputy attempted to rescue the 45-year-old male driver by breaking the driver's side window but was forced to retreat due to the intensifying flames. The driver was pronounced dead at the scene. On September 22, 2024, the Everett Police Department pursued a suspect involved in a shooting at the Park View Apartments in Everett. Officers responded to the scene around 1 p.m., where they found one deceased man and one woman with non-life-threatening injuries. By 1:40 p.m., officers located the person’s vehicle and initiated a pursuit that led onto northbound Interstate 5. The pursuit ended with a crash near milepost 190. Thurston police received a call that a woman had shown up at someone’s residence and demanded fentanyl. Police were called and located the woman who was allegedly driving with a flat tire in a stolen vehicle. Police chased the vehicle and placed spike strips which flattened the second tire on her right side, causing her to swerve into head on traffic where she crashed into a bystander and killed a person in that vehicle. The chase was allegedly initiated for a failed attempt to procure fentanyl and driving erratically with a flat tire. This is one of several police pursuit fatalities that have occurred since Washington’s governor Jay Insleee eviscerated a previous band on police pursuits in 2023, leading to Washington police chasing and causing fatal accidents again.

On 08/28/2024, at approximately 12:35 a.m., Kent Police Department officers responded to Madison Plaza Apartments at 102 Madison Avenue North after a 911 caller reported hearing six or seven gunshots. Two officers initially arrived, got out of their patrol vehicles and began searching the area on foot.

Police reported that additional gunshots came from an upper-floor apartment after officers arrived. Officers believed the shots were directed toward them, although neither of the two police vehicles was struck. One Kent officer fired nine rounds from a department-issued Colt M4LE rifle toward the apartment.

Additional officers entered and cleared the apartment and found Christian Demetri Moshofsky, 33, on the floor with a gunshot wound to his head. Officers attempted lifesaving measures, but Moshofsky died at the scene.

Kent police initially reported that Moshofsky apparently died from a self-inflicted gunshot wound, based in part on information officers received from one of his relatives. That account proved incorrect. On 08/30/2024, the King County Medical Examiner determined that Moshofsky died from a gunshot wound to the head and classified his death as a homicide. The King County Prosecuting Attorney's Office confirmed that the fatal wound resulted from Kent police gunfire.

Subsequent ballistic testing provided considerably more detail. Investigators determined that 15 cartridge cases recovered at the scene had been fired from a Ruger 5.7mm pistol recovered inside Moshofsky's apartment. Investigators reported that eight shots were fired before police arrived and seven after officers were on scene. Police fired nine rounds in response. Nobody was struck by any of the 15 rounds attributed to Moshofsky's handgun.

The autopsy recovered two projectiles from Moshofsky. Washington State Patrol Crime Laboratory testing subsequently determined that both projectiles had been fired from the Kent officer's Colt rifle, definitively establishing that police gunfire killed Moshofsky rather than a self-inflicted wound.

The Valley Independent Investigative Team conducted the investigation, led by the Port of Seattle Police Department with Kent police excluded from investigating its own shooting. The investigation was not completed until April 2026, approximately 20 months after Moshofsky was killed. On 04/24/2026, investigators forwarded the completed case to the King County Prosecuting Attorney's Office for review and a determination of whether criminal charges against the officer were warranted. As of the most recent King County Prosecuting Attorney public case listing located for this update, the matter remained under review.

Moshofsky's mother, Adrianne Arrington, described her son as a dedicated father who worked to support his son and helped care for his mother, nieces and nephews. She established a verified GoFundMe following his death to assist his family with his burial.

The pursuit began when a Pierce County sheriff’s deputy attempted to stop the Tahoe at 176th Street East and Pacific Avenue South because its vehicle registration tabs were expired. The driver continued east on 176th Street after the deputy activated the patrol vehicle’s emergency lights.

During the pursuit, the Tahoe went through a red light and collided with another vehicle. It then looped back west on 176th Street and was involved in another collision on Spanaway Loop Road. Two drivers were injured in the collisions.

A pursuing Pierce County sheriff’s deputy then performed a PIT maneuver against the Tahoe. The Tahoe spun, struck a curb and flipped into roadside brush. Bagnariol, who was riding in the front passenger seat, was ejected, and the Tahoe landed on top of her. Emergency personnel freed her from beneath the vehicle and transported her to a hospital in critical condition. She died five days later.

The driver later pleaded guilty to DUI vehicular homicide. On 10/29/2024, Pierce County Superior Court Judge Timothy Ashcraft sentenced him to seven years in prison.

On 08/08/2024, a Washington State Patrol trooper attempted to stop a Jeep Grand Cherokee on Boeing Access Road. The publicly released reports did not disclose the reason for the attempted traffic stop. The driver continued driving away while troopers pursued southbound on Airport Way South. During the pursuit, the fleeing vehicle struck the front of a second Washington State Patrol patrol vehicle. That trooper immediately terminated the pursuit.
Moments after the pursuit ended, the Jeep entered the northbound lanes of Airport Way South, left the roadway, struck a utility pole, and rolled over. Passenger Lois Katherine Phair, 66, was ejected from the vehicle and died at the scene. The driver attempted to flee on foot after the crash but was taken into custody. Investigators alleged the driver was impaired by drugs and later sought a second-degree murder charge because the passenger died during the commission of the felony offense of attempting to elude police. Two dogs were also in the vehicle; both survived the crash.
The Seattle Police Department Traffic Collision Investigation Squad investigated the crash in coordination with the Washington State Patrol. Public reporting through 2026 identified no criminal charges against the pursuing troopers, no publicly announced disciplinary action, no publicly reported civil lawsuit or settlement arising from the pursuit, and no public release identifying the troopers involved. A pitbull in the vehicle refused to leave her side after the crash.

On 07/24/2024, Pierce County Sheriff's Office deputies responded to a domestic violence call at a residence in the 7500 block of 242nd Avenue Court East near Buckley. A woman who called 911 reported that 48-year-old Timothy McDonald was armed with a handgun. She was able to leave the residence safely before deputies arrived.
Deputies established a perimeter and attempted to communicate with McDonald. According to investigators, McDonald exited the residence onto the front porch while armed with a handgun. Deputies ordered him to drop the weapon. Police stated that McDonald instead raised the handgun toward deputies, and multiple deputies fired their weapons, striking him.
Deputies rendered medical aid until firefighters and paramedics arrived, but McDonald was pronounced dead at the scene.
The Pierce County Force Investigation Team conducted the investigation and submitted its findings to the Pierce County Prosecuting Attorney's Office. In May 2025, prosecutors concluded that the deputies' use of deadly force was legally justified under Washington law and declined to file criminal charges. Bystander killed by police chase crashOn 07/13/2021, at approximately 11:35 p.m., Kent police attempted to stop a stolen Hyundai Sonata after its owner located the vehicle and notified authorities. The 16-year-old driver continued driving away while police pursued through Kent. Investigators determined the Hyundai reached speeds of approximately 103 mph on city streets before entering the intersection of 104th Avenue SE and SE 261st Street, where it struck a vehicle driven by 41-year-old Alizar A. Kachiyev of Auburn. Kachiyev suffered fatal injuries and died at the scene. The Hyundai's 14-year-old passenger was seriously injured, and the driver fled on foot before being identified through the investigation. Later court filings alleged the teen had driven at extremely high speeds through residential neighborhoods while attempting to escape police. He was subsequently charged as an adult with second-degree murder, vehicular homicide, felony hit-and-run resulting in death, attempting to elude a pursuing police vehicle, possession of a stolen vehicle, and two counts of vehicular assault. Prosecutors argued the circumstances demonstrated extreme indifference to human life. Court records also showed the teen had recently been released from electronic home detention on unrelated felony cases before the fatal pursuit.
