On November 28, 2024, deputies from the Volusia County Sheriff's Office responded to reports of a suicidal individual at the Kingston Shores Condominiums in Ormond-By-The-Sea, Florida. The individual, identified as 56-year-old Joseph DiFusco, a veteran from Connecticut with a history of mental health issues, was staying in an Airbnb rental.

Initially, around 4 a.m., deputies made contact with DiFusco, who declined medical assistance. Approximately two hours later, DiFusco began firing hundreds of rounds into neighboring condominiums and at responding deputies.

The Volusia County Sheriff's Office SWAT team was deployed, and residents were evacuated for safety. Despite attempts at negotiation, DiFusco continued firing, targeting negotiators and SWAT vehicles.

Armed with multiple firearms, including a shotgun, handgun, and a tommy gun, DiFusco retreated to a back bedroom. After deploying tear gas without success, a SWAT sniper fatally shot DiFusco to protect the community and law enforcement personnel.

On 06/21/2024, the Apopka Police Department responded to a report of an individual experiencing a possible overdose at an apartment complex located at 518 Lake Bridge Lane in Apopka, Florida. Upon arrival, officers claimed the man had barricaded himself inside the apartment and was believed to be armed. The situation escalated, and the SWAT team was deployed to the scene.

SWAT officers used a surround-and-callout strategy, deploying ballistic helmets, vests, and shields while setting up a secure perimeter around the apartment. Officers were armed with rifles and handguns and brought specialized breaching tools to gain entry. The standoff lasted several hours, during which officers reportedly attempted to engage the individual in negotiation. Despite these efforts, SWAT breached the apartment using tactical entry equipment and found the man deceased from a self-inflicted gunshot wound.

On June 19, 2024, officers responded to a report of a woman, Lisa Fordyce-Blair, in a mental health crisis, allegedly threatening a neighbor with a rifle. Following the initial call, officers arrived and attempted to deescalate the situation over the next several hours. A SWAT team was deployed after Fordyce-Blair refused to respond to negotiators' attempts to make contact via phone and PA announcements. The police then fired non-lethal rounds at the house to create a path for a drone to provide video surveillance.

After the SWAT team fired "knock knock" rounds to break a window, the negotiator managed to contact Fordyce-Blair for a brief phone conversation. During this time, she reportedly fired two or three rounds at officers. After the police breached the house, they found Fordyce-Blair deceased from gunshot wounds.

An investigation by the Office of Special Prosecutions later found that Sergeant Jonathan Butler, the officer involved, was legally justified in the shooting. This incident marked the fourth police-related fatality in Anchorage within the span of a month. The incident has since been ruled an officer-involved shooting, and no charges were filed against the officers involved​.

On April 24, 2024, at approximately 9:50 p.m., North Andover police responded to a 911 call reporting that 36-year-old Sean Robert DeAdder was armed and experiencing a mental health crisis at his residence on Waverly Road. Upon arrival, DeAdder fired at responding officers, injuring one officer with metal shards from a bullet striking a cruiser. The officer sustained minor injuries.

The Northeastern Massachusetts Law Enforcement Council (NEMLEC) SWAT team was deployed to the home. DeAdder locked himself inside the home, continuing to fire from both the front and rear windows. After several hours, he was found deceased from a self-inflicted gunshot wound.

The identities of the officers involved have not been publicly disclosed. The Massachusetts State Police are conducting the investigation into the incident.

On January 17, 2024, at approximately 9:15 PM, the Bibb County Sheriff's Office responded to a report of 49-year-old Christopher Clyde Valentine, Sr. who had barricaded himself inside a restroom at Atrium Health Navicent Medical Center in Macon, Georgia. Valentine had recently faced shoplifting charges and alleged probation violations, and was reportedly experiencing a mental health crisis and feared police. Law enforcement, including SWAT, K-9 units, and hostage negotiators, responded to the scene.

As officers attempted to negotiate, shots were heard from inside the restroom. SWAT units set up a perimeter and intensified efforts to communicate with Valentine. After several hours of pressure and negotiations, the SWAT team began making entry into the restroom. At approximately 12:45 AM on January 18, 2024, Valentine took his own life. No one else was injured during the incident.

The Bibb County Sheriff's Office was actively engaged in this situation with a militarized response, deploying SWAT and other specialized units. The Georgia Bureau of Investigation (GBI) is conducting an independent review of the incident.

Paton Pinette had a long history of mental health issues and broke into his former employer’s office, stole a gun and was seen with it elsewhere when police arrived and created a standoff with SWAT eventually killing him. He had been treated and was compliant with medications and needed treatment but was killed by police instead. He had been harassed by a coworker prior to being fired until he broke down according to family members. His family said he was no threat to anyone and was distressed and confused but nonviolent and needed support.

Ryan Clay was experiencing a mental health crisis and per his mother, he was firing shots inside of the house. Joseph Ramos was in a mental health crisis in a vehicle threatening to kill himself. Bernalillo SWAT showed up, deployed drones, police K9s, militarized SWAT bearing down on the man in the vehicle for 12 hours until Deputy Joshua Mora fired a single shot in the back of his head, killing him. Ramos never fired his gun.

This was the third killing for Deputy Mora who placed spike strips, chased by a helicopter for 20 miles, initiated a PIT maneuver and shot and killed Martin Jim and his passenger Isaac Padilla both for allegedly driving a stolen vehicle.

Kootenai County deputies were called to an alleged mental health emergency.
According to someone who knew Ms. Smith, she had been an elderly woman who was 95% blind, had been served a 60-day eviction notice which was then reduced to 30 days. She reportedly had run out of options. Five consecutive gunshots were heard by witnesses, suggesting suicide to be highly unlikely. A helicopter and SWAT armored vehicle added to the military racket causing distress to the whole community per eyewitnesses. They arrived and for unknown reasons, they fired shots at her. SWAT arrived. Police stated that the woman was “found dead in the apartment.”

Police are refusing to answer any questions about the incident and the police chief was confident on 11/10/2023 in an interview that his officers would be cleared from the incident and back to work shortly. A bodycam was eventually released, showing that the officer fired at least 18 bullets at a woman who was hiding in her bed with a pistol.

Andrew Jerome Washington was in an agitated state, having a mental health crisis when police appeared with SWAT and according to the family, they escalated an already tense situation.
