On April 28, 2021, New York State Police identified Barry K. Stewart, 55, concerning a double homicide at BridgeView Real Estate Services in Watertown, New York. A State Police K9 unit spotted Stewart's vehicle, a gray 2020 Ford F-150 pickup truck, on Route 11B in Dickinson, Franklin County. When troopers attempted a traffic stop, Stewart's vehicle left the roadway. Troopers found Stewart inside the vehicle with a self-inflicted gunshot wound; he was pronounced dead at a nearby hospital.

Officers spotted a possibly stolen Ford F-150 pickup truck on John Ralston Boulevard and attempted to pull the driver over. The driver fled, and a police chase ensued for about four minutes. The driver lost control of the vehicle and crashed, causing the truck to burst into flames. The driver was killed in the crash.

On 04/27/2021, Bakersfield police shot and killed 20 year old Dalton James Gerrit Kooiman following an attempted traffic stop in southeast Bakersfield. Police attempted to stop a vehicle they said was stolen. The driver did not stop and continued driving before crashing the vehicle near Feliz Drive.
After the crash, officers attempted to take Kooiman into custody. During the encounter that followed, police stated that Kooiman fired a weapon toward officers. Officers opened fire and killed him at the scene.
The Kern County District Attorney reviewed the shooting and ruled the use of deadly force justified. No officers were charged.

On 04/26/2021, El Paso police restrained and killed 40 year old Daniel Diaz, Jr. during a response to a reported possible break-in at a residence on Buckner Street. Officers encountered Diaz experiencing what family and reporting described as a panic attack. As an officer attempted to speak with him, police deployed a Taser. Additional officers arrived and took Diaz to the ground. During the arrest, multiple officers struck Diaz while attempting to restrain him.
After officers placed Diaz in handcuffs, they observed that he was no longer responsive. Diaz was transported for medical care and died a short time later. The death was ruled a homicide. A departmental review board later exonerated the officers involved, and no criminal charges were filed.

A trooper from the Florida Highway Patrol allegedly clocked a vehicle at 111 mph on a highway near Tampa. When the trooper attempted to pull over the driver, Jennifer Carvajal, she made a hard right turn, sending the Hyundai Elantra down an embankment. On April 24, 2021, officers with the Gary Police Department pursued a stolen U-Haul truck following a stolen vehicle alert in the area. The pursuit ended when the U-Haul collided with an SUV at 6th Avenue near Colfax Street, struck a utility pole, and rolled over. The male driver of the U-Haul was pronounced dead at the scene. One person in the SUV was transported to a hospital with non-life-threatening injuries. The crash caused power outages in the area as crews responded to repair downed utility lines. On 04/24/2021, Metro Nashville police shot and killed 32 year old Marvin Veiga during a traffic stop on Clarksville Pike in the Bordeaux area of Nashville. Officer Christopher Royer initiated the stop after running a license plate check. Police stated that the driver remained in the vehicle and cooperated with the stop.
During the encounter, Veiga exited the vehicle from the passenger side while holding two butcher knives. Officer Royer backed away while issuing commands. As Veiga continued moving toward the officer while holding the knives, Royer opened fire, striking Veiga three times. Veiga died at the scene.

The incident began in the late afternoon after McClure barricaded himself inside a residence on Ridgewood Lane South. On 04/23/2021, Casa Grande police shot and killed 52 year old Edward Robbesom near the intersection of West McMurray Boulevard and North French Street. Police responded to reports of a shooting and arrived to find Robbesom standing while holding a gun, with a woman on the ground nearby in distress. Officers issued commands for Robbesom to drop the gun. Officer Curtis Kieser opened fire and killed Robbesom at the scene.

The vehicle became disabled after striking a curb, and Quintana exited while holding an AR-15 style rifle.
Quintana placed the rifle under his chin and walked across the gas station parking lot. As he moved toward another person at the gas pumps, an officer attempted to deploy a Taser. A Colorado Springs police officer then opened fire, killing Quintana at the scene.

On April 22, 2021, at approximately 7:15 a.m., the Coos County Sheriff's Office received reports of a reckless driver in a red Mitsubishi Eclipse near Port Orford, Oregon. Witnesses reported the driver engaging in dangerous behaviors, including speeding, swerving between lanes, making sudden U-turns, and speeding through town at 60 mph.
The Oregon State Police and Bandon Police Department jointly escalated a pursuit. An Oregon State Police trooper located the vehicle near mile marker 247 on U.S. Highway 101, near Belloni Boys Ranch. A minor collision occurred between the trooper’s vehicle and the red Mitsubishi, prompting the driver to flee southbound on the highway.
The driver then crossed over the double yellow line and collided head-on with a vehicle in the opposite lane.

On 04/21/2021, an officer from the Wyoming Department of Public Safety noticed a vehicle without a license plate near 44th Street SW and S. Division Avenue in Grand Rapids, which matched the description of a stolen car reported earlier that evening. The Wyoming Department of Public Safety is under the city of Wyoming, Michigan, not the state of Wyoming. The officer attempted to pursue Shynia Jones, who was driving the vehicle, but she did not stop. Shortly after, another officer spotted Jones' car and escalated a chase to between 80 and 90 mph. The officer began a pursuit but allegedly “called off the chase” moments before she crashed into another vehicle, then a boulevard median at high speed, became airborne, lost control, and collided with a light pole on the east side of the Street, killing her.

It was later determined that Jones' car was not the stolen vehicle officers were initially looking for.

The driver did not stop and drove northbound on State Route 9. Police continued to chase him into Toquerville, where the driver navigated through side roads and returned to the highway. Around 3:32 a.m., while still being chased, the vehicle rolled over, ejecting the driver. He was pronounced dead at the crash site. A female passenger was hospitalized. The vehicle had been reported stolen out of Salt Lake City.

On April 21, 2021, deputies from the Pasquotank County Sheriff's Office shot and killed Andrew Brown Jr., 42, in the 400 block of Perry Street in Elizabeth City, North Carolina. Deputies were attempting to serve drug-related warrants when Brown allegedly tried to escape by driving in the direction of officers.

Brown was fatally shot, reportedly in the head, as he attempted to flee in his vehicle. The incident prompted protests and widespread public scrutiny. The family and community believe that Brown was trying to escape, and that the deputies violated the use of force policy by shooting at a moving vehicle. Despite the release of body camera footage, which showed Brown driving toward deputies, the family maintains their claim that the shooting was unjustified.
At the time of the shooting, the deputies were reinstated after an investigation, with one officer later retiring. Brown's family reached a $3 million settlement with the Pasquotank County Sheriff's Office but continues to seek full accountability. They are also calling for the release of the full body camera footage and for a federal civil rights investigation to yield results.

A second officer later encountered Olson on the street. During that encounter, the officer issued commands for Olson to drop the pole. As Olson continued moving toward the officer while holding the pole, the officer opened fire and killed him at the scene.
The San Diego County District Attorney later reviewed the shooting and ruled the use of deadly force justified. No officers were charged.

On 04/21/2021, around 12: 05 a.m., Worcester Police Department officers encountered Phet Gouvonvong at 265 Grafton St in Worcester. Gouvonvong was wearing body armor, appeared to be armed with what looked like an assault rifle, and had an explosive device with wires attached, along with carrying a backpack. During the encounter, Gouvonvong reportedly became agitated and made a move toward the police. Police shot and killed him.

On 04/20/2021, officers from the Detroit Police Department responded to an incident at E 8 Mile Road and HannaStreet in Detroit involving a man in a mental health crisis. During the encounter, the man reportedly stabbed an officer in the leg with a knife. In the ensuing chaos, another officer accidentally shot the injured officer in the hand. Police opened fire, killing him.

On 04/20/2021, a 22-year-old male driving a Dodge Charger was allegedly chased at high speed by officers of the Grosse Pointe Park Police Department when he lost control of his vehicle at the intersection of ConnerStreet and E Jefferson Avenue in Detroit, landed under the trailer of a semi-truck, killing him. Police did not disclose the reasons why they escalated the fatal chase.

On April 20, 2021, at approximately 2:17 a.m., a California Highway Patrol officer attempted to stop a white Chevrolet pickup at Fairfax Road and Niles Street for a registration violation. The driver, 25-year-old Branden Lewis LeMay Sr., failed to stop and fled northbound on Fairfax Road before turning westbound onto Panorama Drive. West of Mt. Vernon Avenue, the pickup lost control, veered off the roadway, and rolled down an embankment, landing on its roof in the westbound lane of China Grade Loop. LeMay was ejected from the vehicle and pronounced dead at the scene. A passenger sustained moderate injuries and was taken to a local hospital. The Kern County Coroner’s Office confirmed LeMay’s identity and ruled the death accidental. No family statements or employment details were released.
