On April 2, 2021, at approximately 5:15 a.m., Long Beach Police Department officers responded to a 911 call reporting a man who appeared to have had a heart attack and a woman threatening suicide at a residence in the 2800 block of Golden Avenue. Upon arrival, officers observed a large amount of blood inside the residence and were immediately confronted by 38-year-old Lauren Nicole Archibeque, who was armed with a rifle. Officers quickly exited the residence, set up a perimeter, and attempted to de-escalate the situation by making repeated attempts to communicate with Archibeque, urging her to surrender. She did not respond.

After some time, Archibeque emerged at the doorway of the residence, and officers observed her pointing the rifle at them. At that point, three officers discharged their firearms, fatally wounding her. She was pronounced dead at the scene. Upon re-entering the residence, officers discovered a 49-year-old man on a bedroom floor with a serious laceration to his neck. He was also pronounced dead at the scene. The Los Angeles County Coroner’s Office later determined that the man's death was a suicide.

NARRATIVE:
On 04/02/2021 at approximately 8:30 p.m., Coweta County sheriff’s deputies were stationed near the Interstate 85 exit at Weldon Road when they saw a Kia Forte driven by Jamaal Willis-Powell, 30, of Covington, pass with a defective headlight. A deputy followed and observed the Kia illegally pass a vehicle stopped at a red light before turning onto Collinsworth Road. The deputy then activated emergency lights and siren to initiate a traffic stop.

Willis-Powell continued driving while deputies pursued. The Kia crossed double-yellow lines to pass other vehicles and reached approximately 70 mph in a 45-mph zone. The pursuit covered about 1.7 to two miles and lasted approximately one minute and 41 seconds.

As the pursuit approached the Fulton County line, deputies fell behind and lost sight of the Kia. Shortly afterward, they found it crashed into a tree near Fayetteville Road and Taylor Circle. Willis-Powell was trapped in the wreckage and had to be cut from the vehicle. He died at the scene. His passenger, Jasmine Simone Hayes, 28, was ejected and also killed.

After the crash, authorities reported that the Kia’s license plate was registered to a different vehicle and that Willis-Powell had outstanding felony burglary warrants from Gwinnett County. Those warrants were not the reason deputies initially attempted to stop the Kia.

On 04/01/2021, a 61-year-old man died by suicide during a police response to a mental health crisis at a residence on Valley Way in Antioch. At approximately 7:52 p.m., Antioch Police Department officers were dispatched after a family member called 911 regarding a disturbance and reported that the man was armed with a firearm and experiencing a mental health crisis.

The man left the residence with the firearm and drove away. Officers located his vehicle in the neighborhood, turned around and began following him. The man pulled to the side of the road, got out of the vehicle and fatally shot himself. Officers summoned emergency medical personnel, but he died at the scene.

Texas DPS Trooper Brady Germeroth, 37, of Crockett was traveling west in a marked Chevrolet patrol vehicle when he observed a traffic violation by a separate eastbound vehicle and made a U-turn to pursue that vehicle.

As Germeroth attempted to re-enter the eastbound lane, a 2003 Mack truck tractor pulling a pole trailer, driven eastbound by Chad Deford, 35, crossed into the westbound lane and collided head-on with Painter's Jeep. The Mack then crossed back over the eastbound lane, struck the right rear of Germeroth's patrol vehicle, left the roadway, overturned onto its passenger side and caught fire.

Painter suffered critical injuries and was transported to Crockett Medical Center, where she was pronounced dead. Deford and Germeroth were not injured. The vehicle Germeroth had turned around to stop was not reported as being involved in the collision.

On 03/30/2021 at approximately 6:00 p.m., Helen M. Quarles, 65, of Locust Grove was driving home from her job at a state probation office. Quarles was driving a Nissan Altima on Route 669 and attempting to cross Route 3 near Carrico Mills Road when her car was struck in the driver's side by a Culpeper County Sheriff’s Office Chevrolet Tahoe driven by Lt. Ashleigh Kristen Baughan. Quarles was wearing her seat belt and died at the scene. Baughan suffered minor injuries.

Baughan was on duty but was not responding to an emergency or participating in a pursuit. Her emergency lights and siren were not activated. Evidence developed after the crash alleged that Baughan was traveling approximately 75 mph in the 55-mph zone immediately before the collision.

Virginia State Police investigated the crash with its Culpeper Division Crash Reconstruction Team. Baughan was subsequently charged with reckless driving in connection with Quarles' death. In October 2022, she was found not guilty of reckless driving.

On 03/30/2021 shortly after 3:15 a.m., Daiquan Tarez Wright, 25, of Emporia was sitting in a Ford Crown Victoria in the parking lot of the Quality Motel on U.S. Route 301 when the Greensville County Sheriff’s Office received a disturbance complaint about loud music coming from the vehicle. A deputy responded, and police said Wright drove from the parking lot as the deputy arrived.

The deputy pursued Wright onto Liberty Road. Virginia State Police reported that Wright reached approximately 60 mph during the pursuit.

Wright encountered standing water on Liberty Road and lost control of the Crown Victoria. The car left the roadway and struck a mailbox, fire hydrant, fence and tree before overturning onto its side. On 03/30/2021 at approximately 4:00 p.m., Gage Allen Salsman, 27, of Henrietta was riding a motorcycle when law enforcement attempted to stop him in Henrietta. Police did not publicly disclose the original reason for the attempted traffic stop. Salsman continued driving and officers pursued him from Clay County into Wichita County.

The pursuit continued west on U.S. Highway 287 toward Wichita Falls and reached approximately 105 mph. During the chase, police reported seeing Salsman slow the motorcycle and throw unidentified objects from it. The pursuit eventually entered Wichita Falls.

At Kell Boulevard and Scott Avenue, witnesses reported that Salsman lowered his head, accelerated and entered the intersection against a red light at more than 85 mph. His motorcycle struck the front-left side of an SUV. The motorcycle exploded on impact and was engulfed in flames. Salsman was killed.

NARRATIVE:
On 03/30/2021 shortly before 8:00 a.m., Troy Police Officer Shane Marker went to the Imperial Court area looking for Jalen T. Alexander, 19, who was wanted on Miami County and Greene County warrants. The Miami County case included allegations of felonious assault and child abuse/endangering after Alexander was accused of firing a handgun at his girlfriend's father. Marker knew Alexander was associated with a red Jeep Cherokee registered to his girlfriend, Tashaya Tipton.

Marker saw the Jeep running in a driveway and requested another officer before attempting to stop it after it left the residence. Alexander continued away with Tipton riding in the front passenger seat, and Marker pursued south on State Route 202.

The pursuit lasted approximately seven and a half minutes and covered about 11 miles, reaching approximately 120 mph. Alexander crossed center lines, traveled through intersections and nearly struck other vehicles. Police radio traffic recorded Marker saying several times that he was backing off, including because of traffic, but the pursuit was never terminated before the fatal crash.

A Miami County sheriff's deputy deployed stop sticks at State Route 202 and Ross Road. Alexander attempted to avoid them, but the Jeep's right-front tire was punctured. Police later said the crash occurred too soon afterward for substantial tire deflation to have occurred.

At U.S. 40 and State Route 202, Alexander entered the intersection against a red light and struck a Volkswagen driven by Chelsey Vollmer, 32, who was traveling to work with her 17-month-old daughter in the back seat. Alexander and Vollmer were pronounced dead at the scene. Tipton and Vollmer's daughter survived with serious injuries. Vollmer's daughter suffered fractured ribs and an enlarged spleen.

Police said Tipton later told investigators that Alexander saw the red light and Vollmer's approaching vehicle but made no attempt to avoid the collision, telling Tipton immediately beforehand, “If I’m going, you’re coming with me.” No gun or other contraband was recovered from the Jeep after the crash.

The pursuit prompted public criticism over whether an 11-mile chase reaching 120 mph should have continued. Signs placed near the crash site asked whether the chase had been necessary and called attention to the danger police pursuits pose to uninvolved motorists. Troy Police Chief Shawn McKinney maintained that Marker acted within the department's pursuit policy.

Vollmer's estate filed a wrongful-death lawsuit in 2023 against the City of Troy, Marker, Sgt. John Marshall, Capt. Zachariah Mumford and McKinney, alleging police should have terminated the pursuit because of its extreme speeds, traffic conditions and danger to the public. In July 2026, the Ohio Second District Court of Appeals ruled that the city and officers were entitled to governmental immunity and ordered judgment in their favor.

Shane Marker was the Troy police officer who pursued Alexander. Marshall, Mumford and McKinney were supervisory officials whose actions during the pursuit were examined in the subsequent wrongful-death litigation. McKinney testified that Alexander's warrants and police belief that he might possess a handgun justified continuing the pursuit. Troy police had conducted 12 pursuits in 2020 and terminated six of them for safety reasons.

On 03/30/2021 shortly after 8:00 a.m., Chelsey R. Vollmer, 32, of Dayton was driving to work with her young child secured in the back seat when a Jeep driven by Jalen Alexander, 19, approached the intersection of U.S. 40 and State Route 202 during a high-speed Troy Police Department pursuit.

Officer Shane Marker had been looking for Alexander, who had outstanding Miami and Greene County warrants. The Miami County warrant arose from an earlier incident in which Alexander was accused of firing a gun at his girlfriend's father. Marker saw a Jeep associated with Alexander leave an Imperial Court residence and followed it before attempting a stop.

Alexander continued away and Marker pursued for approximately 11 miles. The chase lasted about seven and a half minutes and reached approximately 120 mph. During the pursuit, Alexander crossed center lines, traveled through intersections and nearly struck other vehicles. Marker reported backing off at points during the chase, and Chief Shawn McKinney directed that the pursuit be terminated if it reached traffic in Huber Heights.

A Miami County sheriff's deputy deployed stop sticks at State Route 202 and Ross Road. Alexander attempted to avoid them, but the Jeep's right-front tire was punctured. Police said the crash occurred too soon afterward for the stop sticks to have substantially deflated the tire.

Alexander continued toward U.S. 40 and entered the intersection against a red light. His Jeep struck Vollmer's Volkswagen broadside at approximately 108 mph. Vollmer and Alexander were pronounced dead at the scene. Alexander's passenger, Tashaya Tipton, survived. Vollmer's young child also survived and was taken to Dayton Children's Hospital with injuries.

Neighbors who reached Vollmer's vehicle after the crash heard her child crying and remained with the child until paramedics arrived. The child was alert with a bump and scratches and was later expected to be released from the hospital.

In January 2023, Marsha Gillespie, administrator of Vollmer's estate, filed a wrongful-death lawsuit against the City of Troy, Officer Marker, Sgt. John Marshall, Capt. Zachariah Mumford and Chief McKinney. The lawsuit alleged police continued an extraordinarily dangerous pursuit despite speeds exceeding 120 mph, heavy traffic and Alexander's dangerous driving, and alleged the pursuit should have been terminated under department policy. The city and officers denied wrongdoing and asserted statutory immunity.

The case remained active for years. In 2026, the Ohio Second District Court of Appeals reversed the trial court's refusal to grant immunity. On 07/02/2026, the appellate court ruled that Marker, Marshall, Mumford, McKinney and the City of Troy were legally immune from damages, finding the evidence insufficient to establish the level of willful, wanton or reckless conduct required to overcome Ohio governmental immunity. The case was remanded for entry of judgment in their favor.

On March 29, 2021, at approximately 9:11 a.m., officers responded to a domestic incident at a home involving Pierce L. Flinchum. When approached by police, Flinchum ran into the house and barricaded himself inside. Police obtained an arrest warrant for him on charges of assault and battery, and possession of a firearm as a felon. Throughout the day, police made multiple attempts to contact him by telephone. At approximately 10:30 p.m., officers entered the home and discovered Flinchum dead from a self-inflicted gunshot wound.

NARRATIVE:
On 03/29/2021 at approximately 3:00 a.m., Nathaly Medina, 19, was riding in the back seat of a car driven by 23-year-old Jose Alan Lara-Garcia when Tulsa police encountered the vehicle driving recklessly near East 61st Street and South Peoria Avenue. Officers attempted to stop the car, and Lara-Garcia continued west on 61st Street while police pursued.

The pursuit continued toward Riverside Drive. At 61st Street and Riverside, Lara-Garcia lost control, traveled through the intersection, grazed a pole and launched the car off the roadway into the Arkansas River.

A male passenger was found unconscious in the river. A Tulsa police officer entered the water and rescued him. He was hospitalized in critical condition. Lara-Garcia survived the crash and was also recovered from the river. Medina could not initially be located, and police determined that a third person had been inside the vehicle.

On the morning of 03/30/2021, after river conditions had calmed and the water receded, the Tulsa Police Department Dive Team located Medina's body near where the vehicle had entered the Arkansas River. Medina would have turned 20 the following day.

Police initially questioned Lara-Garcia after he was evaluated at a hospital and released him. After Medina's body was recovered, police sought him on a first-degree manslaughter charge. Lara-Garcia subsequently turned himself in and was booked into the Tulsa County Jail on manslaughter and eluding charges. Police stated that he had not been wanted on an outstanding warrant before the pursuit began.

On 03/29/2021, at approximately 3:50 a.m., LAPD officers attempted to stop Matthew Blaylock near 7th and Bixel Streets. Blaylock fled on foot, and officers escalated a chase that ended near 8th and Bixel Streets, where Blaylock allegedly produced a handgun. Officers opened fire, killing him.
