OPEN RECORDS SUMMARY :

Herman Whitfield was experiencing a mental health crisis and his father had asked for an ambulance. Police arrived and it was obvious that Whitfield had difficulty understanding police. He wandered around the house in this confused state while police repeatedly tased him, handcuffed him and restrained him, using their body weight on top of him. He died during the ordeal. No mental health help was ever provided.
