On June 16, 2021, at approximately 7:35 a.m., deputies from the Guilford County Sheriff's Office responded to a residence on Nellie Gray Place in Whitsett, North Carolina, following reports of a juvenile suffering from a gunshot wound. The child, the 3-year-old son of Elon Police Officer Orlando Maynard, accessed an unsecured firearm within the home. He was transported to a local hospital, where he was pronounced dead.

Officer Maynard, who was not present at the time of the incident, was charged with failure to store a firearm to protect a minor, a misdemeanor. He pleaded guilty to the charge and was sentenced to 18 months probation, during which he is prohibited from possessing firearms. The Elon Police Department announced that Officer Maynard’s status as a sworn officer is under review by the North Carolina Criminal Justice Training and Standards Commission.
