OPEN RECORDS SUMMARY :

On April 9, 2024, at approximately 7:00 p.m., officers from the North Vernon Police Department and Jennings County Sheriff's Office responded to a disturbance call at 25 Thomas Street in North Vernon, Indiana. Officers repeatedly instructed Rachel Danielle Blake, 23, to drop a knife while attempting to disarm her with tasers. Blake allegedly advanced toward the officers, leading Sgt. Adam Driver and Officer Allen to open fire. Blake was pronounced dead in her home by the Jennings County Coroner's Office.

The North Vernon Police Department and Jennings County Sheriff's Office requested the Indiana State Police to conduct an independent investigation. All involved officers were placed on administrative leave. Upon completion, the findings were submitted to the Jennings County Prosecutor's Office for review.
