Deputies reportedly observed Welch operating a motorcycle at high speed on Sweden Road in Waterford and attempted to conduct a traffic stop. Welch did not stop. Officers escalated a pursuit at high speeds along rural roads.

According to the Sheriff's Office, deputies briefly lost sight of the motorcycle and moments later discovered it crashed into a tree on Sweden Road. Welch was pronounced deceased at the scene.

The crash is under investigation by the Maine State Police, and findings will be reviewed by the Oxford County District Attorney's Office.

On September 29, 2024, Anchorage Police responded to a mental health crisis call involving James V. Afuvai, a 24-year-old man with a knife. Afuvai made several confusing 911 calls, one of which involved him falsely reporting gunfire and claiming to be the victim of a threat. Upon arrival, Sgt. James Dokken encountered Afuvai, who was allegedly holding a 14-inch knife and advancing toward the officer. Despite verbal commands to stop and retreat, Afuvai continued to approach. Sergeant Dokken fired his weapon and fatally shot him. The entire altercation lasted approximately 15 seconds, and no bystanders were injured. Afuvai’s family expressed confusion, noting that he had been diagnosed with schizophrenia but had never shown violent behavior before. The incident was the second fatal officer-involved shooting of a Samoan Native in Anchorage within six weeks.

Valencia deputies were called to the 100 block of Ladera Road in Los Lunas for a man arguing with neighbors and firing a gun. The officers were not able to resolve this and returned multiple times. During their last visit, they saw the quarrelsome man point guns at his neighbor, Michael Gabaldon who was allegedly defending himself on his property. Police shot and killed Gabaldon, stating he had a gun. Neighbors and his family in unison said that Gabaldon had been a “very good” person and the other neighbor was constantly threatening everyone in the neighborhood. Police were reportedly aware of this because there had allegedly been a number of similar previous incident involving this neighbor. Michael Gabaldon had been a former sheriff's deputy.

Mesa Police officers engaged Edward Johnson, 37, during an attempted traffic stop while he was riding a bicycle near Crismon and Broadway Roads on 09/29/2024. Officers pursued Johnson after he continued biking away, eventually encountering him near a canal. Police claimed Johnson pointed what appeared to be a firearm at them, prompting officers to shoot him. Johnson was transported to a hospital, where he succumbed to his injuries. Authorities later determined that the firearm he allegedly displayed was a replica gun.

On 09/29/2024, Holland Public Safety officers responded to a reported altercation at a gas station on Washington Avenue. A vehicle rapidly departed the scene without headlights. Officers escalated a high-speed chase that lasted several minutes. Alfredo Mojica Majin, the driver, lost control of the vehicle near the intersection of 32nd Street and South Shore Drive. The vehicle struck a tree, causing severe damage and catching fire. Alfredo and his brother Bryan Mojica, the passenger, were pronounced dead at the scene.

Plano PD were sent to a home in the 3800 block of Pratt Street for a “shots fired” call. Police found Russell Novak, 70, dead from multiple gunshot wounds. His son Nicolas Novak had fled the residence. Police located him and escalated a chase from Plano to Aurora including with helicopters. On September 28, 2024, at approximately 6:20 p.m., a man contacted the Lone Tree Police Department, reporting that he had fired a shot inside his apartment and was experiencing suicidal thoughts. Officers from the Lone Tree Police Department, along with deputies from the Douglas County Sheriff's Office and a mental health clinician, responded to the scene in the 12000 block of Roosevelt Lane in unincorporated Douglas County.

Upon arrival, crisis negotiators engaged the man via phone, urging him to exit the apartment peacefully. Approximately an hour later, around 7:20 p.m., the man emerged from the residence armed with a handgun, discharged a round into the air, and then pointed the weapon at the deputies. In response, several deputies fired at the man, who was subsequently transported to a hospital and pronounced dead.

The 18th Judicial District Critical Incident Response Team is conducting an investigation into the incident. As of now, the man's identity has not been released pending notification of next of kin.
