On 08/19/2024, Gerald Dwain “Jerry” Rahier II, 42, was killed when Kansas Highway Patrol Trooper Ethan Masterson crashed his patrol SUV into the driver’s side of Rahier’s 1999 Chevrolet Tahoe at the rural intersection of 100 South Road and 40 East Road in Sheridan County. Masterson was responding to a separate fatal crash on K-23 south of Hoxie.

Masterson was traveling west on 100 South Road while Rahier was traveling south on 40 East Road. Kansas Highway Patrol reported that Masterson had his emergency lights and siren activated as he approached the intersection. Masterson entered the intersection and struck the driver’s side of Rahier’s Tahoe. On 08/15/2024, at approximately 2:23 a.m., Morrisville police responded to a reported armed robbery and carjacking at the Camden Westwood Apartments on West Shore Court. The victim reported being assaulted by four individuals, one reportedly armed with a handgun, before his 2016 BMW SUV was taken. About eight minutes later, a Morrisville police officer located the BMW traveling on Chapel Hill Road toward Durham and initiated a pursuit.
The pursuit continued toward Durham before officers lost sight of the BMW near the Chapel Hill Road/Page Road area. Morrisville police terminated the pursuit. Approximately two hours later, at about 4:41 a.m., a passing motorist discovered the BMW crashed in a field off the 5900 block of South Miami Boulevard in Durham and notified authorities. Durham officers found the 16-year-old driver had been ejected from the vehicle and pronounced him dead at the scene. Investigators determined he was the only person inside the SUV when it crashed.
The crash investigation was conducted by the Durham Police Department while the Morrisville Police Department conducted an internal review of the pursuit. One Morrisville officer involved in the pursuit was placed on administrative leave pending that review. Morrisville Police Chief Pete Acosta later stated the review concluded departmental pursuit procedures had been followed. The juvenile's identity has not been publicly released because of his age. Police reported that Moore was wanted on an outstanding felonious-assault warrant. Officers attempted to take him into custody, but Moore continued driving away while police pursued. Bird activated his emergency lights and siren, but the driver continued east at high speed, used the highway shoulder and exited at Exit 12 while Bird pursued. The Explorer then traveled south on County Road 521 through Hope Township. The publicly released investigation did not specify the traffic violation that prompted Bird to initiate the stop.
At approximately 10:20 a.m., the Explorer moved into the opposing lane to pass another vehicle near Dogwood Road. As it returned to the southbound lane, the Explorer struck a motorcycle operated by 76-year-old Michael J. Ceddia Jr. Ceddia was a bystander traveling on County Road 521 and was not involved in the reason for the traffic stop or pursuit. The impact threw him from the motorcycle. On 02/10/2023, 28-year-old Delmy “Patty” Alvarez was killed when a Lexus being pursued by Orlando Police Officer Esequiel Colon crashed into the Honda Civic she was driving at South Parramore Avenue and West Anderson Street in Orlando. Alvarez was completely uninvolved in the police encounter. Her girlfriend, Gina Mustacchio, was riding in the passenger seat and survived with devastating injuries.

The pursuit began at approximately 10:18 p.m. when Colon saw a Lexus run a red light at Westmoreland Drive and South Street in the Parramore neighborhood. Colon later told an Orlando Police internal-affairs investigator that he considered running the red light reckless driving. This traffic violation—not a violent felony—was the specific initiating reason for Colon's decision to follow the vehicle.

Colon began following the Lexus in a marked Ford Explorer. As its driver increased speed, Colon continued behind him through residential city streets. Internal police records subsequently established that the vehicles reached speeds exceeding 60 mph.

Critically, Colon did not activate his emergency lights or siren during the pursuit. He later acknowledged this during the internal investigation. Consequently, Alvarez and other motorists approaching the eventual crash intersection received no emergency lights or siren warning that a high-speed police pursuit was entering the intersection.

The Lexus ran another red light at Parramore Avenue and West Anderson Street and struck the driver's side of Alvarez's Honda. Alvarez died from her injuries. Mustacchio suffered a broken jaw, multiple rib and pelvic fractures, a broken arm and injuries that ultimately cost her a kidney.

Orlando Police Department's own investigation determined that Colon's pursuit violated department policy. OPD policy permitted vehicle pursuits when an officer had reasonable suspicion that the fleeing person had committed or attempted a specified forcible felony. Running a red light did not qualify.

The internal investigation established that Colon himself acknowledged he did not have reasonable suspicion that the driver had committed or attempted a forcible felony and that the incident did not meet OPD's criteria for a vehicle pursuit.

The department nevertheless stopped short of attributing Alvarez's death to Colon. Colon received a 40-hour suspension and additional training for violating the pursuit policy. Under the police union agreement, he was permitted to surrender five days of paid leave instead of losing pay during the suspension.

No independent law-enforcement agency investigated Colon's role in Alvarez's death. The Orlando Police Department investigated both the fatal crash involving its own officer and Colon's policy violations. OPD later said there was no procedure requiring an outside investigation for a crash of this type.

The department's handling of the aftermath became a major part of the case. OPD did not issue a public news release explaining that Alvarez had been killed during a police pursuit and did not tell her mother, Marta Lopez, the full circumstances of her daughter's death. Lopez initially understood only that a reckless driver had run a red light and killed Delmy.

For months, OPD also denied an Orlando Sentinel public-records request by citing an active-investigation exemption even though the administrative investigation of Colon had already concluded. When records were eventually produced, substantial portions were redacted. Unredacted internal records became available through legal discovery in Mustacchio's civil litigation.

Lopez said she first began learning that police had been pursuing the Lexus through Mustacchio rather than through the department. When Lopez later confronted the crash investigator about the existence of a police pursuit, she recalled his response as asking who had told her.

The family has consequently been outspoken in challenging OPD's handling of Alvarez's death. Lopez said she believed the crash could have been prevented and explicitly assigned responsibility both to the pursued driver and to police. She criticized the department for failing to contact her or keep her informed.

Mustacchio likewise became an outspoken critic of the department. She argued that although the pursued driver was responsible for his actions, Colon's decision to chase him without emergency lights or siren materially contributed to the circumstances that killed Alvarez.

Mustacchio sued the City of Orlando, alleging that Colon's unauthorized pursuit caused or contributed to the collision. The lawsuit specifically cited Colon's violation of OPD's restrictive pursuit policy and his failure to use emergency lights and siren.

The City of Orlando settled the litigation in 2025 for $300,000. The settlement did not require the city to admit legal responsibility. Mustacchio said her purpose in bringing the case was accountability and exposing what she believed OPD had attempted to keep from public view.

Although police possessed evidence from the crash, charges were not submitted to the Orange-Osceola State Attorney's Office until September 2024, approximately 19 months after Alvarez was killed and only days after investigative reporting publicly exposed OPD's role in the pursuit.

Records showed that DNA evidence from the Lexus airbag and suspected marijuana recovered from the vehicle had been submitted to FDLE in April 2023. OPD did not report receiving the DNA result until September 2024. The driver was ultimately arrested in February 2025 and prosecuted for the fatal crash. At his 2025 sentencing proceedings, Mustacchio took the unusual position of speaking partly in his defense because she believed Colon and OPD also bore substantial responsibility. Alvarez's mother likewise told the court she believed there were two responsible parties: the driver and the pursuing police officer.

The case gained renewed significance because OPD subsequently experienced another fatal pursuit involving an officer violating the department's pursuit restrictions. In February 2025, Detective Christopher Moulton attempted to stop a driver over an unreadable license plate; the ensuing pursuit ended with 49-year-old Gerald Neal being killed. On 08/11/2024, deputies with the Lowndes County Sheriff's Office were responding to an active domestic violence call on Mississippi Highway 12 in Steens with emergency lights and sirens activated. According to Lowndes County Sheriff Eddie Hawkins, one deputy had already passed 85-year-old Sarah Elizabeth Jackson while responding to the emergency. On 08/10/2024, at approximately 4:11 a.m., 21-year-old Joshua Andrew VanderZiel was riding his motorcycle on Washington Boulevard (U.S. Route 1) in Howard County when Anne Arundel County Police Officer Alexander Rodriguez began pursuing him. State investigators determined Rodriguez initiated the pursuit without authorization, never notified dispatch, never activated his emergency lights or siren, and never obtained permission to continue the pursuit into Prince George's and Howard counties. The Attorney General's Office has not publicly identified the reason Rodriguez initiated the pursuit.
Investigators determined Rodriguez drove at speeds approaching 100 mph while following VanderZiel for more than four miles across three counties. VanderZiel's motorcycle collided with a flatbed truck that was making a left turn onto Washington Boulevard. He died at the scene from his injuries.

On 08/08/2024, a Washington State Patrol trooper attempted to stop a Jeep Grand Cherokee on Boeing Access Road. On 08/07/2024, at approximately 9:50 p.m., a Town of Deerpark police officer attempted to stop 31-year-old Wade Christopher Conklin Jr. while he was riding a motorcycle on U.S. Route 209 near the Port Jervis city boundary.
Conklin continued riding away at high speed while Deerpark police pursued him toward Port Jervis. He entered the city on Kingston Avenue and continued onto East Main Street. Port Jervis police joined the pursuit after the motorcycle entered their jurisdiction.
Conklin traveled west on East Main Street, passed through two red traffic signals and entered the intersection of East Main Street and Orange Street, where the motorcycle collided with an SUV. The collision ejected Conklin from the motorcycle and threw him into a light pole in the center median.
Emergency personnel transported Conklin to Bon Secours Community Hospital, where he was pronounced dead from blunt-force trauma.
Conklin was originally from Middletown and was a member of the H.O.O.D. Ryderz Motorcycle Club in Port Jervis. His obituary described him as a former Middletown High School varsity diver, football player and lacrosse player who was devoted to his family and protective of his friends.
The New York State Police investigated the crash. The New York Attorney General’s Office of Special Investigation opened a review because Conklin died during a police pursuit involving the Town of Deerpark Police Department and Port Jervis Police Department. n 08/07/2024, Bernalillo County Sheriff's Office Deputy Bryan Lassley reported that he attempted to stop a silver Honda he considered suspicious while driving north on Edith Boulevard NE shortly after midnight. Lassley reported over the radio that he lost sight of the vehicle and moments later stated the Honda had stopped abruptly.
Deputy Lassley struck the rear of Alexandria Gerard's Honda with his marked Ford Explorer. The impact sent Gerard's vehicle into a stone wall at the intersection of Edith Boulevard NE and Alameda Boulevard NE. Gerard was trapped behind the steering wheel and died at the scene. Deputy Lassley was not injured.
Bernalillo County Sheriff John Allen requested that the Albuquerque Police Department conduct the criminal traffic investigation to avoid a conflict of interest. Deputy Lassley was placed on paid administrative leave while the investigation proceeded.
The Albuquerque Police Department's crash investigation later concluded that data downloaded from Lassley's patrol vehicle showed he accelerated to approximately 70 mph in a 25 mph zone seconds before the collision. On 08/05/2024, a Dallas police officer driving a marked patrol vehicle was traveling eastbound on Elam Road in the 8700 block while responding to a service call without emergency lights or siren activated. Twenty-year-old Aresly Jaramillo attempted to cross Elam Road near Cedarcliff Drive after passing in front of two vehicles waiting to turn left. The patrol vehicle struck Jaramillo in the far-left lane. The officer stopped immediately and rendered first aid until Dallas Fire-Rescue personnel arrived. Jaramillo was transported to a hospital in critical condition and died on 08/06/2024 from her injuries.
On 08/14/2024, Jaramillo's family announced plans to pursue a claim against the City of Dallas under the Texas Tort Claims Act. The family's attorney stated dash-camera footage showed the patrol vehicle was speeding while responding to a call that did not require emergency lights or sirens. The family contended the City and Dallas Police Department were negligent and also called for installation of pedestrian crosswalks near Crawford Memorial Park.
