On December 15, 2021, at approximately 5:30 a.m., Elkhart Police Department officers responded to a report of a man attempting to break a liquor store window using a bicycle near the intersection of West Hively and Benham Avenues in Elkhart, Indiana. Upon arrival, officers encountered 20-year-old Jahad Patterson in a nearby parking lot.
When officers identified themselves and ordered Patterson to show his hands, he allegedly drew a firearm and began raising it toward Officer Scott Swanson. In response, Officer Swanson and Sergeant Benjimin Martin discharged their weapons, fatally striking Patterson.

A .380 caliber handgun was recovered from Patterson, with his finger still inside the trigger guard. The Elkhart County Prosecutor's Office later determined that the officers' use of deadly force was justified, citing self-defense.

On December 11, 2021, at approximately 3:15 a.m., Kokomo Police attempted to stop a 2016 Nissan in the area of Mulberry and Washington Streets. The vehicle did not stop, and officers initiated a pursuit. After losing sight of the vehicle, officers terminated the chase. Shortly thereafter, police were dispatched to a crash at North Berkley Road and West Jefferson Street. The Nissan had collided with a Dodge Ram while heading westbound. Sydney Leigh Glenn, a 24-year-old passenger in the Nissan, was pronounced dead at the scene. The driver and another passenger were injured, as was the driver of the Dodge Ram.

On November 28, 2021, at approximately 2:04 p.m., the BP Express gas station in Culver, Indiana, reported an intoxicated individual attempting to purchase alcohol. The individual, later identified as 33-year-old Dylan Wade Bush, departed the scene in a white Chevrolet Cavalier before officers arrived.

A Culver Police Department officer located Bush's vehicle near State Road 17 and Washington Street around 2:07 p.m. and initiated a pursuit when Bush failed to stop. The chase, involving high speeds exceeding 100 mph, continued through Marshall County for approximately 13.5 miles.

Sergeant Matthew Brown of the Marshall County Sheriff's Department joined the pursuit near State Road 10 and Sycamore Road. The pursuit concluded when officers boxed in Bush's vehicle at the First National Bank of Monterey's parking lot on North Lake Shore Drive. Sergeant Brown positioned his patrol car in front of Bush's vehicle and exited to issue commands. Bush ignored the commands and accelerated, pinning Sergeant Brown between the vehicles. In response, Sergeant Brown fired three shots from his service weapon, striking Bush.

After being shot, Bush's vehicle traveled several hundred feet before colliding with an unoccupied pickup truck in a nearby parking lot. Officers administered first aid until paramedics arrived, but Bush was pronounced dead at the scene by the Marshall County Coroner.

An autopsy revealed Bush had a blood alcohol content of .23%, nearly three times the legal limit, and the presence of cannabinoids and methanol in his system. The Marshall County Prosecutor's Office determined Sergeant Brown's use of deadly force was justified.

At approximately 12:52 p.m. on November 3, 2021, LaPorte County deputies initiated a pursuit of a reportedly stolen 2020 Infiniti QX60. The vehicle was traveling at high speeds and avoided stop sticks placed on the roadway. The chase continued into Lake County, where the Infiniti reportedly collided with a Porter County Sheriff's vehicle. The impact caused the driver, 23-year-old John Kennedy Fitzgerald Bolden, to lose control, resulting in the vehicle hitting a median wall and rolling over multiple times. The vehicle came to a stop on the opposite side of the median wall. Bolden and a passenger were ejected from the vehicle. Bolden was killed, and the passenger sustained injuries.

On April 14, 2021, a Henry County Sheriff’s Deputy attempted to stop a vehicle for a traffic violation and suspected impaired driving. The driver, identified as Zacery Kane Antrim, fled through Henry and Delaware Counties before crashing into another vehicle at McGalliard Road and Tillotson Avenue in Muncie. The passenger, 25-year-old Nicholas Wayne Wollum, was pronounced dead at the scene. Both Antrim and the other driver, 47-year-old Shanta Sharrel Schmiesing, were transported to a hospital in critical condition. The Indiana State Police are investigating the pursuit and crash.
