They found 56-year-old Regina Darlene Smith shot and killed in a vehicle, and a 79-year-old man critically injured. Deputies identified 43-year-old Catrell Ford as the shooter. Later that evening, deputies located Ford at a convenience store near Spring Street and Riverside Drive. He ran into nearby woods and exchanged gunfire with deputies. On 08/31/2025, shortly after 12:45 a.m., Phoenix Police officers Matthew Olmstead and Matthew Paz encountered 36-year-old Efrain Hidalgo while he was riding a bicycle near Hermoso Park at 20th Street and Southern Avenue. Early police statements did not explain why officers had targeted Hidalgo for investigation, but records and the later body-camera release established that police stopped him because his bicycle did not have a headlight. Hidalgo was Latino.

Olmstead and Paz got out of their patrol vehicle to stop Hidalgo. Hidalgo moved away from them on foot, and Paz chased him a short distance and tackled him to the ground. Paz and Hidalgo struggled on the ground for several seconds. Police alleged that Hidalgo then removed a knife. Video shows Olmstead repeatedly asking what was in Hidalgo's hand and then warning Paz that Hidalgo had a knife.

Olmstead then fired nine rounds at Hidalgo from approximately 10 feet away. The body-camera footage shows that Olmstead did not order Hidalgo to drop the knife before opening fire. Hidalgo fell after approximately the third shot, but Olmstead continued firing. Police have not disclosed precisely how many bullets struck Hidalgo, although an officer's report described multiple gunshot wounds.

The video does not clearly establish a police claim that Hidalgo swung the knife at Paz. Paz later told another officer that Hidalgo had swiped at him with the knife, but Phoenix New Times reported that the available footage was too unclear to confirm that account visually. A knife was recovered beside Hidalgo.

After the shooting, Hidalgo lay largely motionless on the ground. Approximately 30 seconds later, officers ordered him to roll away from the knife so they could help him, but no response from Hidalgo can be heard on the recording. Police waited approximately five minutes after the shooting before officers approached and began medical treatment. Officer Joseph Corso subsequently wrote that Hidalgo had no pulse, was not breathing and had fixed and dilated pupils when officers began treating him. Hidalgo was transported to a hospital and pronounced dead at approximately 1:28 a.m.

The Arizona Department of Public Safety's Major Incident Division conducted the outside criminal investigation. The case was submitted to the Maricopa County Attorney's Office on 01/06/2026. On 01/29/2026, the County Attorney's Office listed the Hidalgo shooting as “Cleared,” ending the criminal review without charges against the officers.

Phoenix Police separately conducted an internal administrative review of the officers' actions. The department released a critical-incident briefing and body-camera footage in September 2025. The footage and additional police records obtained by Phoenix New Times provided substantially more detail than the department's initial public account, including the bicycle-light basis for the stop, Paz's tackle, the nine shots fired by Olmstead, the absence of a command to drop the knife before the gunfire, and the approximately five-minute delay before officers began medical aid.

Matthew Olmstead was the Phoenix Police officer who fired all nine shots at Hidalgo. Matthew Paz was Olmstead's partner and tackled Hidalgo before becoming physically engaged with him on the ground immediately before the shooting. On 08/30/2025, at approximately 7:30 p.m., Aurora Police Department Officer Matthew Neely was conducting speed enforcement on East 6th Avenue as part of a Labor Day weekend DUI operation. Neely reportedly observed Rajon Montrel Lee Belt-Stubblefield driving above the speed limit near Toledo Street and activated his emergency lights and siren. Belt-Stubblefield continued driving away while Neely followed at increasing speed with his emergency equipment activated. Approximately one minute and 20 seconds later, Belt-Stubblefield swerved, struck one vehicle, crossed a median and collided with a second vehicle near East 6th Avenue and Billings Street.
Neely arrived while Belt-Stubblefield’s damaged vehicle was stopped in an opposing traffic lane and emitting smoke. Neely immediately exited his patrol vehicle with his handgun drawn and approached Belt-Stubblefield, initially ordering him to remain inside the vehicle and then giving conflicting commands to show his hands and raise them. Loud music was playing inside the vehicle. Belt-Stubblefield opened the door, said, “Don’t shoot me,” exited and walked toward a grassy area beside the roadway.
Video from Neely’s body camera, civilian vehicles, nearby surveillance cameras and witnesses showed Belt-Stubblefield throw a handgun into the grass. Neely grabbed Belt-Stubblefield by his clothing and pulled him to the ground but did not handcuff him. Belt-Stubblefield stood up and moved away. Neely continued pointing his handgun, pushed Belt-Stubblefield and then struck him in the side of the head while repeatedly ordering him onto the ground.
Belt-Stubblefield gestured toward the discarded firearm and repeatedly told his adult son, who had arrived in another vehicle, to retrieve it. Neely ordered those present to stay away from the gun. Belt-Stubblefield then faced Neely, raised his hands into a fighting posture and repeatedly asked whether Neely was ready. His son pleaded with both men to calm down. Neely backed approximately 50 feet along the sidewalk and into East 6th Avenue while continuing to point his handgun at Belt-Stubblefield.
Neely fired two rounds into Belt-Stubblefield’s upper body. After those bullets stopped Belt-Stubblefield’s advance, Neely fired a third round into his head. Belt-Stubblefield was unarmed when Neely shot him. The handgun remained in the grass behind him. The grand jury later found that Belt-Stubblefield never verbally threatened to harm Neely and made no overt attempt to disarm Neely or recover the discarded firearm.
Neely stood over Belt-Stubblefield with his handgun pointed at him after reporting that shots had been fired. A woman who identified herself as an emergency medical professional approached with gauze and a tourniquet, but Neely rejected the offered supplies and did not permit her to begin treatment. Additional officers and paramedics arrived approximately two minutes after the shooting and began medical care. Belt-Stubblefield died from three gunshot wounds, including wounds to his chest or shoulder and head. His 19-year-old son, Zion Murphy, witnessed the shooting.
The Arapahoe County Coroner’s Office classified Belt-Stubblefield’s death as a homicide. Toxicology testing documented a blood-alcohol concentration of .274. Family members and their attorneys said his intoxication and the preceding violent collision impaired his ability to understand and respond appropriately, but did not create a lethal threat justifying the shooting.
Aurora Police Chief Todd Chamberlain publicly defended Neely within days of the killing, blamed Belt-Stubblefield for creating the confrontation and described Neely’s physical blows as attempts at de-escalation. Chamberlain made those statements before Neely had submitted a formal account or been interviewed by investigators. Belt-Stubblefield’s family and attorneys accused the department of constructing a public narrative to justify the killing before the investigation was completed.
The Critical Incident Response Team investigated the shooting and submitted its evidence to 18th Judicial District Attorney Amy Padden. After reviewing the investigation and consulting experts in police tactics, operations and use of force, Padden presented the case to an Arapahoe County grand jury.
On 07/31/2026, the grand jury indicted Neely on one count of second-degree murder, a Class 2 felony, and one count of manslaughter, a Class 4 felony. The indictment alleged that Neely never told Belt-Stubblefield he was under arrest and deliberately did not give an immediate warning before firing because Neely wanted to preserve an “element of surprise.” The grand jury concluded that Belt-Stubblefield had not threatened to harm Neely, attempted to disarm him or attempted to rearm himself.
An arrest warrant was issued for Neely following the indictment. He surrendered on 08/01/2026 and was released after posting a $200,000 cash or surety bond. The Aurora Police Department placed him on unpaid administrative leave. His next court appearance was scheduled for 08/17/2026.
The indictment is especially significant because the central justification originally offered for the shooting—that Neely faced an imminent lethal threat—is directly disputed by the grand jury’s factual findings. Belt-Stubblefield had discarded the firearm, remained separated from it, made no movement to recover it and did not attempt to take Neely’s gun. The third gunshot was fired into Belt-Stubblefield’s head after the first two rounds had stopped his forward movement. The indictment also identifies Neely’s failure to announce an arrest, his decision to withhold an immediate shooting warning and his deliberate effort to retain tactical surprise as evidence supporting the criminal charges.
Belt-Stubblefield’s wife, Tandra Blankson, his son and other family members filed two notices of claim against Aurora in February 2026. The contemplated wrongful-death and emotional-distress lawsuits each seek at least $20 million. Attorneys Ben Crump and Milo Schwab are among the lawyers representing the family. Community organizations, clergy members and the Aurora and Rocky Mountain branches of the NAACP organized vigils and repeatedly called for Neely to be criminally prosecuted.
Neely joined the Aurora Police Department as a recruit in 2002, retired in 2023 and was rehired in March 2024. His personnel history included an internal-affairs investigation arising from a 2019 burglary case. An Arapahoe County judge found that Neely and Officer Paul Timmons gave testimony inconsistent with body-camera footage and ruled that neither officer was credible. The judge found that the arrest was unlawful, the force used was unjustified and the officers detained, searched and questioned the man without constitutional justification.
Aurora’s internal investigation sustained findings that Neely violated department policies governing conduct unbecoming an officer and unsatisfactory performance. The department did not sustain allegations involving dishonesty, unconstitutional conduct or improper force, and the disciplinary penalty was not publicly specified. The 2019 court findings nevertheless established a documented prior instance in which video evidence contradicted Neely’s account and a judge rejected his testimony concerning police force and constitutional authority.

The patrol said the trooper observed the Tahoe failing to remain within a single lane and nearly striking a guardrail several times. Dane County authorities had issued a statewide alert seeking the vehicle because it had been taken from the scene of the killing of 70-year-old Thomas Muckler at a cabin in the Town of Mazomanie. Police had been instructed to locate the vehicle and identify anyone inside it.

A Marinette police officer located the Hyundai in the City of Marinette and attempted to stop it. Collamore continued driving away while police pursued him south on US Highway 41. Multiple law-enforcement vehicles joined the pursuit.

Investigators later reviewed squad-camera recordings, dispatch records and other evidence and determined that Collamore reached speeds of approximately 105 mph. Police alleged that he drove between other vehicles as the pursuit continued southbound.

Marinette County Sheriff Randy Miller positioned spike strips on US Highway 41 in an attempt to stop the Hyundai. Collamore avoided the spike strips but lost control of the SUV. The vehicle spun out, rolled multiple times across the median and entered the northbound side of US Highway 41 before coming to rest near Schacht Road in the Town of Peshtigo.

Marinette Police officers Parker J. De Los Santos and Andrew D. Bonjean approached the crashed vehicle. Collamore eventually climbed out through the passenger side. Police said he was holding a knife. Officers repeatedly ordered him to drop it while attempting to communicate with him.

Body-camera and squad-camera recordings subsequently reviewed by the Marinette County District Attorney captured the confrontation. According to District Attorney DeShea Morrow’s written review, Collamore continued walking away from officers while holding the knife. At one point he referred to “Area 51” and told officers they were supposed to execute him.

Sheriff Randy Miller approached Collamore carrying a Taser. According to the district attorney’s review of the video and investigative evidence, Collamore then turned and ran toward De Los Santos while holding the knife. De Los Santos and Bonjean opened fire while Miller simultaneously deployed his Taser. The Taser was ineffective. Collamore was struck by police gunfire and collapsed.

Police and emergency medical personnel provided medical treatment before Collamore was transported to a hospital, where he was pronounced dead. Investigators recovered the knife at the scene.

The Wisconsin Department of Justice Division of Criminal Investigation conducted the independent investigation. DCI reviewed body-camera and squad-camera recordings, dispatch and CAD records, witness statements, physical evidence and the medical examiner’s findings. One of the Marinette officers had been wearing a body camera, and both officers’ squad-camera systems were operating.

On 10/03/2025, Marinette County District Attorney DeShea Morrow completed her review of the shooting. Morrow determined that De Los Santos and Bonjean acted legally when they shot Collamore and declined to file criminal charges. The district attorney then closed the criminal review of the shooting.

The man was subsequently identified by St. Clair County Coroner Calvin Dye Sr. as 53-year-old Matthew J. Blanke of O’Fallon.

When Officer Brett Johnson arrived, two women were running from the property. Later information released by the O’Fallon Police Department and Fraternal Order of Police identified them as Blanke’s wife and stepdaughter. Both had suffered stab wounds and were bleeding.

Johnson’s body-camera recording, released by Illinois State Police in February 2026, shows Johnson getting out of his patrol vehicle as the two women moved away from the property and Blanke approached from the front yard holding a knife. Johnson drew his handgun and twice ordered Blanke to drop the knife.

Blanke continued moving toward Johnson while holding the knife. Johnson then fired seven shots in approximately two seconds. Blanke fell forward onto the lawn.

Additional officers arrived and police performed chest compressions and other emergency medical treatment. Blanke was pronounced dead at the scene at 8:07 p.m. The two women were transported to a hospital and treated for their wounds.

The O’Fallon Police Department requested an independent investigation by the Illinois State Police. Investigators reviewed Johnson’s body-camera recording, patrol-car video and other evidence before submitting the completed investigation to the St. Clair County State’s Attorney’s Office.

On 11/25/2025, the St. Clair County State’s Attorney’s Office declined to file criminal charges against Johnson. Illinois State Police subsequently released the body-camera and patrol-car recordings on 02/03/2026 after consulting with prosecutors and providing Blanke’s family an opportunity to view the recordings.

Johnson’s identity was not disclosed in the initial reporting or in the February 2026 Illinois State Police video release. Police had been investigating Howard in connection with a series of residential burglaries and home invasions during August. Police alleged that firearms were taken during some of those burglaries and that on 08/27/2025 Howard entered an occupied home while armed with a handgun. The following day, the department's Major Crimes Suppression Unit organized an operation to locate and arrest him.

Detectives arranged to meet Howard at the shopping center as part of an operation involving the purported purchase of stolen property. Howard arrived on a motorcycle. Police watched him and confirmed his identity before three detectives—Marcus Gonzalez, Craig Pruitt and Ricardo Cardona—entered the ThunderCloud Subs shortly before 5:30 p.m.

The detectives were wearing police-marked vests and ordered Howard to get on the ground. Body-camera video subsequently released by Austin police shows Howard moving away from the detectives toward customer Fernando Martinez and pulling a handgun from a crossbody bag.

Martinez grabbed Howard and the two struggled. Police said Howard fired at least one round during the struggle. Gonzalez, Pruitt and Cardona then opened fire, striking Howard. Howard was pronounced dead at the scene at approximately 5:46 p.m.

According to the Minnesota Bureau of Criminal Apprehension, Thao allegedly fired repeatedly from inside the residence during the hours-long barricade. Police reported that gunfire struck a handheld ballistic shield carried by an officer and an armored SWAT vehicle. Police subsequently deployed chemical irritants into the residence, but Thao remained inside.

At approximately 6:30 p.m., Apple Valley Police Sgt. Sean Marben, who was participating as a member of the South Metro SWAT Team, was positioned on a SWAT vehicle outside the residence. The BCA said Marben saw Thao armed at the bottom of the basement stairs. West St. Paul police provided the more specific allegation that Thao pointed a long gun toward officers. Marben then fired his department-issued rifle, striking Thao.

Police entered the residence and provided medical aid. Thao was transported to a hospital, where he was pronounced dead. OFFICER BACKGROUND
Sgt. Sean Marben of the Apple Valley Police Department fired the shot that killed Jerry Va Thao. Marben was serving with the multi-jurisdictional South Metro SWAT Team during the operation.

Marben was previously involved in a 2018 arrest that resulted in both suppression of evidence and a federal civil-rights lawsuit. On 05/20/2018, Marben encountered James Paul Aery asleep in a vehicle in an Apple Valley Walmart parking lot. Marben woke Aery, and after another officer arrived, police searched Aery and recovered suspected methamphetamine and drug paraphernalia. Aery was subsequently prosecuted for drug possession.

A Dakota County judge later ruled that the search was unconstitutional. The prosecution conceded that the search was impermissible on its face but argued that the evidence would inevitably have been discovered because police had probable cause to arrest Aery for impaired driving. The judge rejected that argument, found insufficient probable cause for an impaired-driving arrest, suppressed the evidence and dismissed the criminal case.

Aery subsequently filed a federal civil-rights lawsuit in January 2020 naming Marben, another Apple Valley officer and a Dakota County prosecutor. Aery's claims against Marben and the other officer were dismissed with prejudice by stipulation in September 2020.

On 08/27/2025, at approximately 5:34 p.m., Iowa State Patrol Trooper Zach Jensen attempted to stop 48-year-old Michael Patrick Broyles while Broyles drove a Volkswagen Beetle south on Interstate 29 in Harrison County. Jensen had driven alongside the Volkswagen and determined that Broyles was not wearing a seat belt. The seat-belt violation was the specific reason for the attempted traffic stop.

Broyles did not immediately stop. After approximately three miles, he took the Loveland exit and stopped at the bottom of the ramp. According to the subsequent investigation and recorded video, Broyles got out carrying a double-barreled 12-gauge shotgun and pointed it toward Jensen. Jensen drew his handgun, took cover and repeatedly ordered Broyles to put the shotgun down. Jensen did not shoot. Broyles returned to the Volkswagen and drove back onto southbound I-29, and Jensen pursued.

Multiple agencies joined the pursuit, including the Iowa State Patrol, Harrison County Sheriff’s Office, Pottawattamie County Sheriff’s Office and Council Bluffs Police Department. Trooper Paul Hutson eventually took the lead because Jensen was driving an unmarked patrol vehicle. Police made several attempts to deploy stop sticks. At approximately 5:54 p.m., Pottawattamie County Sheriff’s Sgt. Doug Kelsey successfully deployed stop sticks near mile marker 57, deflating tires on both the Volkswagen and the small trailer it was towing.

Broyles continued south and exited at North 16th Street. At approximately 5:55 p.m., he stopped beneath the I-29 overpass, got out carrying the shotgun and moved behind a concrete support pillar. Police surrounded the position. Trooper Hutson repeatedly ordered Broyles to put down the shotgun and show his hands.
.
For approximately six minutes, police attempted to get Broyles to surrender. Trooper Jensen eventually retrieved a PepperBall launcher and fired several rounds toward him. At least one PepperBall struck Broyles in the arm.
.
Police alleged that at approximately 6:01 p.m., Broyles raised the shotgun toward Trooper Dillon Malone’s position and appeared to fire. Malone fired one rifle round but did not hit Broyles. Moments later, as the shotgun became visible from the other side of the pillar, Council Bluffs Officer Colton Thompson and Pottawattamie County Sheriff’s Sgt. Eric Shea fired.

Seconds later, five officers fired during the final volley: Iowa State Patrol Trooper Dillon Malone fired a second rifle round; Council Bluffs Officer Colton Thompson fired one rifle round; Council Bluffs Officer Nathan Classe fired three handgun rounds; Pottawattamie County Sheriff’s Sgt. Eric Shea fired three rifle rounds in total during the encounter; and Pottawattamie County Sheriff’s Sgt. Doug Kelsey fired two rifle rounds. Altogether, the five officers fired eleven rounds.

Broyles was struck twice. One bullet passed through his right forearm. Malone’s second rifle round struck Broyles in the right shoulder, traveled through both lungs and his spine, and lodged in his upper back. The autopsy and firearms examination determined that Malone fired the fatal shot.

Police waited until they could determine Broyles’ location and condition using a drone and then approached behind ballistic shields. Officers moved the shotgun away, handcuffed Broyles and began CPR. He was transported to Mercy Hospital in Council Bluffs and pronounced dead at approximately 6:36 p.m.

The Iowa Division of Criminal Investigation conducted the independent investigation. Investigators reviewed extensive body-camera, patrol-car, helicopter, drone and surveillance video; physical and firearms evidence; photographs; witness statements; dispatch information; and interviews with all five shooting officers.

On 10/01/2025, Pottawattamie County Attorney Matthew Wilber released a detailed report identifying all five officers who fired and declined criminal charges. The Iowa Attorney General’s Office conducted a separate review and also concluded that the shooting was legally justified.

Young was compliant and did not resist. Two days later, Washington investigators determined that Young was not the person wanted, the warrant was quashed, and he was released on 05/26/2018. Young subsequently sued Thompson, Brown, the City of Council Bluffs and federal officers, alleging constitutional violations arising from the mistaken arrest. In 2021, a federal judge granted summary judgment to the defendants.

Dillon Malone was involved in a serious police-pursuit crash on 04/25/2018 while responding to a pursuit involving people accused of firing at law enforcement in southwest Iowa. Malone crashed his patrol vehicle near Hamburg and was airlifted to Nebraska Medicine with serious injuries.

On 08/27/2025, 46-year-old Martin James Benes was shot and killed by Dallas Police Department SWAT officers Samuel McDonnold and Christopher Nilsen inside his family's home in the 13800 block of Far Hills Lane in Far North Dallas. The encounter began the previous evening after Benes' mother called 911 seeking help because her son was suicidal, had been drinking alcohol and had a knife. Benes' brother also contacted 911 expressing concern that Benes might harm himself or their mother.

Dallas police arrived at approximately 7:40 p.m. Body-camera video later released by the department shows Benes' mother answering the door and telling officers she needed “medical” help. Benes appeared farther inside the house and told officers to stay away from him. Officers drew their guns and moved outside while ordering him to put down the knife.

Benes briefly came outside while carrying the knife and then returned inside the residence. Police removed his mother from the immediate area and established a perimeter. The department called its SWAT team and attempted for several hours to persuade Benes to come outside.
.
Police also learned that Benes had an outstanding felony warrant related to an aggravated-assault-with-a-deadly-weapon family-violence case. The immediate police response, however, had begun with his family's calls reporting a suicidal crisis and requesting assistance.

After 1:20 a.m., SWAT officers entered the residence. Body-camera video shows officers moving upstairs and finding Benes lying on a bed, wrapped in a sheet and facing away from them. Officers saw a knife near him and quietly discussed attempting to use a Taser.

Officers illuminated Benes with flashlights and attempted to take him into custody. Police deployed Tasers twice, but neither deployment incapacitated him. Benes got up from the bed while holding the knife. Video shows him moving toward the officers and swinging his arms and the knife in their direction.

Senior Corporals Samuel McDonnold and Christopher Nilsen opened fire, striking Benes. After initially falling, Benes sat upright and moved toward the nearby banister as though attempting to stand. An officer fired again. Benes collapsed. A SWAT officer checked him and reported that he had no pulse before officers began medical treatment and called Dallas Fire-Rescue personnel into the residence. Benes died at the scene.

Benes' family strongly criticized the police response after viewing the released recordings. His brother, Paul Benes, said the family had repeatedly sought medical and mental-health assistance and accused police of escalating the crisis rather than providing the requested help. On 08/26/2025, at approximately 3:50 p.m., 35-year-old Shaunte Denise Canty of Washington, D.C., was driving east on Martin Luther King Jr. Highway in Landover with her young child when a stolen vehicle carrying four teenage boys ran a red light on Sheriff Road and struck the side of Canty’s vehicle. Canty died at the scene. Her child was seriously injured but survived.

The events leading to the fatal crash began when Seat Pleasant police officers spotted a vehicle that had been reported stolen on 08/22/2025 in the area of Seat Pleasant Drive and Joplin Street. Rather than immediately attempting a traffic stop, a Seat Pleasant officer began following the stolen vehicle from a distance while police waited for helicopter assistance from the Prince George’s County Police Department.

Seat Pleasant police said the officer was deliberately maintaining distance to avoid alerting the driver. An on-duty police commander subsequently ordered the officer to stop following the vehicle. Police said the officer complied with the order and turned back toward Seat Pleasant.

Shortly afterward, the stolen vehicle accelerated north on Sheriff Road. Its driver ran the red light at Martin Luther King Jr. Highway and crashed into Canty’s vehicle as she traveled through the intersection.

Three teenagers in the stolen vehicle were injured and hospitalized. A fourth was taken into custody at the scene. Police initially reported that the four boys were between 15 and 17 years old.

Prince George’s County police subsequently identified the driver as Michael Collins. Prosecutors charged Collins as an adult. In October 2025, a Prince George’s County grand jury indicted him on charges connected to the stolen vehicle and Canty’s death.

On 03/31/2026, Collins pleaded guilty to grossly negligent manslaughter by motor vehicle and auto theft. Prince George’s County prosecutors confirmed that Collins was driving the stolen vehicle when he ran the red light and struck the vehicle carrying Canty and her child.

The Prince George’s County Police Department Collision Analysis and Reconstruction Unit investigated the fatal collision. On 10/04/2016, at approximately 1:08 a.m., Statesboro police responded to Campus Evolution Villages at 1699 Statesboro Place Circle after receiving a report of an armed home invasion and robbery. Police alleged that three men had entered a residence and held people inside at gunpoint. Officers from the Statesboro Police Department, Bulloch County Sheriff’s Office and Georgia Southern University Police Department searched the apartment complex after receiving descriptions of the men.

Police located one man near the scene and arrested him. Officers then encountered 21-year-old Olajuwon James of Hinesville. Police attempted to use a Taser on James, but it did not incapacitate him. James ran from police and was later located behind another residence within the apartment complex.

During the second encounter, Statesboro Police Officer Adam Paquette reported that James had a firearm. Paquette fired two shots during the confrontation. James suffered a gunshot wound to the head and died at the scene. Initial police and GBI statements did not establish whose gunfire had caused the fatal wound.

A preliminary autopsy subsequently established that neither of Paquette’s two shots struck James. The Georgia Bureau of Investigation determined that James died from a self-inflicted gunshot wound to the head. GBI Special Agent-in-Charge Bahan Rich publicly confirmed the autopsy findings and James’s identity.
