Adam Michael Green, a 40-year-old White male, died on June 14, 2021, in Hermitage, Tennessee, following an encounter with Mt. Juliet Police Department officers. Officers responded to a 911 call reporting an "unruly guest" at a home. A woman claimed an unknown man had entered her home, was sweating profusely, destroying property, and not speaking clearly. On June 14, 2021, officers from the Parma Police Department attempted to stop a vehicle driven by 64-year-old Kevin Richard Giesel. Officers made contact with a man and a woman outside the vehicle. The man, identified as Louis Nathan Leyba, entered the driver’s seat while the woman entered the passenger seat. As Officer Derrick Valdez attempted to speak with the woman, Leyba began driving the vehicle in reverse. Officer Valdez became caught between the open door and the vehicle as it moved, and his feet dragged along the pavement. Officer Valdez ordered Leyba to stop. Leyba continued driving. Officer Valdez fired his weapon, and Officer Anthony Martinez also fired. Leyba was struck and died from his injuries. The shooting was placed under investigation. No officers were charged.

On June 7, 2021, around 11:45 a.m., a two-vehicle collision occurred near the intersection of East 220 Road and South 500 Road, northwest of Afton, Oklahoma. The crash involved a 2016 Ford Explorer driven by 48-year-old Douglas "Chad" Goodner, an off-duty reserve officer with the Collinsville Police Department, and a large farming vehicle. Goodner and his passenger, 59-year-old William McWhirt, were both pronounced dead at the scene due to massive injuries. Another passenger in the Explorer sustained serious injuries and was transported to a nearby hospital. The driver of the farming vehicle was not injured.

Officers responded to multiple 911 calls around 9:45 a.m. from the Home2 Suites by Hilton, where a man, Antonio Diaz, was reportedly pointing a handgun at people in the lobby and asking, "Do you want to die?" Police arrived at the hotel five minutes later and confronted Diaz, who was still armed in the lobby. At least one officer shot and killed Diaz.

Officers responded to a report of gunfire shortly before 2 a.m. inside a residence near the Homestead Air Reserve Base. A 38-year-old woman and a 15-year-old boy were found dead inside the home. Three other victims, ages 18, 16, and 11, were taken to the hospital. The shooter, the 42-year-old boyfriend of one of the victims, died by apparent suicide after shooting himself at a nearby location as officers tried to negotiate with him.

On June 7, 2021, at approximately 2:10 a.m., Deputy Casey Newton of the Happy Valley Police Department attempted to stop a vehicle driven by 44-year-old Jeremiah Lee Wright for operating a motor vehicle without license plates. Wright did not stop. Deputy Casey Newton initiated a high-speed chase up to 100 mph that ended when Wright crashed into a fence at the intersection of SE 122nd Avenue and SE Eagle Glenn Drive.

Wright ran away, and Deputy Newton chased him on foot until Wright stopped and turned toward the deputy. He was allegedly armed, and Deputy Newton opened fire, killing him.

A grand jury reviewed the incident and determined that Deputy Casey Newton's use of deadly force was justified and reasonable under Oregon law.
