Around 11:30 a.m., officers responded to a call about a man with a gun. Patrol officers found a woman with a fatal gunshot wound inside a neighbor's garage. Officers determined that a nearby home was involved. When they approached the house, officers heard what appeared to be a gunshot inside the residence. When they entered the house, they found the man dead of a self-inflicted gunshot.

On November 24, 2021, while following up on a tip, Nebraska State Patrol officers spotted a vehicle with German "Pee-Wee" Pedrazo and a woman inside. Rockwall police attempted to stop a white Chrysler 300 for the violation of a stolen license plate. The pursuit continued into the city of Fate, and Fate Police Department took over the pursuit. The vehicle traveled into the median of the interstate and lost control, striking a culvert. The driver of the Chrysler was killed, and the passenger was injured.

On November 23, 2021, at approximately 2:35 p.m., Pasadena Police Department officers were called to a report of a shooting at a residence where 70-year-old GevorkMinissian had been shot. Devin Edward Hall fled the scene on foot and allegedly attempted to carjack a woman by jumping into her vehicle. Hall was unable to steal the car and fled on foot. Police engaged him in a shootout.

Hall ran onto Sunnyslope Avenue, where he allegedly attempted to carjack a second woman but again failed to take the vehicle. He reportedly held the woman at gunpoint until he was cornered by officers. Police engaged him in a shootout and he was shot and killed.

Investigation:
The Los Angeles County Sheriff's Department is assisting the Pasadena Police Department in the investigation. The names of the involved officer(s) have not been publicly disclosed.

On November 23, 2021, at approximately 9:00 a.m., a Vermilion Parish Sheriff's Deputy conducted a traffic stop on a motorcycle near the intersection of LA-700 and Shelly Road in Kaplan, Louisiana, due to the rider not wearing a helmet. The motorcyclist, identified as 43-year-old Kelly Vaughn (also known as Kelly Griffin), did not provide identification and reportedly gave false information regarding his identity. Police alleged that he retrieved a handgun from his saddlebag; however, they admitted that he did not threaten them. Officers drew their firearms, and Vaughn fled on foot into a dense grassy field. Law enforcement established a perimeter, and the Vermilion Parish Sheriff's Office Special Response Team, along with Louisiana State Police, deployed aerial drones to locate him. Negotiators demanded his surrender, but Vaughn responded by firing shots at the drones and stated he would end his life rather than be taken to jail. At approximately 4:24 p.m., many hours after the standoff began, Vaughn died from a self-inflicted gunshot wound during the SWAT standoff. It is unknown whether he had committed any crimes, as police did not comment further.

On November 23, 2021, deputies from the Douglas County Sheriff's Office were involved in a pursuit of a vehicle in which John Schulze was a passenger. The driver, 31-year-old Blake Edward Thacker, was wanted on charges from Oregon. During the chase, Thacker's vehicle collided with another car, causing their vehicle to roll over. Schulze was ejected from the vehicle and died at the crash site.

On 11/22/2021, U.S. Marshals were attempting to serve an arrest warrant in the 700 block of N Sunset Avenue in West Covina, California. According to law enforcement, the individual ran from officers and allegedly produced a firearm. A West Covina police officer assigned to the U.S. Marshals Pacific Southwest Regional Fugitive Task Force opened fire, killing the individual.

On November 22, 2021, Warwick Police Department officers responded to a welfare check at the Lockwood Condominiums, located at 3524 West Shore Road in Warwick, Rhode Island. The call was initiated by a family member concerned about Frank J. Nappa Jr., who was reportedly experiencing a mental health crisis and armed with a loaded firearm. Nappa exited the condominium multiple times, each time holding a rifle and pointing it at the officers before retreating inside. Despite repeated commands to disarm, Nappa emerged a third time, advancing toward the officers with the rifle aimed in their direction. Officer Justin Martin open fire, striking Nappa. Immediate medical assistance was provided, and Nappa was transported to Rhode Island Hospital, where he was pronounced dead. Subsequent investigations by the Rhode Island Attorney General's Office and the Rhode Island State Police concluded that the use of lethal force was legally justified.

On November 21, 2021, at approximately 10:00 p.m., officers from the Louisiana State Police and the East Jefferson Levee District Police Department were directing traffic at the intersection of Airline Highway and Severn Avenue in Metairie, Louisiana, due to traffic signal repairs. Several concerned motorists alerted the officers to a man walking along Airline Highway near Causeway Boulevard, armed with a firearm and a knife. The individual was identified as 52-year-old Jabari Farafiai Asante-Chioke of LaPlace. He fled on foot when police demanded that he stop. A trooper fired a Taser. Asante-Chioke turned and pointed a gun. A state trooper and two East Jefferson Levee District officers all opened fire at the same time and killed him. He was pronounced dead on the highway.

On November 19, 2021, Greensboro Police Department officers responded to a 911 call reporting a wanted person at a residence on the 3500 block of Cloverdale Drive in Greensboro, North Carolina. A resident at the location informed police that 29-year-old Joseph Thomas Lee Lopez was attempting to force his way into the house. Officers located Lopez in a shed behind the residence. An officer was patrolling a park when he reportedly encountered Cody Rebischke, a homeless man living out of his vehicle. The officer, identified as James Aguirre, asked Rebischke to leave the area. An alleged altercation ensued between the two, during which Rebischke reportedly attempted to grab the officer's firearm. During the confrontation, Officer Aguirre shot and killed Rebischke.

In November 2021, 45-year-old Maurice Monk was found deceased in his cell at Santa Rita Jail in Alameda County, California. Monk, who suffered from schizoaffective disorder, had been arrested for a misdemeanor related to a dispute over mask-wearing on public transit during the COVID-19 pandemic. Unable to pay the $2,500 bail, he remained in custody.Reports indicate that Monk was left unattended in his cell for several days. Body camera footage revealed that deputies and medical staff delivered food and medication by tossing them into his cell without verifying his condition. When deputies eventually entered his cell, they discovered Monk unresponsive, surrounded by uneaten food trays and bodily fluids. The official cause of death was hypertensive cardiovascular disease. Monk's family subsequently filed a lawsuit, which resulted in a $7 million settlement. In November 2024, Alameda County District Attorney Pamela Price filed felony charges against 11 individuals connected to Monk's death, including seven current deputies, two former deputies, and two medical staff members. The charges encompass dependent adult abuse, with three defendants also accused of falsifying documents related to the incident. If convicted, each could face up to four years in state prison.

Maurice Monk, a 45-year-old Black man and security guard, died on November 15, 2021, while in custody at Santa Rita Jail in Dublin, California. He was incarcerated in early October after missing a court hearing related to a prior arrest. Monk suffered from diabetes and high blood pressure and was unable to receive his necessary medication despite his sister, Elvira Monk, making repeated attempts to provide it. Jail staff failed to process the prescription information. Two years after his death, body camera footage revealed that Monk had been left unattended for days before deputies discovered he had died. The Alameda County Sheriff’s Office is under scrutiny for the handling of his medical care.

On November 14, 2021, at approximately 9:35 p.m., Hollywood Police Department officers pursued 16-year-old Christopher Walls, who was possibly involved in a carjacking a black 2018 Alfa Romeo and fleeing in the vehicle. The chase ended when Walls crashed the car near Northwest 62nd Street and Northwest 27th Avenue in Miami, Florida. Witnesses to the arrest alleged that police used excessive force, stating that Walls was beaten during the encounter. After being handed over to Miami-Dade Police custody, Walls became unconscious and later died. Miami PD did not disclose the names of the officers who beat him to death.

On November 13, 2021, at approximately 3:09 p.m., 33-year-old barber Rafael Jeffers was fatally shot inside Bladi Style Barbershop at 5711 O'Donnell Street in Baltimore, Maryland. Carlos Ortega, 38, entered the barbershop and shot Jeffers while he was cutting the hair of off-duty Baltimore Police Sergeant David Burch. In response, Sergeant Burch retrieved his personal firearm and shot Ortega six times, killing him.

Earlier that day, Ortega was involved in two other shootings. At approximately 2:30 p.m., he was involved in an altercation on East Oliver Street, where he was stabbed and subsequently shot another individual who survived. Shortly after, Ortega fatally shot 44-year-old Javier Villegas Cotto outside Crown Barbershop on Eastern Avenue.

The Maryland Attorney General's Independent Investigations Division conducted a review of the barbershop shooting. In October 2022, the Baltimore City State's Attorney's Office declined to file charges against Sergeant Burch, determining that his actions were justified under the circumstances.

On November 13, 2021, around 2:30 p.m., Biloxi police responded to reports of Isaac Bell threatening individuals with a knife in a parking lot near Edgewater Mall. The situation escalated when Bell ran into an apartment complex courtyard, leading to a physical struggle with an officer. Both Bell and the officer sustained injuries during the confrontation and were hospitalized. Isaac Bell succumbed to his injuries a week later.

On 11/13/2021, Marcus Orlando Mann, 42, was outside the emergency room entrance of a hospital in Jackson, Tennessee, where he was reportedly being combative with hospital security. A Jackson Police officer who was already in the emergency room shot and killed Mann. He is believed to have been unarmed. The officer involved has not been publicly named.

On November 12, 2021, U.S. Marshals attempted to arrest a man who had an active warrant for a parole violation in connection with an assault and rape in California. The fugitive was also wanted for a murder in Los Angeles. As deputy marshals approached, the man allegedly opened fire on them, leading to a return of gunfire from the officers. The fugitive was shot and killed during the exchange.

On November 11, 2021, at approximately 4 p.m., NYPD officers pulled over 65-year-old Brian Astarita for allegedly speeding in a gray Jeep Cherokee near Bay 8th Street in Brooklyn, New York. As the officer approached the vehicle, Astarita fled, prompting a pursuit. During the chase, Astarita reportedly rammed the patrol car after it stopped in front of his Jeep.

Officers pursued Elliot Lewis in an hour-long, high-speed chase across the Houston area. He reportedly made phone calls to dispatch during the chase, which started near the Texas Medical Center/OST area about 11 a.m. Police cornered him in a parking lot after intentionally crashing into his vehicle. Lewis allegedly jumped out and started firing an AR-15 rifle at officers. Four officers shot and killed him.

Deputy Sean Speakman reportedly saw Jessiram Rivera in an obvious state of agitation while holding a shovel. The deputy ordered her to stop and put the shovel down and began backing away. Rivera did not comply, continuing to advance toward the deputy and threatening him with the shovel. Speakman then shot and killed Rivera.

Officers were investigating a stabbing that occurred about noon on Las Vegas Trail where the victim's gray truck was also stolen. At about 1:45 p.m., officers located the man suspected in the stabbing. Chassity Brooks crashed the stabbing victim's truck into the Taz game room, killing 45-year-old Jeffrey Mazurowski inside. Officers were nearby and began following Brooks. About a mile down the road, he allegedly crashed into a woman sitting at a bus stop, Lailani Snell, who was killed. Police arrested Brooks.

On November 8, 2021, at around 10: 45 a.m., a standoff began at 1523 Washington Avenue in Kalamazoo when police from Kalamazoo Public Safety and the Kalamazoo County Sheriff's Office attempted to arrest Alex Rawls, a 35-year-old male, on charges related to a shooting incident on November 1 where he allegedly shot his ex-girlfriend's sister five times. The standoff lasted approximately 16 hours, ending around 3 a.m. on November 9, 2021. During this time, Rawls fired multiple shots at law enforcement, although no officers were injured. Officers returned fire, and Rawls was struck by several bullets. After the standoff concluded, Rawls was found deceased on the second floor of the residence. An autopsy later confirmed that while Rawls was hit by police gunfire, the fatal shot was self-inflicted, though police provided no proof of this assertion at the time. The incident drew significant attention due to the length of the standoff, the use of 'directed destruction' tactics by police to view inside the home, and the subsequent demolition of the property. Rawls' family has questioned the circumstances of his death, with his sister stating that he never spoke of suicide and that the system had failed him since childhood. The investigation was handed over to Michigan State Police, who placed the involved officers on administrative leave pending review. The family's skepticism and the lack of immediate evidence for the self-inflicted wound have contributed to ongoing discussions regarding the transparency and handling of such incidents by law enforcement.

On 11/09/2021, the Brooklyn Park Police Department responded to a call for a stolen truck. Officers saw a man driving the truck. The truck drove through a ditch onto Highway 610, losing a construction trailer. It then dragged fencing for miles onto Highway 252. Police pursued the truck. At the intersection of MN-252 and 66th Avenue N, police escalated a chase which ended when he crashed Into a fence and a light pole and died.
The Minnesota Bureau of Criminal Apprehension (BCA) and Hennepin County Sheriff's Office are involved in the investigation.
