On December 7, 2021, at approximately 9:00 p.m., deputies from the Bernalillo County Sheriff's Office responded to a domestic violence call at the Las Mañanitas apartment complex near Coors and Montaño in Albuquerque. The victim had previously reported a domestic violence incident at an Allsup's gas station earlier in the day. Upon arrival, deputies arrested Mario Armando Diaz, 41, at the apartment complex. While being transported in a patrol vehicle, Diaz managed to slip one hand out of his handcuffs and attempted to flee. A brief foot pursuit ensued, leading to a physical altercation between Diaz and Deputy Taylor Feist. During the struggle, Deputy Feist called out that Diaz had her gun. In response, Deputy Dereck Gallegos fired two shots, fatally wounding Diaz. The incident was captured on body-worn cameras and a street camera.

An officer saw a van driving in reverse on Southeast 14th Street before it entered a parking lot. The officer stopped the van and questioned the driver, who gave a false name. The driver, identified as Donald Hill, ran from the scene on foot. The officer chased Hill, and they began struggling. Around 8 p.m., the officer radioed for help, reporting a violent encounter. As Hill ran into Southeast 14th Street, the officer followed. A vehicle struck both Hill and the officer. Hill died at the scene, and the officer suffered injuries.

On December 6, 2021, Jose Mario Velez, 47, of Lompoc died after a pursuit initiated by Vandenberg Space Force Base Security Forces. Officers attempted a traffic stop due to erratic driving, but Velez did not stop. He was traveling northbound on Highway 1 when he lost control near the "Y" interchange with Highway 135, struck a milepost, became airborne, and went off an embankment. His vehicle landed on its roof and slid into a guardrail. He was declared dead at the scene.

On December 6, 2021, at approximately 9:50 a.m., the Portland Police Bureau received reports of an armed man involved in carjackings along Interstate 5 near Rosa Parks Way. During one of the attempts, the man fired a shot at a person who was sent to the hospital but was later found not to be injured. Police opened fire and killed 30-year-old Brandon L. Keck.
The Portland Police Bureau's Detective Division is leading the internal investigation into the fatal shooting. Officer John Hughes, a 17-year veteran of the Portland Police Bureau, will be interviewed within 48 hours and will remain on paid administrative leave.

On December 5, 2021, Lawton Police Department officers responded to a 911 call reporting that Quadry Sanders was inside a residence in violation of a protective order and allegedly waving a firearm. Upon arrival, officers established a perimeter and attempted to communicate with Sanders. Sanders exited the residence and was ordered to show his hands. He initially complied but then moved behind a refrigerator. Officer Robert Hinkle fired four shots, striking Sanders. After falling, Sanders sat up with his hands raised, at which point Hinkle fired seven additional shots, and Officer Nathan Ronan fired four times. Sanders was unarmed at the time of the shooting and died from his injuries.

Michael Wayne Jackson was walking on a sidewalk in the Sunnyside neighborhood around 5:40 p.m. when Houston Police Department officers engaged in a high-speed chase of carjacking suspects. On 12/03/2021, a Ford Fusion reportedly carjacked in Cleveland was spotted traveling eastbound on Solon Road near Erico Drive. The driver accelerated, and police escalated a high-speed pursuit, reaching approximately 80 mph in a 35 mph zone. On November 18, 2021, 34-year-old Antonia Lee Crum was found deceased at the America's Best Value Inn in West Melbourne, Florida. Emmanuel Joe Ayala, 35, initially cooperated with investigators but later ceased communication. Authorities located Ayala in Palm Bay and obtained a search warrant for his residence. Upon arrival, Ayala armed himself and barricaded himself inside his home. After several hours of negotiation, Ayala died from an apparent self-inflicted gunshot wound. On November 28, 2021, at approximately 1:30 p.m., Anne Arundel County Police officers responded to a 911 call reporting that a male family member, armed with a knife, had chased a woman from a residence in the 1400 block of Braden Loop in Glen Burnie, Maryland.

Upon arrival, officers knocked on the door multiple times but received no response. They forcibly entered the home and encountered 32-year-old Digno "DJ" Ramon Yorro Jr., who was armed with a knife. Officers issued multiple commands for Yorro to drop the weapon, which he ignored. Non-lethal measures, including beanbag shotgun rounds and a Taser, were deployed but proved ineffective. Yorro then advanced towards the officers, still holding the knife, at which point Corporal Joseph Burger discharged his firearm, striking Yorro multiple times. Yorro was pronounced dead at the scene.

During a subsequent search of the residence, officers discovered the body of Yorro's mother in the garage; she had suffered multiple stab wounds. The Maryland Attorney General's Independent Investigations Division conducted a review of the incident. On November 28, 2021, at approximately 2:05 a.m., the Lakeland Police Department received a call about a suspicious vehicle parked in a construction zone on Interstate 4 near exit 31 at Kathleen Road. Officers Tammy Hathcock and Garrett Zeigler responded to the scene. Upon arrival, they observed a 61-year-old man sitting in the vehicle. As they approached and tapped on the window to speak with him, the man retrieved a handgun and pointed it toward Officer Hathcock. In response, both officers discharged their weapons, fatally wounding the man. He was pronounced deceased at the scene, and the handgun was recovered. No officers were injured. On November 28, 2021, at approximately 2:04 p.m., the BP Express gas station in Culver, Indiana, reported an intoxicated individual attempting to purchase alcohol. The individual, later identified as 33-year-old Dylan Wade Bush, departed the scene in a white Chevrolet Cavalier before officers arrived.

A Culver Police Department officer located Bush's vehicle near State Road 17 and Washington Street around 2:07 p.m. and initiated a pursuit when Bush failed to stop. The chase, involving high speeds exceeding 100 mph, continued through Marshall County for approximately 13.5 miles.

Sergeant Matthew Brown of the Marshall County Sheriff's Department joined the pursuit near State Road 10 and Sycamore Road. The pursuit concluded when officers boxed in Bush's vehicle at the First National Bank of Monterey's parking lot on North Lake Shore Drive. Sergeant Brown positioned his patrol car in front of Bush's vehicle and exited to issue commands. Bush ignored the commands and accelerated, pinning Sergeant Brown between the vehicles. In response, Sergeant Brown fired three shots from his service weapon, striking Bush.

After being shot, Bush's vehicle traveled several hundred feet before colliding with an unoccupied pickup truck in a nearby parking lot. Officers administered first aid until paramedics arrived, but Bush was pronounced dead at the scene by the Marshall County Coroner.

An autopsy revealed Bush had a blood alcohol content of .23%, nearly three times the legal limit, and the presence of cannabinoids and methanol in his system. The Marshall County Prosecutor's Office determined Sergeant Brown's use of deadly force was justified.

On November 28, 2021, Winston-Salem Police officers responded to a disturbance involving weapons at a residence on Bell West Drive in Kernersville, North Carolina. Residents reported that 33-year-old Adam Brent McKnight had assaulted a family member and posed a threat to others. Officers encountered McKnight at the back of the property near a pool house. McKnight emerged holding a machete and reportedly refused repeated commands to drop the weapon. Officers deployed Tasers in an attempt to subdue him. Despite this, McKnight continued advancing toward Officer R.T. Phillips. When McKnight got within a few feet, Officer Phillips discharged his firearm, fatally striking him. McKnight was pronounced dead at the scene.

On November 28, 2021, officers from the Madison Police Department responded to a call around 1:45 p.m. regarding a man, about 18 years old, threatening a murder-suicide. Family members were attempting to de-escalate the situation when police arrived. The man, armed with a weapon, retreated to the basement of the home. One officer fired his weapon but did not hit the man, who then proceeded to shoot and kill himself.

On 11/28/2021, officers from the Seekonk Police Department responded to a report of a break-in at a local business. They later located Jeffrey W. Groulx inside his car in a nearby parking lot. Upon spotting the officers, Groulx fled the scene, leading to a crash at the intersection of US-6 and School St. After the crash, Groulx allegedly opened fire on the officers with an AK-47 rifle. The officers returned fire, resulting in Groulx's death.

On 11/27/2021, at the Kalamazoo Transportation Center located at 459 N Burdick St, Anthony Oliver, a 54-year-old male, allegedly opened fire on a bus, shooting three passengers. The Kalamazoo Public Safety officers responded to the scene approximately two minutes after the initial 911 call was made. Upon arrival, they encountered an active shooter situation. Responding officers, engaged in a shootout with Oliver and Sean and killed him. The three others sustained non-life-threatening injuries. The shooting led to a temporary suspension of Metro bus services for the day. Investigations into the incident, including Oliver's motives and whether he knew any of the victims, were ongoing at the time. Oliver had a history of mental health issues, with his family indicating he was seeking help for these problems prior to the incident. The Kalamazoo County prosecutor later ruled that the officers' actions were in self-defense, and no criminal charges were filed against them. Video evidence from body cameras, bus cameras, and the transportation center's security cameras captured much of the incident. Oliver's family has spoken out about his struggles with mental health, and community reactions have called for increased security at the bus station and better mental health support.

On November 26, 2021, at approximately 2:00 p.m., deputies from the Bernalillo County Sheriff's Office responded to a report of a hit-and-run crash at the intersection of Edith Blvd NE and Montano Rd NW in North Valley, Albuquerque. Deputy Ronald Perez, a 10-year veteran of the department, was among the responding officers. The man, identified as 18-year-old Elijah Richie, fled the scene on foot after crashing his red SUV into a chain-link fence at Edith Blvd and Nikanda Road. Surveillance footage showed Richie attempting to flee the scene, armed with an AR-15 style pistol and carrying a backpack containing additional firearms. Deputy Perez pursued Richie and, after verbal commands to drop the weapon, fired a single shot, fatally wounding Richie. Richie was pronounced dead at a local hospital shortly after the incident. The shooting was captured on body-worn cameras and surveillance footage. The incident is under investigation by the Multi-Agency Task Force, a collaborative team that investigates officer-involved shootings.

On November 26, 2021, at approximately 2:30 p.m., a New Mexico State Police officer arrived at a Starbucks on Promenade Boulevard in Santa Fe and was informed by employees that the store had just been robbed by a man and a woman with firearms. They fled in a black Kia without a license plate. A pursuit ensued, with them traveling south on Richards Avenue, then north on Interstate 25, and subsequently south on U.S. 285. During the chase, the female passenger, identified as Christean "Christy" Ann Dimas, fired shots at officers. Officers from the New Mexico State Police, Santa Fe County Sheriff's Office, and Torrance County Sheriff's Office returned fire. The vehicle crashed near Clines Corners, and the driver, Jacob Montoya, was apprehended. Dimas was found deceased in the passenger seat. Montoya was airlifted to a nearby trauma center for gunshot wounds and later booked into the Torrance County Detention Facility on multiple charges, including assaulting a peace officer and shooting from a motor vehicle.

A motorcyclist was killed after officers pursued him across multiple Houston-area cities, reaching speeds in excess of 100 mph. The pursuit ended around 9:45 p.m. when the motorcyclist entered Dixie Farm Road, turned into oncoming traffic, and hit a curb, causing him to lose control of the motorcycle and crash, resulting in his death.

Around 11:30 a.m., officers responded to a call about a man with a gun. Patrol officers found a woman with a fatal gunshot wound inside a neighbor's garage. Officers determined that a nearby home was involved. When they approached the house, officers heard what appeared to be a gunshot inside the residence. When they entered the house, they found the man dead of a self-inflicted gunshot.

On November 24, 2021, while following up on a tip, Nebraska State Patrol officers spotted a vehicle with German "Pee-Wee" Pedrazo and a woman inside. Rockwall police attempted to stop a white Chrysler 300 for the violation of a stolen license plate. The pursuit continued into the city of Fate, and Fate Police Department took over the pursuit. The vehicle traveled into the median of the interstate and lost control, striking a culvert. The driver of the Chrysler was killed, and the passenger was injured.

On November 23, 2021, at approximately 2:35 p.m., Pasadena Police Department officers were called to a report of a shooting at a residence where 70-year-old GevorkMinissian had been shot. Devin Edward Hall fled the scene on foot and allegedly attempted to carjack a woman by jumping into her vehicle. Hall was unable to steal the car and fled on foot. Police engaged him in a shootout.

Hall ran onto Sunnyslope Avenue, where he allegedly attempted to carjack a second woman but again failed to take the vehicle. He reportedly held the woman at gunpoint until he was cornered by officers. Police engaged him in a shootout and he was shot and killed.

Investigation:
The Los Angeles County Sheriff's Department is assisting the Pasadena Police Department in the investigation. The names of the involved officer(s) have not been publicly disclosed.

On November 23, 2021, at approximately 9:00 a.m., a Vermilion Parish Sheriff's Deputy conducted a traffic stop on a motorcycle near the intersection of LA-700 and Shelly Road in Kaplan, Louisiana, due to the rider not wearing a helmet. The motorcyclist, identified as 43-year-old Kelly Vaughn (also known as Kelly Griffin), did not provide identification and reportedly gave false information regarding his identity. Police alleged that he retrieved a handgun from his saddlebag; however, they admitted that he did not threaten them. Officers drew their firearms, and Vaughn fled on foot into a dense grassy field. Law enforcement established a perimeter, and the Vermilion Parish Sheriff's Office Special Response Team, along with Louisiana State Police, deployed aerial drones to locate him. Negotiators demanded his surrender, but Vaughn responded by firing shots at the drones and stated he would end his life rather than be taken to jail. At approximately 4:24 p.m., many hours after the standoff began, Vaughn died from a self-inflicted gunshot wound during the SWAT standoff. It is unknown whether he had committed any crimes, as police did not comment further.

On November 23, 2021, deputies from the Douglas County Sheriff's Office were involved in a pursuit of a vehicle in which John Schulze was a passenger. The driver, 31-year-old Blake Edward Thacker, was wanted on charges from Oregon. During the chase, Thacker's vehicle collided with another car, causing their vehicle to roll over. Schulze was ejected from the vehicle and died at the crash site.

On 11/22/2021, U.S. Marshals were attempting to serve an arrest warrant in the 700 block of N Sunset Avenue in West Covina, California. According to law enforcement, the individual ran from officers and allegedly produced a firearm. A West Covina police officer assigned to the U.S. Marshals Pacific Southwest Regional Fugitive Task Force opened fire, killing the individual.
