On 08/16/2024, an unidentified male passenger was killed when a pickup truck crashed at Commerce Way and Atlantic Avenue in Woburn shortly after a Massachusetts State Police trooper attempted to stop the truck on Interstate 93 North. Two other people in the pickup were hospitalized. The driver of a sedan involved in the collision was not injured.

The police encounter began at approximately 9:30 p.m. when a Massachusetts State Police trooper patrolling I-93 North saw the pickup pass the cruiser at a high rate of speed. The trooper activated the cruiser's emergency lights and attempted to stop the pickup specifically because of the speeding.

The pickup continued toward Exit 30 and left I-93. State Police said the trooper terminated the pursuit as the pickup exited the highway. Shortly afterward, the pickup crashed into a sedan at Commerce Way and Atlantic Avenue.

The collision severely damaged both vehicles and knocked out electrical equipment near the intersection. One person traveling in the pickup died, while two other people from the pickup were conscious and alert when transported to hospitals.

The Mercury caught fire and became engulfed in flames. Dunbar and his two passengers, brothers Barry Fields Jr., 15, and Casey Fields, 13, died at the scene.

On 08/15/2024, at approximately 2:23 a.m., Morrisville police responded to a reported armed robbery and carjacking at the Camden Westwood Apartments on West Shore Court. The victim reported being assaulted by four individuals, one reportedly armed with a handgun, before his 2016 BMW SUV was taken. About eight minutes later, a Morrisville police officer located the BMW traveling on Chapel Hill Road toward Durham and initiated a pursuit.
The pursuit continued toward Durham before officers lost sight of the BMW near the Chapel Hill Road/Page Road area. Morrisville police terminated the pursuit. Approximately two hours later, at about 4:41 a.m., a passing motorist discovered the BMW crashed in a field off the 5900 block of South Miami Boulevard in Durham and notified authorities. Durham officers found the 16-year-old driver had been ejected from the vehicle and pronounced him dead at the scene. Investigators determined he was the only person inside the SUV when it crashed.
The crash investigation was conducted by the Durham Police Department while the Morrisville Police Department conducted an internal review of the pursuit. One Morrisville officer involved in the pursuit was placed on administrative leave pending that review. Morrisville Police Chief Pete Acosta later stated the review concluded departmental pursuit procedures had been followed. The juvenile's identity has not been publicly released because of his age. No subsequent public reports were located identifying the pursuing officer, announcing civil litigation, criminal charges, or other significant investigative findings related to the fatal pursuit.

On 08/15/2024, Virginia State Police attempted to stop a black 2024 Honda Passport on Interstate 95 in Fairfax County after a citizen recognized the vehicle from an AMBER Alert issued for three children abducted from a Virginia Beach home. The alert followed an alleged domestic violence incident in which police said 36-year-old Dana Demetrius Plummer stabbed the children's mother and a juvenile before leaving with his three children. Troopers confirmed the vehicle and initiated a traffic stop, but the driver continued driving away while police pursued.
The pursuit continued north across the Woodrow Wilson Bridge into Prince George's County, Maryland. Virginia State Police had been authorized to continue the pursuit across state lines because the case involved felony child abduction. As the SUV attempted to exit Interstate 495 at Branch Avenue, it struck other vehicles, lost control, and overturned. Four people were inside the Honda: Dana Plummer and his three children. Sixteen-month-old Za'riyah Plummer suffered critical injuries and was flown to Children's National Hospital in Washington, D.C., where she died. Her brothers, ages 5 and 7, sustained serious injuries but survived. Dana Plummer was also hospitalized following the crash.

On 08/14/2024, Van Alstyne police received reports of a reckless driver traveling north on U.S. Highway 75. Officers located a pickup truck driven by 37-year-old Delio Lomas of Dallas and attempted a traffic stop. Instead of stopping, Lomas continued driving away while police pursued.
The pursuit continued into the Howe area. According to police, Lomas drove through the grassy median while attempting to re-enter the highway, lost control of the stolen pickup truck, and crashed. He was ejected from the vehicle during the rollover. Emergency responders transported him to Texoma Medical Center in Sherman, where he later died from his injuries.

On 08/14/2024, Columbus Division of Police officers located 33-year-old Justin Randolph Moore in the South Linden neighborhood. Police reported that Moore was wanted on an outstanding felonious-assault warrant. Officers attempted to take him into custody, but Moore continued driving away while police pursued. On 08/13/2024, at approximately 10:19 a.m., New Jersey State Police Trooper Christopher Bird attempted to conduct a traffic stop of a Ford Explorer traveling east on Interstate 80 near milepost 11. Bird activated his emergency lights and siren, but the driver continued east at high speed, used the highway shoulder and exited at Exit 12 while Bird pursued. The Explorer then traveled south on County Road 521 through Hope Township. The publicly released investigation did not specify the traffic violation that prompted Bird to initiate the stop.
At approximately 10:20 a.m., the Explorer moved into the opposing lane to pass another vehicle near Dogwood Road. As it returned to the southbound lane, the Explorer struck a motorcycle operated by 76-year-old Michael J. Ceddia Jr. Ceddia was a bystander traveling on County Road 521 and was not involved in the reason for the traffic stop or pursuit. The impact threw him from the motorcycle. The Explorer then left the roadway and came to rest in a wooded area. Emergency personnel attempted to aid Ceddia, but he was pronounced dead at the scene.
The New Jersey Attorney General’s Office of Public Integrity and Accountability investigated the pursuit because Ceddia died during an encounter involving law enforcement. Investigators reviewed Bird’s body-worn camera, patrol-vehicle dashboard video, police radio transmissions, witness statements and Ceddia’s autopsy findings. The Attorney General’s Office later released video from the pursuit after first allowing Ceddia’s family to view the recordings.
The evidence was presented to a state grand jury, which voted on 05/20/2025 not to file criminal charges against Trooper Bird. The Attorney General’s Office announced that the grand jury found no basis for criminal prosecution arising from Bird’s initiation or continuation of the pursuit. The matter remained subject to any separate administrative review of Bird’s compliance with New Jersey State Police policy.

Near the intersection of Loop 1604 and North Graytown Road, the motorcycle entered the intersection against a red light and collided with a pickup truck driven by 40-year-old Michael Husky, who was not involved in the pursuit. Husky and the motorcyclist both died from injuries sustained in the collision. The motorcyclist was later identified as Larry Nees.
After the crash, China Grove police announced that the motorcycle Larry Nees had been operating had been reported stolen to the San Antonio Police Department on 07/09/2024. Police also stated that Nees was wanted on an outstanding Bexar County drug-trafficking warrant. Those facts were released after the collision and were not identified by police as the reason the pursuit was initiated.
A review of subsequent reporting through 2026 located no publicly reported civil lawsuit filed by Husky's family, no public disciplinary action involving the pursuing officers, and no publicly released independent prosecutorial findings regarding the pursuit.

On 02/10/2023, 28-year-old Delmy “Patty” Alvarez was killed when a Lexus being pursued by Orlando Police Officer Esequiel Colon crashed into the Honda Civic she was driving at South Parramore Avenue and West Anderson Street in Orlando. Alvarez was completely uninvolved in the police encounter. Her girlfriend, Gina Mustacchio, was riding in the passenger seat and survived with devastating injuries.

The pursuit began at approximately 10:18 p.m. when Colon saw a Lexus run a red light at Westmoreland Drive and South Street in the Parramore neighborhood. Colon later told an Orlando Police internal-affairs investigator that he considered running the red light reckless driving. This traffic violation—not a violent felony—was the specific initiating reason for Colon's decision to follow the vehicle.

Colon began following the Lexus in a marked Ford Explorer. As its driver increased speed, Colon continued behind him through residential city streets. Internal police records subsequently established that the vehicles reached speeds exceeding 60 mph.

Critically, Colon did not activate his emergency lights or siren during the pursuit. He later acknowledged this during the internal investigation. Consequently, Alvarez and other motorists approaching the eventual crash intersection received no emergency lights or siren warning that a high-speed police pursuit was entering the intersection.

The Lexus ran another red light at Parramore Avenue and West Anderson Street and struck the driver's side of Alvarez's Honda. Alvarez died from her injuries. Mustacchio suffered a broken jaw, multiple rib and pelvic fractures, a broken arm and injuries that ultimately cost her a kidney.

Orlando Police Department's own investigation determined that Colon's pursuit violated department policy. OPD policy permitted vehicle pursuits when an officer had reasonable suspicion that the fleeing person had committed or attempted a specified forcible felony. Running a red light did not qualify.

The internal investigation established that Colon himself acknowledged he did not have reasonable suspicion that the driver had committed or attempted a forcible felony and that the incident did not meet OPD's criteria for a vehicle pursuit.

The department nevertheless stopped short of attributing Alvarez's death to Colon. Colon received a 40-hour suspension and additional training for violating the pursuit policy. Under the police union agreement, he was permitted to surrender five days of paid leave instead of losing pay during the suspension.

No independent law-enforcement agency investigated Colon's role in Alvarez's death. The Orlando Police Department investigated both the fatal crash involving its own officer and Colon's policy violations. OPD later said there was no procedure requiring an outside investigation for a crash of this type.

The department's handling of the aftermath became a major part of the case. OPD did not issue a public news release explaining that Alvarez had been killed during a police pursuit and did not tell her mother, Marta Lopez, the full circumstances of her daughter's death. Lopez initially understood only that a reckless driver had run a red light and killed Delmy.

For months, OPD also denied an Orlando Sentinel public-records request by citing an active-investigation exemption even though the administrative investigation of Colon had already concluded. When records were eventually produced, substantial portions were redacted. Unredacted internal records became available through legal discovery in Mustacchio's civil litigation.

Lopez said she first began learning that police had been pursuing the Lexus through Mustacchio rather than through the department. When Lopez later confronted the crash investigator about the existence of a police pursuit, she recalled his response as asking who had told her.

The family has consequently been outspoken in challenging OPD's handling of Alvarez's death. Lopez said she believed the crash could have been prevented and explicitly assigned responsibility both to the pursued driver and to police. She criticized the department for failing to contact her or keep her informed.

Mustacchio likewise became an outspoken critic of the department. She argued that although the pursued driver was responsible for his actions, Colon's decision to chase him without emergency lights or siren materially contributed to the circumstances that killed Alvarez.

Mustacchio sued the City of Orlando, alleging that Colon's unauthorized pursuit caused or contributed to the collision. The lawsuit specifically cited Colon's violation of OPD's restrictive pursuit policy and his failure to use emergency lights and siren.

The City of Orlando settled the litigation in 2025 for $300,000. The settlement did not require the city to admit legal responsibility. Mustacchio said her purpose in bringing the case was accountability and exposing what she believed OPD had attempted to keep from public view.

The criminal investigation of the pursued driver was also marked by an extraordinary delay. Although police possessed evidence from the crash, charges were not submitted to the Orange-Osceola State Attorney's Office until September 2024, approximately 19 months after Alvarez was killed and only days after investigative reporting publicly exposed OPD's role in the pursuit.

Records showed that DNA evidence from the Lexus airbag and suspected marijuana recovered from the vehicle had been submitted to FDLE in April 2023. OPD did not report receiving the DNA result until September 2024. The suspected marijuana was not submitted for testing until September 11, 2024, the day after other charges were forwarded to prosecutors.

The driver was ultimately arrested in February 2025 and prosecuted for the fatal crash. At his 2025 sentencing proceedings, Mustacchio took the unusual position of speaking partly in his defense because she believed Colon and OPD also bore substantial responsibility. Alvarez's mother likewise told the court she believed there were two responsible parties: the driver and the pursuing police officer.

The case gained renewed significance because OPD subsequently experienced another fatal pursuit involving an officer violating the department's pursuit restrictions. In February 2025, Detective Christopher Moulton attempted to stop a driver over an unreadable license plate; the ensuing pursuit ended with 49-year-old Gerald Neal being killed. Prosecutors declined to charge Moulton.

In March 2026, previously unseen video obtained through records requests provided additional visual documentation of the Alvarez pursuit and crash. The footage showed the pursued Lexus running the red light and striking Alvarez's car while Colon had been following without emergency lights or siren.

OFFICER BACKGROUND
Officer Esequiel Colon was the Orlando police officer whose unauthorized pursuit culminated in the crash that killed Delmy Alvarez.

OPD's own internal investigation sustained the pursuit-policy violation. Colon acknowledged that he did not have reasonable suspicion of a qualifying forcible felony and that the circumstances did not meet department pursuit criteria. On 08/11/2024, deputies with the Lowndes County Sheriff's Office were responding to an active domestic violence call on Mississippi Highway 12 in Steens with emergency lights and sirens activated. According to Lowndes County Sheriff Eddie Hawkins, one deputy had already passed 85-year-old Sarah Elizabeth Jackson while responding to the emergency. A second deputy attempted to pass Jackson's vehicle when Jackson began making a left turn, and the deputy's patrol vehicle struck the driver's side of her vehicle. Both vehicles overturned, and the deputy's patrol vehicle came to rest in a wooded area off the highway.
Jackson and the deputy were extricated from their vehicles using hydraulic rescue equipment and transported to Baptist Memorial Hospital–Golden Triangle. Jackson died a short time later from her injuries. The deputy was treated for injuries and later released from the hospital.

On 08/10/2024, at approximately 4:11 a.m., 21-year-old Joshua Andrew VanderZiel was riding his motorcycle on Washington Boulevard (U.S. Route 1) in Howard County when Anne Arundel County Police Officer Alexander Rodriguez began pursuing him. State investigators determined Rodriguez initiated the pursuit without authorization, never notified dispatch, never activated his emergency lights or siren, and never obtained permission to continue the pursuit into Prince George's and Howard counties. The Attorney General's Office has not publicly identified the reason Rodriguez initiated the pursuit.
Investigators determined Rodriguez drove at speeds approaching 100 mph while following VanderZiel for more than four miles across three counties. VanderZiel's motorcycle collided with a flatbed truck that was making a left turn onto Washington Boulevard. He died at the scene from his injuries.
According to the Maryland Office of the Attorney General, Rodriguez stopped briefly after the collision, made a U-turn near the crash scene, and then drove away without rendering aid, notifying dispatch, or reporting the fatal crash. Three civilian witnesses, including the occupants of the truck, called 911. Rodriguez returned to Anne Arundel County, completed the remainder of his shift, and did not prepare a report documenting the pursuit or collision.
An eight-month investigation by the Maryland Attorney General's Independent Investigations Division used GPS data, patrol vehicle records, witness statements, and dashboard camera footage from the flatbed truck to identify Rodriguez as the pursuing officer. On 04/02/2025, a grand jury indicted Rodriguez on charges including manslaughter by vehicle (gross negligence), manslaughter by vehicle (criminal negligence), leaving the scene of a fatal crash, and five counts of misconduct in office.
On 04/10/2026, Rodriguez pleaded guilty to the felony charge of leaving the scene of an accident resulting in death. The remaining charges were dismissed as part of the plea agreement. He resigned from the Anne Arundel County Police Department. In July 2026, VanderZiel's family filed a civil lawsuit against Rodriguez and Anne Arundel County alleging the unauthorized pursuit and subsequent cover-up caused Joshua's death. Joshua VanderZiel had served as a U.S. Navy Russian linguist and had recently completed training before his death.
Officer Rodriguez received a five-year sentence with all but 120 days suspended, followed by two years of supervised probation, 100 hours of community service, and agreed never again to seek a law-enforcement career.
Resigned/ended career. He is a former Anne Arundel County police officer and is barred by the plea agreement from returning to law enforcement in Maryland.

On 08/09/2024, an unidentified woman was killed when a Chevrolet Tahoe carrying her and other migrants crashed and rolled near Arivaca while U.S. Border Patrol agents pursued the vehicle. The encounter began when Border Patrol personnel received information about a gold Chevrolet Tahoe traveling south on Ruby Road that was suspected of being used to transport migrants. The pursuit ended almost immediately when Lopez-Ozuna lost control of the Tahoe. The vehicle left the roadway and rolled, ejecting all seven passengers. One woman was killed and three other passengers suffered serious injuries. Lopez-Ozuna was also injured.

A federal grand jury indicted him in September 2024 on human-smuggling charges resulting in death and serious injury. On 08/08/2024, a Washington State Patrol trooper attempted to stop a Jeep Grand Cherokee on Boeing Access Road. The publicly released reports did not disclose the reason for the attempted traffic stop. The driver continued driving away while troopers pursued southbound on Airport Way South. On 08/07/2024, at approximately 9:50 p.m., a Town of Deerpark police officer attempted to stop 31-year-old Wade Christopher Conklin Jr. while he was riding a motorcycle on U.S. Route 209 near the Port Jervis city boundary.
Conklin continued riding away at high speed while Deerpark police pursued him toward Port Jervis. He entered the city on Kingston Avenue and continued onto East Main Street. Port Jervis police joined the pursuit after the motorcycle entered their jurisdiction.
Conklin traveled west on East Main Street, passed through two red traffic signals and entered the intersection of East Main Street and Orange Street, where the motorcycle collided with an SUV. The collision ejected Conklin from the motorcycle and threw him into a light pole in the center median.
Emergency personnel transported Conklin to Bon Secours Community Hospital, where he was pronounced dead from blunt-force trauma.
Conklin was originally from Middletown and was a member of the H.O.O.D. Ryderz Motorcycle Club in Port Jervis. His obituary described him as a former Middletown High School varsity diver, football player and lacrosse player who was devoted to his family and protective of his friends.
The New York State Police investigated the crash. The New York Attorney General’s Office of Special Investigation opened a review because Conklin died during a police pursuit involving the Town of Deerpark Police Department and Port Jervis Police Department. The Orange County District Attorney’s Office and Orange County Medical Examiner’s Office also participated in the post-crash investigation.

n 08/07/2024, Bernalillo County Sheriff's Office Deputy Bryan Lassley reported that he attempted to stop a silver Honda he considered suspicious while driving north on Edith Boulevard NE shortly after midnight. Lassley reported over the radio that he lost sight of the vehicle and moments later stated the Honda had stopped abruptly.
Deputy Lassley struck the rear of Alexandria Gerard's Honda with his marked Ford Explorer. The impact sent Gerard's vehicle into a stone wall at the intersection of Edith Boulevard NE and Alameda Boulevard NE. Gerard was trapped behind the steering wheel and died at the scene. Deputy Lassley was not injured.
Bernalillo County Sheriff John Allen requested that the Albuquerque Police Department conduct the criminal traffic investigation to avoid a conflict of interest. Deputy Lassley was placed on paid administrative leave while the investigation proceeded.
The Albuquerque Police Department's crash investigation later concluded that data downloaded from Lassley's patrol vehicle showed he accelerated to approximately 70 mph in a 25 mph zone seconds before the collision. The report concluded Lassley slowed briefly, then accelerated again immediately before striking Gerard's vehicle. Investigators also determined Gerard's Honda showed rear-end impact damage consistent with the Sheriff's Office push bumper.
Investigators documented that Gerard was not wearing a seat belt and that the driver's airbag in her Honda had previously been removed. The autopsy found fatal chest injuries consistent with the collision.
Body-worn camera video, lapel camera footage, surveillance video, radio traffic, and investigative reports were later released through New Mexico's Inspection of Public Records Act. The public release showed Deputy Lassley attempting to free Gerard immediately after the crash while additional deputies and Albuquerque Police personnel responded.After the fatal crash that killed Alexandria Gerard, Deputy Bryan Lassley was placed on paid administrative leave while the Albuquerque Police Department conducted an outside investigation. Investigators concluded that Lassley accelerated to approximately 70 mph in a 25 mph zone, followed Gerard's vehicle too closely, and did not have his emergency lights activated immediately before the collision. The case was referred to an outside prosecutor because of a conflict of interest, but no criminal charges were filed and Lassley remained employed by the Bernalillo County Sheriff's Office.
Lassley later became the subject of a separate internal and criminal investigation arising from a December 2025 incident involving a handcuffed juvenile girl outside the Bernalillo County Youth Services Center. Body-camera video showed Lassley pushing the girl and investigators alleged he slammed her head against a patrol vehicle. Sheriff John Allen announced that Lassley would be terminated, describing the conduct as unacceptable. A grand jury later indicted Lassley on felony aggravated battery and child abuse charges. Approximately two minutes after the pursuit began, Braman lost control of the vehicle, crashed into a utility pole, and the vehicle rolled over near the intersection of Northeast 122nd Avenue and Northeast Marx Street. Braman died at the scene. Passenger Nicholas P. McGuire also died, and a third occupant was transported to a hospital with serious injuries. The Portland Police Bureau Major Crash Team investigated the collision. Publicly available records have not identified the pursuing officers or announced criminal charges or disciplinary action related to the pursuit.

On 08/02/2024 at approximately 2:00 a.m., Memphis Police Officer Demetrice Johnson and Officer Bobby Fields were traveling in a marked patrol vehicle while responding to an emergency call with emergency lights and siren activated. They were driving southbound on Danny Thomas Boulevard in downtown Memphis.
At the same time, 22-year-old Rantavious Johnson was driving northbound on Danny Thomas Boulevard. According to the Tennessee Bureau of Investigation and Memphis police, Johnson's vehicle crossed the center line into the southbound lanes and struck the police cruiser head-on near the intersection with North Dunlap Street. The impact caused catastrophic damage to both vehicles.
Emergency crews removed all three men from the wreckage and transported them to Regional One Health. Officer Demetrice Johnson and Rantavious Johnson both died from injuries sustained in the collision. Officer Bobby Fields survived with critical injuries and underwent extensive medical treatment.
The Tennessee Bureau of Investigation assumed responsibility for investigating the crash at the request of Shelby County District Attorney General Steve Mulroy because it involved a Memphis police vehicle. Investigators processed the crash scene, examined the vehicles and collected witness statements and physical evidence.
Memphis Police identified Officer Demetrice Johnson as a 34-year-old officer who had joined the department in May 2023. Johnson was responding to a priority call at the time of the collision.

On 08/01/2024 at approximately 5:11 p.m., Utah Highway Patrol Trooper Withers was conducting speed enforcement on Interstate 70 when a white BMW SUV passed his location traveling approximately 128 mph. Trooper Withers entered the interstate and attempted to catch up to the vehicle but was unable to close the increasing distance. He radioed ahead to the Emery County Sheriff's Office for assistance.
An Emery County deputy located the BMW and attempted a traffic stop. The BMW continued driving east on Interstate 70 while law enforcement pursued. During the pursuit, the driver encountered slower traffic and drove into the highway median to pass other vehicles at high speed. The pursuit covered less than 20 miles.
Near mile marker 160, the BMW left the roadway, rolled multiple times and ejected both occupants. Driver Matthew Koehler died at the scene from injuries sustained in the rollover. Passenger Jasmine Green also died at the scene.
The Utah Highway Patrol investigated the collision with assistance from the Emery County Sheriff's Office. Authorities identified the decedents on August 5, 2024, as Matthew Koehler, 26, of Las Vegas, Nevada, and Jasmine Green, 34, of Henderson, Nevada.

On 07/31/2024, officers with the West Palm Beach Police Department pursued a vehicle driven by Neoni Copeland through Palm Beach County. During the pursuit, Copeland drove into the intersection of North Congress Avenue and Meadows Boulevard in Boynton Beach and crashed into the vehicle carrying 27-year-old Jenice Woods and her mother, Marcia Pochette. Woods, who was approximately two months pregnant, her unborn child, and Pochette all died from their injuries.
The Palm Beach County State Attorney's Office Public Corruption Unit investigated the pursuit. In June 2025, seven West Palm Beach police officers were criminally charged following the investigation. Officers Austin B. Danielovich, Pierre F. Etienne, and Christopher C. Rekdahl were each charged with two counts of leaving the scene of a crash involving death and one count of official misconduct. Officers Michael E. Borgen, William L. Loayza, Brandan C. Stedfelt, and Darien J. Thomas were each charged with one count of official misconduct. Prosecutors alleged the pursuit violated department policy and that officers failed to stop and render aid after the fatal collision. All seven officers were subsequently terminated by the department while the criminal cases proceeded. Police were reluctant to answer questions, but the chase was reportedly done for someone having too dark window tint.
Woods' family publicly questioned why the pursuit had been initiated and called for accountability, saying they were still seeking answers about the decisions that led to the crash.
