REASON FOR POLICE DRIVING: Off duty — drunk driving

NARRATIVE:
On 05/01/2021 at approximately 2:00 a.m., Alfredo Escaname-Hernandez, 50, was traveling south on Interstate 25 with Diego Arellano Robles, 21, and Diego's brother Fernando Arellano, 21. The three Denver residents were driving to Juárez, Mexico, to visit family. Alfredo was making the trip in part to help relatives build a house.

Off-duty Cuba Police Department Officer Brandon Barber, 29, was driving a Ford F-150 north in the southbound lanes of I-25 when he crashed head-on into their GMC Yukon near Tramway Boulevard. Alfredo and Diego died at the scene. Fernando survived with serious injuries, including a brain bleed; prosecutors later said his injuries would affect him for the rest of his life.

Bernalillo County sheriff's deputies reported that Barber smelled of alcohol, had bloodshot and watery eyes, and had a half-consumed bottle of liquor in his truck. He was arrested and initially charged with vehicular homicide, aggravated DWI and an open-container violation.

Alfredo's family publicly demanded stronger consequences after a judge released Barber before trial under supervision, prohibited him from driving and required alcohol monitoring. Alfredo's son Federico called the decision insulting, saying his father could not come home to his family while Barber was allowed to return to his parents. Family members described Alfredo as a man of many trades who was widely regarded as a father figure.

Despite that prior DWI, Barber subsequently became a Cuba police officer. He was still a probationary officer when he killed Alfredo and Diego and was terminated after the crash.

Diego's brother later remembered him as a funny young man who loved cars, video games and car meets and was just beginning to figure out what he wanted to do with his life.

REASON FOR CHASE: Traffic violation not disclosed by police

NARRATIVE:
On 05/01/2021 at approximately 10:50 p.m., Jose Ascencion Astrain Jr., 25, was driving a silver 2008 Ford Fusion north on West Avenue when San Antonio police officers in an unmarked vehicle attempted to stop him for a traffic violation that police did not publicly specify. Police said Astrain continued driving at high speed after officers activated their emergency equipment.

SAPD said the officers then turned off their emergency lights and ended the attempted pursuit. Astrain continued north on West Avenue, lost control in the 3800 block and fishtailed across the roadway into opposing traffic. His Ford Fusion collided with a brown pickup truck.

Astrain suffered serious injuries and was transported by ambulance to University Hospital, where he died. Police said he was not wearing a seat belt. The pickup driver remained at the scene, cooperated with investigators and was not charged. No serious injury to the pickup driver was reported.

Astrain was born in San Antonio on 07/25/1995. He was the father of daughters Dalilah Rose Astrain and Aubreanna Nichole Astrain and stepfather to Cathrina. He was also survived by his parents, Jose Astrain Sr. and Diana Astrain, and his brother Joe Anthony Astrain.

REASON FOR CHASE: by standard killed during stolen vehicle chase

NARRATIVE:
On 04/29/2021 at approximately 11:00 p.m., Emmon Woods, 20, was driving a Chevrolet Malibu near 15 Mile Road and Van Dyke Avenue in Sterling Heights when a stolen Jeep Grand Cherokee traveling south on Van Dyke entered the intersection at extreme speed and struck her vehicle. Woods was killed instantly.

The incident began after four people broke into Jim Riehl’s Friendly Chrysler-Dodge-Jeep-Ram in Washington Township and stole four vehicles. A Macomb County sheriff’s deputy encountered a stolen Jeep driven by 17-year-old Angelo Rickey Smith and pursued it from the dealership area. Authorities said the Jeep was traveling at extremely high speed, and the deputy terminated the pursuit near 26 Mile and Van Dyke because of the danger. No other police agency continued the pursuit.

Smith continued south on Van Dyke after police terminated the pursuit. Later prosecution records established that he reached speeds exceeding 130 mph. Police said he repeatedly ran red lights before entering the intersection at 15 Mile against the signal and striking Woods' Malibu. The collision propelled the vehicles into a Hyundai Sonata and destroyed a streetlight and other roadside property. A witness described the impact as sounding like a bomb.

Woods died at the scene. The driver and passenger of the Hyundai suffered minor injuries. Smith was also injured and hospitalized under police custody. A passenger in the stolen Jeep fled the crash scene on foot.

Smith was charged with felony murder, second-degree murder and fleeing-and-eluding offenses. The case remained pending for more than four years. A second man, Taiyon Jennings, who had been a passenger in the stolen Jeep, was also initially charged with murder, but a judge dismissed those homicide charges after finding insufficient evidence that Jennings intended or encouraged the fatal conduct. Jennings later pleaded guilty to breaking and entering and larceny and received 43 months to 10 years in prison.

On 07/24/2025, Smith pleaded no contest to second-degree homicide, first-degree fleeing and eluding, breaking and entering with intent, and larceny in a building. On 09/04/2025, Macomb County Circuit Judge Michael Servitto sentenced him to 15 to 50 years for second-degree murder, 86 months to 15 years for fleeing and eluding, 38 months to 10 years for breaking and entering, and one to four years for larceny, with the sentences running concurrently. He received credit for 1,585 days already served.

REASON FOR CHASE: Speeding

NARRATIVE:
On 04/29/2021 at approximately 9:00 p.m., Everette Orette Brown, 25, was driving a black 2011 Mercedes-Benz when a Miramar police officer conducting speed enforcement on Hallandale Beach Boulevard attempted to stop him for speeding. Brown continued driving and police followed.

Miramar Police later emphasized that officers initially were not conducting a full high-speed pursuit. Department spokesperson Tania Rues said officers were approximately 20 seconds behind Brown when his Mercedes struck a pedestrian on Southwest 64th Avenue. After the pedestrian was hit, officers accelerated in an effort to stop Brown before another person was struck.

Brown continued east at speeds police estimated at approximately 100 mph. In the 4500 block of Hallandale Beach Boulevard in Pembroke Park, he attempted to maneuver around traffic, lost control and struck multiple light poles and a fire hydrant. The Mercedes overturned and Brown was ejected. He died at the scene.

The pedestrian Brown struck survived and was hospitalized with injuries described as non-life-threatening. No other fatality was reported.

The case became part of a broader South Florida examination of police pursuit policies. REASON FOR CHASE: Struck park entrance barriers and fled

NARRATIVE:
On 04/28/2021 at approximately 7:43 p.m., Robert A. Sanders, 48, of Lakewood, New Jersey, was driving a 2008 Toyota Tundra at Saratoga National Historical Park in Stillwater when National Park Service rangers responded to a suspicious-vehicle complaint. Authorities said Sanders struck barriers at the battlefield entrance and drove away as rangers followed him.

Sanders continued north on Route 32 and the park rangers lost sight of him. Stillwater police subsequently located the Tundra traveling south on Route 4 and initiated another pursuit. Sanders continued through the village of Stillwater and the city of Mechanicville before heading west on Route 67.
.
The pursuit continued toward Malta. At the roundabout connecting Routes 67 and 9, Sanders struck road signs and continued south on Route 9. Near Goldfoot Road, the Tundra crossed into the northbound side of the highway and struck a guardrail. Sanders was found dead inside the truck. No other deaths or injuries were reported.

Because Sanders died during a police pursuit, Saratoga County Sheriff Michael Zurlo notified the New York Attorney General's Office, which had authority to investigate deaths involving police. Contemporary reporting said the Attorney General had been contacted but had not announced whether it would take over the investigation.

Sanders lived in Lakewood, New Jersey, and his Toyota was described by police as carrying a large brown toolbox in the bed, a detail used in the multi-agency effort to locate him after park rangers lost sight of the truck.

On April 28, 2021, officers from the California Highway Patrol initiated a high-speed pursuit of a blue Dodge pickup truck after observing it speeding and running a red light on Century Boulevard near the 405 Freeway. The pursuit continued onto the 405 Freeway, then exited onto Manchester Avenue, with the driver racing through the streets of Inglewood. At the intersection of Glasgow Avenue and Olive Street, the pursued vehicle ran a stop sign and collided with a white sedan occupied by Ryan Davis and his partner, Asia Boatwright. The impact caused severe damage, trapping both victims in the wreckage. Davis was transported to a local hospital, where he was pronounced dead. The driver of the pickup truck fled on foot but was apprehended shortly thereafter and identified as 42-year-old Gustavo Ruelas. He was booked on multiple felony charges, including driving under the influence of a drug.

REASON FOR CHASE: Bystander killed during red-light violation pursuit

NARRATIVE:
On 04/28/2021 at approximately 10:35 p.m., Asia Marie Boatwright, 22, was riding with her boyfriend, Ryan Davis, 27, as they traveled through Inglewood. Boatwright was a phlebotomist pursuing a nursing career. She had attended college courses while still in high school and, unknown to her when she died, had been accepted into the nursing program at Los Angeles Valley College.

California Highway Patrol officers attempted to stop a blue Dodge pickup after its driver, Gustavo Ruelas Jr., allegedly sped through a red light on Century Boulevard near the 405 Freeway. Ruelas entered the northbound 405, exited at Manchester Avenue and continued through Inglewood while CHP pursued. The pursuit lasted approximately one minute and reached roughly 90 mph.

At Glasgow Avenue and Olive Street, Ruelas ran a stop sign and struck Davis and Boatwright's sedan at high speed, forcing it onto the sidewalk. The pickup then struck a parked vehicle. The sedan was so badly crushed that firefighters had to extricate Boatwright and Davis. Boatwright died at the scene; Davis was transported to a hospital and died from his injuries. Ruelas suffered minor injuries, ran from the crash and was captured after a short foot pursuit.

Boatwright's and Davis' families immediately challenged the decision to conduct the pursuit. At a vigil two days after the crash, Davis' mother, Aziza King, and Boatwright's relatives called for an end to dangerous police chases. Boatwright's grandmother, Maria Dones, emphasized that the couple had simply been traveling home when they were killed.

Ruelas was charged with two counts of murder and other offenses. On 04/20/2022, he pleaded no contest to murder, gross vehicular manslaughter while intoxicated and felony evading while driving recklessly and admitted leaving the scene. He was sentenced to 26 years to life in state prison.

In May 2022, Boatwright's mother, Melissa Gonzalez, and Davis' mother filed a wrongful-death lawsuit against the State of California, CHP and Ruelas. The lawsuit alleged CHP negligently continued the high-speed pursuit, failed to obtain air support in time, failed to terminate the pursuit after losing radio contact, failed to adequately warn other agencies and failed to promptly summon medical assistance after the crash.

CHP later sought dismissal of the lawsuit, disputing liability and arguing that the mothers lacked legal standing under California wrongful-death law.

Boatwright's and Davis' families continued advocating for safer streets after their deaths. In October 2023, relatives and community members installed a ghost-tire memorial near the crash site. Their families became involved with road-safety advocacy, using Asia and Ryan's deaths to draw attention to speeding, dangerous driving and fatal police pursuits.

REASON FOR POLICE DRIVING: Pedestrian killed by speeding police vehicle

NARRATIVE:
On 04/28/2021 at approximately 8:30 p.m., Lyndon Fitzgerald Williams, 53, was crossing the four-lane Dr. Martin Luther King Jr. Boulevard near Caswell Street when Kinston Police Officer Treyston Sean Johnson struck him with a marked police cruiser. Williams, who was homeless, was well known to people in the neighborhood and was frequently seen walking in the area. He died at the scene.

Kinston police initially said Johnson may have been responding to a call but could not confirm whether he was actively responding when he struck Williams. Johnson suffered non-life-threatening injuries and was placed on administrative leave while the North Carolina State Highway Patrol investigated.

Witness Darius Howell disputed the later official account of Johnson's speed. Howell said the cruiser passed him immediately before striking Williams and estimated it was traveling 55 to 60 mph. The Highway Patrol's completed crash report instead estimated Johnson's speed at approximately 35 mph. Investigators reported that Williams was crossing mid-block, was wearing dark clothing and that a streetlight in the area was not working.

Williams' family publicly questioned the circumstances of his death, while community members remembered him as a familiar presence in the area. The Highway Patrol ultimately did not charge Johnson. Kinston police returned him to full duty after the investigation, with the city's interim police chief later stating that Williams was considered at fault because he was not using a crosswalk.

Treyston Sean Johnson remained a Kinston police officer after the fatal collision and was still employed by the city in 2022. In July 2024, he was hired and sworn in as a police officer in Muncie, Indiana. His killing of Williams resurfaced publicly after the hiring; Muncie Police Chief Nathan Sloan said Johnson disclosed the fatal collision during his hiring interview and defended his qualifications to serve.

REASON FOR CHASE: Bystander killed during felony-warrant pursuit

NARRATIVE:
On 04/28/2021 at approximately 8:20 p.m., 5-year-old Sariah Marie Sheppards was riding in a 2003 Honda Accord driven by her mother, La'Shae Northington, when a fleeing 2007 Scion ran a red light at East 9th and Sutro streets and crashed into their car. Sariah suffered fatal injuries and died several hours later at Renown Regional Medical Center. Her mother survived.

Police had attempted to stop 33-year-old Jesse Allen Bennett of Eugene, Oregon, because he had felony warrants from Oregon involving domestic violence and attempted kidnapping. Both Sparks and Reno police were involved in efforts to stop Bennett before the crash. Police later maintained that officers were no longer actively pursuing the Scion at the moment it struck Sariah's mother's car.

Bennett ran the red light and struck the Honda. He and his passenger abandoned the Scion and fled on foot. Bennett was captured and arrested; authorities reported charges including DUI and possession of drugs for sale in addition to charges arising from the fatal crash.

Two days later, Sariah's mother joined dozens of family members, friends and community members at a vigil at the crash site. Balloons, flowers and candles covered the intersection. A motorcyclist who witnessed the crash followed Bennett after he fled and provided police information that helped locate him; Sariah's family publicly thanked him. Family remembered Sariah as an exceptionally bright, affectionate child whose smile drew people to her.

Sariah's mother subsequently sued Bennett and the cities of Reno and Sparks. The lawsuit alleged that police negligence contributed to Sariah's death and challenged the agencies' handling of the attempt to apprehend Bennett. The case reached the Nevada Supreme Court in 2022 after the district court dismissed the claim against Sparks. The Supreme Court dismissed Northington's appeal on procedural grounds because claims against Bennett and Reno were still pending, meaning there was not yet a final appealable judgment resolving the entire case.

Sariah's death also became part of Nevada advocacy over police pursuits. In testimony submitted to the Nevada Legislature in 2023, her death was specifically identified as an innocent-motorist pursuit death involving Sparks and Reno police.

On 04/27/2021 at approximately 3:20 p.m., Harris County sheriff’s deputies pursued a male driver in a stolen vehicle in northeast Houston. The pursuit traveled onto North McCarty Street. Contemporary reports did not disclose where deputies first encountered the vehicle or the circumstances through which police determined it was stolen.

In the 4500 block of North McCarty, the driver lost control and crashed into a utility pole. The vehicle erupted in flames and the driver died at the scene. He was the only person in the vehicle, and no additional deaths or injuries were reported.

REASON FOR CHASE: Passenger killed during 111-mph speeding pursuit

NARRATIVE:
On 04/25/2021 at approximately 1:38 a.m., Pedro Carbajal, 22, was riding in a Hyundai Elantra with his cousin Jennifer Carvajal, 24, his girlfriend Lexcia “Lexi” Gonzalez, 20, and Grady Ramirez, 19, after the group attended a family birthday party. Pedro and Gonzalez had a young son, Julian.

Florida Highway Patrol Sgt. Jason Moore was driving east on Interstate 4 when his radar clocked the westbound Hyundai repeatedly at 111 mph. Moore made a U-turn, activated his emergency lights and siren and accelerated after the Hyundai. About a minute later, near Mango Road, he caught up with the car.

As Moore approached, Carvajal abruptly turned right onto the shoulder, traveled down a grassy slope and up an embankment, and became airborne. The Hyundai clipped a wire fence, flew into the Gator Ford dealership lot, struck a parked Ford F-250, overturned, and crashed into a concrete light pole and palm tree.

Pedro and Gonzalez were ejected. Pedro suffered critical injuries and later died. Gonzalez suffered two broken legs. Ramirez suffered incapacitating injuries after being thrown from the front seat into the rear of the vehicle. Carvajal, the only person wearing a seat belt, survived.

At Tampa General Hospital, Trooper Joshua Lugo reported that Carvajal had slurred speech, glassy eyes, difficulty remaining awake and smelled of alcohol. A hospital blood test measured her blood-alcohol concentration at approximately .10. Investigators also used seat-belt bruising and witness information in identifying her as the driver after she denied driving.

Carvajal was permanently barred from driving and was still on probation for a 2014 DUI manslaughter conviction in which, at age 16, she drove drunk through a red light and killed 52-year-old Keith Allen Davis. She had served approximately four years of a five-year prison sentence and was released in 2019.

Pedro's mother, Jazmin Dominguez, attended Carvajal's detention proceedings. Prosecutors told the court that Carvajal had attempted to contact Pedro's family after the crash, and the judge ordered her to have no further contact with the victims or their families. Carvajal was held without bond.

The criminal case continued for nearly four years. In February 2025, Carvajal pleaded guilty to DUI manslaughter, vehicular homicide, three counts of DUI causing serious bodily injury, reckless driving causing serious bodily injury, driving with a suspended or revoked license, a second DUI offense and two counts of DUI causing property damage or personal injury.

On 04/23/2021 at approximately 5:45 a.m., an unidentified male was driving a pickup truck north on South Shadeland Avenue with one passenger. In the 1000 block, the pickup crossed the median and collided with a privately owned vehicle driven south by an off-duty Indianapolis Metropolitan Police Department officer.

The pickup driver was pronounced dead at the scene. His passenger was transported to a hospital with injuries whose severity was not disclosed. The off-duty officer was also hospitalized with injuries of undisclosed severity.

On 04/22/2021 beginning at approximately 7:15 a.m., police received repeated reports of Cory Marcus Orsenico, 32, driving a red Mitsubishi Eclipse dangerously north on U.S. Highway 101 from the Port Orford area. Over the following hour, callers reported Orsenico speeding, swerving through traffic, passing on curves, making sudden U-turns, cutting off vehicles and semi-trucks, running red lights without slowing and traveling more than 60 mph through a Bandon school zone. Multiple law-enforcement agencies attempted to locate him.

Near mile marker 247 by Belloni Boys Ranch, an Oregon State Police trooper traveling south encountered Orsenico traveling north. The trooper turned around to follow him, and the patrol vehicle and Mitsubishi made minor contact during the maneuver. Orsenico then accelerated away southbound.

Investigators determined that Orsenico moved from the slow lane across the three-lane highway, crossed the double-yellow centerline and intentionally drove head-on into an oncoming vehicle driven by Dawn Adams, 54, with Jeffrey Adams, 78, as her passenger. On 04/21/2021, Shynia Jones, 18, of Grand Rapids was driving south on Division Avenue when a Wyoming Department of Public Safety officer saw her vehicle near 44th Street. The car had no license plate and police believed it matched the description of a vehicle reported stolen earlier that night. The officer attempted to stop Jones, but she continued south and ran a red light at Division Avenue and 36th Street at high speed. Police subsequently determined that Jones's vehicle was not the stolen vehicle they were looking for.

Jones temporarily eluded police. Another Wyoming officer subsequently encountered the vehicle farther north on Division Avenue and followed it as it traveled at approximately 80 to 90 mph. Police reported that the officer discontinued the pursuit shortly before the fatal crash.

Near Division Avenue and Alger Street, Jones's vehicle collided with another vehicle, struck the boulevard median at high speed, became airborne and went out of control before crashing into a light pole. On 04/15/2021, Stephen B. Swears, 56, of Boynton Beach, Florida, was traveling north through South Carolina in a 2013 Ford Mustang. Law enforcement notified St. George police that the U.S. Marshals Service was looking for Swears in connection with a kidnapping charge from West Palm Beach, Florida. Authorities reported that he had already passed Yemassee and was traveling north toward Walterboro.

A St. George police officer located Swears on Interstate 95 near mile marker 74. Police attempted to stop the Mustang shortly before Exit 82, and Swears continued driving. The pursuit lasted approximately 20 minutes and involved the St. George police chief and two St. George police officers, traveling into Orangeburg County and then back into Dorchester County on U.S. Highway 15.

On Highway 15 near Fox Run Road, police said Swears' southbound Mustang ran off the roadway, lost control, crossed into the northbound lanes and collided head-on with a 2012 Ford Fusion. Swears was wearing a seat belt and was pronounced dead at the scene at approximately 1:30 p.m.

On 04/15/2021 at approximately 3:45 a.m., Roger Fuentes, 50, was lying in the roadway at Colorado Street and West 13th Street, immediately west of the Texas State Capitol in downtown Austin. Fuentes' last known address was in Taft, Texas. A Texas Department of Public Safety trooper was patrolling the Capitol area northbound on Colorado Street and turned left onto West 13th Street, striking and running over Fuentes. DPS said it was not known why Fuentes was lying in the roadway.

On 04/15/2021 at approximately 1:53 a.m., Oliver Sevin Frazier-Savoy, 24, of Beaverton was walking near Southwest Murray Boulevard and Southwest Bowerman Drive, adjacent to Nike World Headquarters, when he was struck by a Washington County Sheriff’s Office patrol vehicle traveling south on Murray Boulevard. Beaverton police were called to investigate because a Washington County deputy was driving the vehicle.

Police reported that the deputy had a green traffic signal and investigators did not initially believe the patrol vehicle was speeding. Frazier-Savoy suffered fatal injuries. The deputy, responding Beaverton officers and paramedics attempted lifesaving measures, but Frazier-Savoy was pronounced dead at the scene.

NARRATIVE:
On 04/18/2024 at approximately 6:00 a.m., Nicholus “Nick” Wayne Woolums, 25, of Muncie was a passenger in a Dodge Dart traveling north on State Road 3 in Henry County. Henry County Sheriff’s Deputy Tanner Strelecky, who had just completed his shift but remained in uniform and was driving his marked patrol vehicle, reported seeing traffic violations that led him to suspect the driver was impaired. Strelecky attempted a traffic stop near the Henry-Delaware County line and the driver continued north.

Strelecky pursued the Dodge into Delaware County and through Muncie. Investigative records later described speeds of approximately 70 to 100 mph as the pursuit traveled through the south side of Muncie, downtown, the west and northwest sides, and eventually east on McGalliard Road.

At approximately 6:15 a.m., the Dodge approached a red light at McGalliard Road and Tillotson Avenue. The driver moved into the left-turn lane to pass vehicles stopped for the signal and entered the intersection, where the Dodge collided with a white Jeep driven by 38-year-old Shanta Sharrel Schmiesing. Both vehicles came to rest in a grassy area northwest of the intersection.

Woolums was pronounced dead at the scene. The Dodge driver and Schmiesing were both transported to IU Health Ball Memorial Hospital in critical condition; Schmiesing was subsequently transferred to IU Health Methodist Hospital in Indianapolis. Muncie police did not participate in the pursuit but investigated the crash with Indiana State Police.

Woolums was remembered by his family as outgoing, kind and intensely family-oriented. His obituary said he routinely put other people before himself, was an Indianapolis Colts fan and particularly cherished time with his father, whom he considered his best friend.

NARRATIVE:
On 04/14/2021 at approximately 1:47 a.m., Alyssa Roman, 19, of San Jose was a passenger in a 2004 Toyota Camry traveling south on Highway 1 with three other young people from San Jose. California Highway Patrol officers first observed the Camry traveling at high speed north of Dimeo Lane and attempted a traffic stop. The driver continued south toward Santa Cruz while CHP pursued.

The Camry continued at high speed onto Mission Street and through Santa Cruz. Additional CHP units encountered the vehicle near Highway 1 and Highway 9 and again attempted to stop it. Santa Cruz police were also involved as the pursuit continued south on Highway 1.

Near Morrissey Boulevard, the driver attempted to take the off-ramp but struck a concrete curb, causing the Camry to pitch forward and crash into a metal guardrail. Approximately 20 feet of guardrail was damaged, and the wrecked Camry caught fire.

CHP officers pulled Roman and another passenger from the burning wreckage. Roman was pronounced dead at the scene. Passenger Kenny Tran, 20, and a 17-year-old female passenger were injured and hospitalized; later reporting described their injuries as minor. The driver, Richard A. Quihuis Jr., 19, suffered minor injuries and ran from the wreck before a CHP officer apprehended him.

On 04/10/2021, Chassadee Chy Ann Costales, 22, of Springdale was driving a Toyota 4Runner in Tontitown when Officer Rebecca Martis checked the vehicle's license plate and learned that the SUV had been reported stolen in Fayetteville approximately one month earlier. Martis attempted a traffic stop, and Costales continued driving while police pursued her onto Bush Creek Road.

Costales failed to negotiate a sharp turn on Bush Creek Road and crashed the 4Runner into a tree. The vehicle erupted in flames. Witness James Larson, who lived nearby, described seeing a large fireball and went to the crash site to help as neighbors arrived with fire extinguishers.

Officer Garrett Henry climbed into the burning vehicle to reach Costales while Martis fought the fire with an extinguisher. Henry suffered minor injuries during the rescue. The officers and responding residents succeeded in pulling Costales from the wreckage. She suffered third- and fourth-degree burns and multiple broken bones and was hospitalized in critical condition.

Costales died from her injuries on 04/13/2021, three days after the crash. She was born in Galveston, Texas, and had participated in marching band and wrestling in high school. On 04/13/2021 at approximately 10:20 p.m., Cam-Ron P.M. Poelinitz, 19, of Fitchburg was driving south on U.S. Highway 51 with four passengers when a McFarland Police Department officer attempted to stop the vehicle for speeding. Poelinitz continued driving and exited Highway 51 onto Siggelkow Road while the officer followed with emergency lights activated.

The officer terminated the pursuit on Siggelkow Road. Shortly afterward, while traveling east, the officer passed Poelinitz's vehicle accelerating westbound. The officer turned around and headed back in its direction but found that the vehicle had already crashed into a house in the 5400 block of Siggelkow Road near Dream Lane.

The impact drove the vehicle into the residence. Poelinitz suffered fatal injuries and was pronounced dead at the scene. All four passengers were injured and transported to a hospital for treatment. No one inside the house was injured.

Poelinitz's mother publicly identified her son before the Dane County Medical Examiner's Office formally released his name. A forensic autopsy determined that he died from injuries sustained in the crash.

Because a McFarland police officer had attempted the stop and pursuit immediately before the fatal crash, McFarland Police requested an outside investigation. On 04/12/2021 at approximately 1:36 a.m., Angelina Sue Raad, 38, of Winchester was driving on New Circle Road in Lexington. Police had received multiple 911 calls reporting a vehicle traveling the wrong direction on the inner loop near Harrodsburg Road. Raad's vehicle subsequently collided with a Lexington Police Department cruiser driven by Officer Dawn Miller between Harrodsburg Road and Nicholasville Road. Police specifically stated that Miller was not responding to the wrong-way-driver calls when the collision occurred; later reporting established that Miller was driving home after completing her shift.

The collision severely damaged both vehicles and trapped both drivers. Lexington firefighters extricated Raad and Miller. Raad was transported to the University of Kentucky Medical Center, where she was pronounced dead at 3:49 a.m. Miller suffered critical injuries and underwent multiple surgeries.

On 04/11/2021 at approximately 4:00 a.m., Chouteau police attempted to stop a pickup truck reportedly connected to a larceny investigation. The male driver continued away and police pursued, beginning a lengthy multi-jurisdictional chase through northeastern Oklahoma.

The pursuit traveled through several communities and counties, including the Wagoner, Pryor, Okay and Fort Gibson areas, as officers from additional law-enforcement agencies joined or assisted. During the pursuit, police reported that someone inside the pickup fired multiple gunshots toward pursuing officers. Officers returned fire during the chase.

On 04/08/2021 at approximately 3:50 p.m., a male driver was traveling north on Iroquois Road in Apple Valley while police pursued his sedan.

As the pursuit reached Yucca Loma Road, the driver entered the intersection without stopping at the stop sign. A woman driving an SUV on Yucca Loma Road struck the sedan in the intersection. The impact sent the sedan airborne and flipping before it landed in the front yard of a residence at 13721 Iroquois Road.

The male driver was trapped inside the severely damaged sedan. Firefighters and paramedics worked at the vehicle for more than 30 minutes, but he died at the scene. The fatal crash record documents two vehicles and one fatality.

The woman driving the SUV was trapped in her vehicle and had to be extricated by firefighters. Off-duty Illinois Capitol Police investigator Robert Milton Hodson was driving north in a Jeep Wrangler when he attempted to pass another vehicle, crossed into their lane and struck the Ford head-on.

Dudra and Shoot were pronounced dead at the scene. Thomas survived with life-threatening injuries that left her in a wheelchair before she eventually relearned to walk. Hodson suffered serious but non-life-threatening injuries.

The Illinois State Police reconstruction determined that Hodson was traveling approximately 88 mph at impact. Shoot had applied the brakes and was traveling approximately 30–35 mph. Hodson declined a breath test at the scene, and subsequent testing recorded a blood-alcohol concentration of .353 and later .299.

Hodson was arrested after his hospital release and removed from his Capitol Police position without pay. He ultimately pleaded guilty to aggravated DUI resulting in death.
